The infection rate of female patients was higher than that of male patients(P<0. 10). Urinary tract was the most important infected site (35. 1 % ), following gasterointestinal,respiratory tract,blood and wound successively. Most of the infections(86. 4%) occured within 2 months after transplantation. We think this interval may be a high risk period. Among 37 of positive specimens,73% were caused by Candida,27% by yieast. Candida Albicans was the most common pathogen. 40. 9% of 22 patiens multi-infected by fungus, virus and bacteria. The risk factor of susceptible to fungal infections of transplant patients include:①long and larger amount application of broad-spectrum antibiotics;②rejection and immuno-suppresive therapy;③ the longer the preservation of urinary catheter and support catheter, the more succeptible to infections;④surgical complications such as leak of urine and bleeding of wound;⑤ decreased peripheral WBC;⑥diabetes which included diabetes caused by using steriod; ⑦protein malnutrition and so on.

129例肾移值患者,术后真菌感染率为17.1%,死亡率为18.1%。感染率随年龄增长而增高,女性略高于男性(P<0.10),尿路感染占首位(35.1%)。真菌感染绝大多数(86.4%)发生在术后2个月以内,故认为此期为肾移植术后真菌感染危险期。肾移植术后易引起真菌感染的危险因素包括:①长期大量应用广谱抗生素;②排异反应和免疫抑制剂的治疗;③术后保留导尿管和支架管;④尿漏及伤口出血等外科并发症;⑤末梢白细胞减少;⑥糖尿病包括并发类固醇性糖尿病;⑦蛋白质营养不良等。

Eight cases of fungal pyelonephritis after surgical treatment for complicated nephrolithiasis are reported. The patients had had a long history of using antibiotic and the renal function was deteriorated.To diagnose early, it is importent to pay attention to fungal infection of the urinary tract.Early and adequate treatment for fungal pyelonephritis enables the patients to recover in a short time.The treatment for fungal pyelonephritis was descussed.

报告8例复杂肾结石术后发生真菌性肾盂肾炎病例。认为长期应用广谱抗生素及患者肾功能减退是导致该病的重要发病因素。重视真菌感染的发生应做到早期诊断,及时及足够时间的抗真菌治疗可缩短治疗疗程。对抗真菌药物的作用及不同用药途径的治疗效果进行探讨。

All patients experienced the regular antifungal treatment with miconazole or diflucan for 7~17 days. Infection was controlled and cured in 9 patients.One patient died of fungal septicemia. This results showed that attention should be given to systemic fumgal infection in renal transplantation, and diflucan was considered as the first choice of treatment.

1991年1月~1993年12月我所331例肾移植患者中发生深部真菌感染10例,其中9例为念珠菌感染,1例为曲霉菌感染。经咪康唑或大扶康的正规治疗后,除1例发展为真菌性败血症死亡外,余9例感染治愈,但有3例移植肾因此丧失功能。我们认为肾移植术后应重视深部真菌感染问题,抗真菌药物以大扶康为首选。
