Origin of the right pulmonary artery from the ascending aorta is a rare congenital anomaly and has been associated with a high surgical mortality. This paper reports a case which was successfully repaired by direct anastomosis of right pulmonary artery to main pulmonary artery under cardiopulmo-nary bypass combined with moderate hypothermia. In addition, embryology, pathophysiology. clinical feetures and surgical technique of this anomaly are discussed.

At 30, 50 seconds during bypass there is significant difference of ACT in the first group in comparing with second group) p<0.05) .It suggested that not only the individual metabolism of heparin be guite different but hypothermia can also affect ACT during cardiopulmonary bypass. The ratio of 1:1.064±0.18 was given for protamine sulfate to neutrilize heparin post bypass. In our study the dose of protamine sulfate was less than other reports.

作者自1982年6月至1984年12月应用ACT监测了121例体外循环心内直视手术病人,体外循环中中度低温组107例,平均鼻腔温度29℃,深低温组14例23℃,测量结果肝素化前,深低温组和中度低温组ACT均在正常围范内。肝素化后10分钟达53℃左右,两组比较无差别,转机30分钟,50分钟,两组比较有明显差别(p<0.05)显示肝素代谢不但个体差异颇大,体外循环中降温对ACT也有一定影响。术后两组用鱼精蛋白中和肝素,中和比为1:1.064±0.18,手术野可见凝血块,未见肝素反跳现象,效果满意,鱼精蛋白用量比一些作者报道的低。

The present study suggested that the profound hypothermia to the complement system play protective effects.
