This paper deals with three cases of long jumpers suffering from fracture at middle one-third of the femoral shaft. X-ray shows a transverse fracture and angulating deformity at middle one-third femoral shaft in all cases.Two cases...

This paper deals with three cases of long jumpers suffering from fracture at middle one-third of the femoral shaft. This kind of fracture is transverse and produced by indirect force at the landing moment. This is a characteristic fracture which we call a jumping femoral fracture.Physical examination reveals swelling with tenderness and deformity of the injured thigh. X-ray shows a transverse fracture and angulating deformity at middle one-third femoral shaft in all cases.Two cases were performed by open reduction and internal fixation with Küntschcr or medullary nail, and one case was performed by traction through the intercondylus of the lower femur. All patients got good result after one year, but open reduction was better than skeletal traction.According to clinical observations, CT examination of the femur in normal person and experiment of the femoral shaft with fresh cadaver, the mechanism is based on the imbalance of the body rising high into the air, which produces both scissors stress and rotational force at landing. In the meanwhile the posterior group muscles of the thigh were more likely to be relaxed reflectively, leaving the femoral shaft unprotected.

本文报告因跳远训练导致股骨干骨折3例。通过临床观察,正常人股骨干CT值检查,以及新鲜尸体股骨干破坏实验,认为该骨折的发生原理是在跳远运动腾空时,身体失去平衡,在落地的瞬间,一足首先固定于沙坑,躯干剧烈扭转,从而产生剪应力和旋转暴力结合于股骨中部,同时大腿后群肌肉反射性松弛,失去对股骨干的保护作用。近年来我们遇到跳远运动所引起的股骨中1/3处横形或斜横形骨折3例,从发生原因,骨折部位以骨折形状均极相似,我们认为是一种特殊类形的骨折,为了探讨其发病机理,除了临床观察以外,还在正常运动员股骨应用CT检查,以及新解尸体股骨进行CT检查和力学破坏试验。

Having collected the skull of a fallen adult victim,and made artificial fractures on an autopsled adult skull as well as on two living dog skulls,we observed the edges of skull fractures under the scanning electron microscope in order to find out the submicroscopic morphologic features of skull fractures aiming at elucidating the mechanism of fracture,and differentiating ante-from post-mortem fractures.Many microfractures were found along the main fracture lines,they extended into the interior of the skull. The width of the microfractures varied from 5 to 100μm.Some of them were located in the external compact bone and some between the outer compact bone and diploe. The rest were found in the diploe in the form of transverse,longitudinal or spiral fractures of bony trabeculae.Collagenous fibre bunches embeded in the bony matrix were broken and separated. The blood vessels inside the skull were crosscut or longitudinary torn by the fracture lines. Sometimes the torn blood vessels were pulled out of the Harversian canals.In antemortem fractures as a results of hemorrhage, there were not only blood clot on the edges of the broken skull, but also hemorrhage into the microfractures. The fibrin networks and red blood cells were easily found in the depths of these cracks.A deductive mechanism of formation of the microfractures and the possibility of differentiation between ante-and post-mortem fractures are discussed.

作者利用扫描电子显微镜,对颅骨骨折断面上微细结构的形态学特征进行了观察与研究,发现在颅骨骨折断面上有微细的骨折裂线,向断面深部的骨质中延伸,其宽度为5至100um不等,主要分布在颅骨外板内,或外板与板障的交界处.分布于板障内的微细裂线可造成骨小梁横行、纵行和螺旋形骨折.骨折时,骨基质中的胶原纤维束可被分离或断裂,各胶原纤维层可分开.骨内血管可被骨折裂线横断或纵形撕裂,损伤的血管可被从哈佛氏管中拉出.生前骨折时,在微细裂线的深方可清楚见到纤维蛋白网和红细胞相互粘集,死后骨折则未见到此现象.本文还探讨了颅骨骨折发生的机理和推断生前骨折的可能性.

70 case reports are presented. An analysis is made on cranial base contre-coup opposite to the point of impact. This may take place as a result of either deceleration or acceleration impacts on the skull, more frequently on cranial fossa anterior,less on cranial fossa middle and seldom on cranial fossa posterior. Serious contre-coup contusions of the brain may be in correspondence with the contre-coup fractures caused by the brain tissue accelerated on the cranial base,hence,a result of interaction. Others are mainly at ributed to the supportive inertia exerted by the cranial base.

作者对70例头部损伤引起的对冲性颅底骨折进行了分析.认为在骨质菲薄部位,如眶板、筛板及蝶骨翼等处出现孤立性线状或粉碎性骨折,系由于头部受钝性暴力作用,于着力点的对侧被眼球或脑的加速运动撞击造成;在对冲性颅底骨折的相应部位的脑组织出现严重的对冲性脑挫伤,系由于该部脑组织加速运动撞击形成;二者是相互作用的结果.其它部位的对冲性颅底骨折主要是由于颅底部突出部分的反作用力造成.
