Objective To evaluate the caries preventing effect of slow-release fluoride tablet in vivo.Methods Seven subjects with healthy gum and DMFT=0 were selected.Each wore two acrylic resin bases in control and test stage (4 weeks each).One base was placed on palatal surfaces of right maxillary molars,the other on buccal surfaces of left mandibular molars.Each base had a normal enamel cube and an enamel cube with early artificial caries.A fluoride tablet was adhered on the mucosa surface of maxillary anterior vestibule after each supper in the test stage.After experiment,microhardness values,sediment on enamel surface and histology changes of normal enamel treated by acid gel for 48 hours after experiment were examined.Results The increased microhardness values in the test stage (maxillary: 73.9 ± 6.3 kg/mm 2 mandibular: 35.1 ± 8.9 kg/mm 2,P< 0.5 ) were bigger than the control stage (maxillary: 55.9 ± 8.3 kg/mm 2,mandibular: 31.1 ± 14.5 kg/mm 2,P> 0.5 ).Irregular sediments were observed on enamel surfaces,and were rough and sparse in the control stage but small and compact in the test stage.There were an obvious surface layer and a wide subsurface demineralization layer in enamel of the control stage.Conclusion Slow-release fluoride tablet could enhance saliva's remineralization effect and improve the caries resistance of enamel.

目的 评价缓释氟粘贴片体内防治龋齿的作用。方法 受试者 7例 ,无氟期 (唾液单独作用 )和有氟期 (唾液与氟片共同作用 )戴右侧上颌舌侧和左侧下颌颊侧装置各一 ,每装置有正常和 4小时人工龋釉质标本各 1块。有氟期氟片置于上颌中切牙前庭沟处的粘膜表面 ,每天一次 ,一次一片。实验后将正常釉质块放入 3 7℃酸胶中 4 8小时。观察两期人工龋表面硬度值的改变、表面沉积物的情况和实验后釉质抗酸能力的变化。结果 有氟期人工龋表面硬度值增加量 (上下颌分别为 73 .9± 6.3kg/mm2 和 3 5.1± 8.9kg/mm2 ,P <0 .5)高于无氟期 (上下颌分别为 55.9± 8.3kg/mm2 和 3 1 .1± 1 4.5kg/mm2 ,P >0 .5) ,上颌硬度值增加最多 ,P <0 .5。有氟期人工龋釉质表面有小而致密的矿物盐沉积 ,无氟期的沉积物大而疏松。有氟期正常釉质经酸处理后脱矿程度低于无氟期。结论 缓释氟粘贴片能提高唾液对早期龋的再矿化作用 ,增进正常釉质的抗酸能力。

Objective To investigate the effect of pulse Nd:YAG laser combined with NaF on caries resistance of human enamel in vitro.Methods 24 caries-free, constant premolares were cut into enamel mass. Surface ultrastructure and fluoride distribution of enamel in laser-NaF treated group, laser treated group, NaF treated group and control group were observed by scanning electron microscope (SEM) and electron probe microanalyzer.Results ①SEM observation: In laser-NaF treated group, the smear layer was almost removed, the enamel surface showed partially melted-like and bubble-like appearance, diffuse white-small particles distributed on enamel surface, and melted position, microcrack and microhole appeared whitely blocking-reflection. ②Electron probe microanalyzer analysis: In enamel samples, the highest average Ci of F+- and the hightest depth which F+- entered in enamel were recorded in the laser-NaF treated group. The average Ci of F+- in laser-NaF treated group was significantly increased compared with that in NaF-treated group or laser-treated group or control group (P<0.001).Conclusion The pulsed Nd:YAG laser combined with sodium fluoride could make the enamel surface partially melt, change the structure of the crystal, enhance the absorption of fluoride, and clear smear layer. This technology may resist the caries.

目的 探索脉冲Nd :YAG激光结合氟化钠使用的防龋机制。方法 选择 2 4颗健康离体人恒前磨牙制成牙釉质标本。扫描电镜 (SEM)、电子探针分别观察和分析激光加氟化钠组、激光组、氟化钠组、空白组 (不处理 )牙釉质表面超微结构及牙釉质内部氟离子分布水平。结果 ①SEM观察 :激光加氟化钠组牙釉质表面玷污物全部去除 ,并呈部分熔融状和空泡状改变 ;牙釉质表面散在分布着细小的白色颗粒 ;熔融灶、裂纹、微孔呈白色高度阻射。②电子探针分析 :激光加氟化钠组中氟离子平均重量百分比值增加明显 ,5 μm处氟离子平均重量百分比值最高 ,明显高于空白组、激光组和氟化钠组 (P <0 0 0 1) ;氟离子进入牙釉质的有效深度约 15 μm ,高于氟化钠组的 5 μm。 结论 脉冲Nd :YAG激光结合氟化钠作用后引起牙釉质表面部分熔融、改变牙釉质晶体结构、促进牙釉质对氟离子的吸收、去除牙面玷污层 ,因而具有防龋作用。
