Becouse implant materials are fixed firmly with alveolar bone and gingival tissue,implant substance can not be moved.Improved ridge height,width and form were satisfactory.Since alveolar ridge was. augmentated, denture retention increased significantly and the function of the denture improved.Particle bioactive glass-ceramic currently have been considered as a quite ideal artificial bone.The procedure of alveolar ridge augmentation is simple with little injury and good effects.

作者应用本研究组研制的颗粒型生物活性玻璃陶瓷,对15例牙槽骨严重萎缩患者作了牙槽骨增高术并获得成功。患者于句麻下形成牙槽骨骨膜下隧道,用特制注射器将种植材料填入隧道内,关闭创口后行牙槽嵴塑形,使形态、大小适度。术后1.5月作义齿修复。经临床初步应用表明:所用种植材料理化性能稳定,生物相容性好,与种植床牙槽骨、龈粘膜结合牢固,能形成高度、宽度、形态较好的牙槽嵴。由于牙槽骨的增大,义齿固位力及稳定性显著增加、功能改善。颗粒型活性玻璃陶瓷确属目前较理想的种植材料。该手术操作简单,创伤小,效果肯定。此外,文章还对术前、术中、及术后等有关问题作了详尽的讨论。

The male health rabbits were used in this study.The three implanting mat-erials were zirconium hydroxyapatite ceramic implant,the bioactive glass cera-mic implant,and the titanium alloy Tc4 implant.The later was used as a control.All the materials were implanted into the lumbodorsal muscles of the rabbits.The muscle samples derived from the 7th day,30th day and the 90th day of the experiment for the observation of pathology and enzyme histochemistry.In the 7th day,a number of necrotic muscle fibers,inflammatory ceils and an active hyperplasia of fibroblast cells were seen in all the implanting areas,but at the titanium alloy Tc4 implanting areas there were less fibroblast cells.A decreased activity of the enzymes of metochondria and a increased activity of hyperplasia of connective tissue surrounding the three implants were observed,of which the titanium alloy Tc4 implanting areas were more considerable.After 1~3 months,the necrotic muscle fibers had been absorbed,the inf-lammatory ceils did not notable and the connective tissue had become scart.At the surface of the connective tissue,surrounding the two ceramic implant appeared an adaptive synovial metaplasia of connective cells.But in the surface of the connective tissue surrounding the titanium alloy Tc4 implant,there were only a number of foreign body giant cells.A normal activity of the enzyme of metochondria that connective tissue surrounding the two ceramic implant were been seen,but that of the titanium alloy Tc4 implanting area is lower than normal tissue.A increased activity of all enzyme in the synovial metaplasia cells had been observed.It suggested that the two bioceramic implants have better biocompatibility than titanium alloy Tc4.

本文对两种生物陶瓷材料—锆磷灰石陶瓷、活性玻璃陶瓷种植体植入兔子腰背肌内,种植体对周围组织的影响进行了一般组织学及酶组织化学的综合观察,并以钛合金Tc_4种植体作为对照。结果发现,两种生物陶瓷种植体对周围组织中酶活性的影响随种植体植入时间的延长而减弱,实验三个月时,基本恢复正常。种植术后一个月,两种生物陶瓷周围出现滑膜样细胞化生反应,是一种适应性反应,对种植体的组织相容性有一定积极意义。钛合金 Tc_4种植体的组织相容性次于两种生物陶瓷。

The dense bioactive ceramic(bioactive glass ceramic and zirconium hydro-xylapatite ceramic)made in the research laboratory has been used as immediate root-shaped implants in a total of 139 implants among 68 cases.Teeth were extracted under local anesthesia.Mucoperiosteal flaps were prepared and artificial dental roots were selected according to the preoperative x-ray films and the shapes of roots of the extracted teeth.Then,the artificial roots were buried into the sockets.These roots should be placed 2mm below the alveolar ridgo so that a clot could be formed at the top of the implant.After the operation,all implants have been made clinical and radiographical observations regularly and repeatedly.The results in this study showed that these materials have good biocompatibility. Three to six months later the new bone was obviously formed on the surface of the implant which was firmly bonded with the alveolar bone.It is obvious that no change in the shapes of the implants and a slight absorption of the alveolar ridge could be found.The operation is simple and the effect is excellent.

作者采用本组研制的致密型生物活性陶瓷(生物活性玻璃陶瓷和锆——羟基磷灰石陶瓷)作立即牙根种植共68例、139颗。患者在局麻下拔牙,形成粘骨膜瓣,根据离体牙根及X线牙片选择人工牙根,植入牙槽窝内,颌端位于牙槽骨缘下2mm,便于血凝块形成。术后定期作临床及x线片观察,结果表明:该材料的生物相容性好,3~6个月后人工牙根周围有明显的新骨形成,种植体与牙槽骨的界面结合牢固,人工牙根稳定且无形状改变,牙槽嵴无明显吸收,故提高了义齿修复的效果。本法操作简单,效果显著。
