The subgingival microflora of 38 adults with normai gingiva, gingivitis, and chronic periodontitis was analyzed respectively in 4 bacterial morphtypes (spirochetes, motile rods, cocci cells+straight rods and others).The results indicated that spirochetes and motiles appeared at significant higber percentage at gingivitis and periodontitis sites than at healthy sites. The subgingival microflora of 38 adults with normai gingiva, gingivitis, and chronic periodontitis was analyzed respectively in 4 bacterial morphtypes (spirochetes, motile rods, cocci cells+straight rods and others).The results indicated that spirochetes and motiles appeared at significant higber percentage at gingivitis and periodontitis sites than at healthy sites. Significant differences among the mean percentages of 4 morphtypes at different groups were found by analysis of variance, except for that of motile rods and cocci cells + straight rods between gingivitis and periodontitis. Wide range of variability in each morphotype percentage was observed, so it is unlikely to establish the microbial threshold Ievel for diagnosis.Significant positive correlations(R's) were found between the percentages of spirochetes and P1I, BI and PD, whereas negative correlations between the percentages of cocci cells + straight rods and BI, PD were also detected. Meanwhile, it was also found that many technical aspects of Darkfield microscopy should be improved.

本文对38例不同临床诊断的四组受检者龈下菌丛的暗视野显微镜观察表明,病变部位龈下螺旋体及能动菌数高于健康牙周。健康牙周龈下螺旋体为3.4%。健康与病变处各类菌分布的变异较大,提示单独作为牙周健康与否的诊断指标不准确。相关分析表明螺旋体数与临床指数值间呈正相关,球菌与BI、PD间呈负相关;袋深相同而病变性质不同的龈下螺旋体分布并无显著差异。

本文用 S-100蛋白和溶菌酶抗体的免疫过氧化物酶技术,对10例正常人和20例成人慢性牙周炎龈上皮中郎格罕氏细胞进行鉴定和计数观察。结果表明:牙周炎时龈上皮中郎格罕氏细胞的数目明显高于正常龈上皮(P<0.01),且随炎症细胞浸润程度的加重和菌斑指数记分值的升高而增多,呈显著正相关(P<0.05)。

对17例慢性牙周炎患者牙龈组织基膜进行电镜观察发现,随着病情加重基膜出现增生,断裂,分离及多层化等不同程度的改变,与基膜关系密切的微原纤维亦称耐酸纤维,同时严重受损并形成颗粒样物,沉集在基膜附近,在基膜断裂之穴口中可见有胶原纤维嵌入。这样既破坏了基膜的防御屏障,也阻碍了牙周其它正常组织的修复,从而进一步加深了牙周病的发展。
