    AbstractWe designed lumbar spondylolysis and spondy-lolisthesis single segment reduction fixation system ac-cording to neural arch measured in the 46 dried speci-men. By Henderson standard,excellent resultwas moted in 13 patients, good in 1 and fair in 1. Weconclude that LSRF has good reduction and rigid fixa-tion and it is a new technique for lumbar spondylolysisand spondylolisthesis.

    作者以国人46具干燥骨标本椎弓的测量结果为依据,自行设计了腰椎崩裂滑脱单节段复位固定系统(LSRF)。生物力学测试结果表明:LSRF固定强度为Hefti钩螺钉的1/6倍(P<0.05),Salib弓根螺钉棘突钢丝的2.7倍。LSRF以椎板钩和椎板接触点为支点,通过杠杆及椎弓根螺钉提拉作用,使滑椎复位。临床应用18例,术前椎体滑移26.67%,滑脱间盘高度14.94mm;术后滑移3.38%,间盘高度17.08mm。15例平均随访12个月,按Henderson标准评定,优13例,良1例,可1例。提示LSRF既有良好复位又坚强固定作用,为椎弓崩裂滑脱治疗增添了新的方法。

Methods: From March 1991 to May 1996, 33 cases of instability of the lumbar spine were treated with the device, of them, 14 were lumbar spondylolisthesis with disruption of bilateral vertebral arches, 3 were lumbar disc herniation with disruption of unilateral vertebral arch (without spondylolisthesis),...

    Objective: To prevent operative failure due to malunion of bone grafting, a spiral typed fusion device was designed for the treatment of lumbar instability through posterior intervertebral fusion. Methods: From March 1991 to May 1996, 33 cases of instability of the lumbar spine were treated with the device, of them, 14 were lumbar spondylolisthesis with disruption of bilateral vertebral arches, 3 were lumbar disc herniation with disruption of unilateral vertebral arch (without spondylolisthesis), and 16 were degenerative vertebral spondylolisthesis. Results: The duration of follow up was 1- 5 years. The rate of bony fusion was 100% . Symptoms and signs disappeared completely and no complication occured. Conclusion: The authors believed that this kind of technique was easy to manipulate, safe and reliable. It played an important role in maintaining the stability of vertebral column.

    目的:通过应用自行研制的螺旋式椎体融合器经后路椎间植骨融合治疗腰椎不稳,以防止因植骨愈合不良而造成的手术失败。方法:1991年3月~1996年5月应用螺旋式椎体融合器植骨融合治疗腰椎不稳33例。其中双侧椎弓断裂伴腰椎滑脱14例,一侧椎弓断裂(无滑脱)伴腰椎间盘突出3例,退变性脊柱滑脱16例。结果:经1~5年随访,本组病例全部骨性愈合,症状基本消失,无并发症发生。结论:本技术操作简便、骨愈合坚固、能早期下床活动、安全可靠,对保持脊柱长期稳定、巩固疗效有重要作用

    Methods:A new internal fixation apparatus—vertebral “U”rod Pedicle Screw for treatment of spondylolisthesis was designed by the authors, and used for treatment of lumbar spondylolisthesis.Results:Its biomechanical,axial rigidity,and torsional rigidity increased 20%,and 16% respectively over that of Steffee plate.It had been used in 11 patients.The results were good.The reduction rate was 80%.The operation had the advantages of less trauma,time consuming and bleeding.

    方法:对一种新型内固定器械———U型棒椎弓根钉,进行生物力学实验,并用以治疗腰椎滑脱症。结果:以Stefee钢板相比轴向刚度增加20%,扭转刚度增加16%。经临床应用11例,效果满意,复位率达80%,手术创伤轻、出血少、手术时间短。
