In this biomechanic study, the authors measured the axial corrective forces exerted by the Edwards rodsleeve (ERSIC) and Harrington rod (HDR). Fifteen specimens with anterior and middle columns disrupted and posterior column intact were made from six fresh, frozen swine thoracic and lumbar spines. Force transducer equipped with strain gauge was fixed anteriorly at vertebral body spanning the disrupted site. Each specimen was instrumented unilaterally with one of the devices. In this biomechanic study, the authors measured the axial corrective forces exerted by the Edwards rodsleeve (ERSIC) and Harrington rod (HDR). Fifteen specimens with anterior and middle columns disrupted and posterior column intact were made from six fresh, frozen swine thoracic and lumbar spines. Force transducer equipped with strain gauge was fixed anteriorly at vertebral body spanning the disrupted site. Each specimen was instrumented unilaterally with one of the devices. The Harrington hooks were placed one level above and below the disrupted site under laminae. Following rod insertion, sleeve was removed along the rod using a spreader until it was centered over the disrupted site. Specimens were kept moist during testing. The axial corrective forces applied with ERSIC were 89.0N which were 82.7% higher than 48.7N with HDR(P<0. 0005). Biomechanic analysis showed that the axial corrective forces are correlated directly to the anterior horizontal forces. The axial corrective forces may be increased significantly when the horizontal forces are raised. The rod-sleeve contributes anterior horizontsl forces much more than straight distraction rod. Authors believe that ERSIC is an ideal internal fixative device for the treatment of thoracic and lumbar spinal unstable injury for its reduction ability and stabilizing effect.

本文报告Edwards套棒装置及单纯Harrington分离棒治疗胸腰椎不稳定损伤的力学实验研究结果。取成年猪新鲜胸腰椎脊柱6具,切断前纵韧带、椎间盘纤维环、后纵韧带,造成前中柱不稳定损伤15处。测力传感器固定于椎体前缘并骑跨损伤处,使之检测纵向复位力。单侧内固定装置固定。实验结果表明:单侧Edwards套棒装置产生纵向复位力量89N,较Harrington分离棒48.7N提高82.7%(P<0.0005);二种内固定装置分别用于胸、腰椎时,纵向复位力均无显著差异(P<0.05),表明胸段肋骨对纵向复位力无影响。生物力学分析,纵向复位力与横向前推力成正比例关系,增大内固定装置对伤椎的横向前推力,可提高纵向复位力量。同时其它学者也报告Edwards套棒装置具有良好的脊柱三维稳定作用。所以,作者认为Edwards套棒装置集对脊柱骨折复位与稳定于一体,是理想的内固定装置。

They were applied sequentially to each of the cadaver specimen injuried at the T 12 -L 1 levels. Three dimensional movements of the spine were measured in each speciment following application of the fixation devices. The results revealed that rigid fixation in restoring the spinal stability in flexion, extension and lateral bendings were provided by all of the five kinds of posterior internal fixation devices, however, for axial rotations the effects of fixation were not as strong as that of the intact spine. Comparatively speaking, the Dick fixation device achieved better stability in arial rotations.

Distraction forces of 5N and 10N were applied every 3 days to the distal growth plate of the right femur in vivo, in 16 rabbits Four weeks later the length of all distracted femurs was significant longer than both the length of control side and the same one of group ON. Histological study showed hyperplasia of the growth plate, an increase in chondrocytes division rate, and no evidence of epiphyseolysis. The in vitro breaking forces of the growth plate were measured after in vivo distraction. The mean breaking force of the distracted side was not lower than mean values for the undistracted control side. Sixteen femora from 8 rabbits underwent epiphseal distraction in vitro, with 20N, 60N and 100N forces. the mean plastic deformation of the growth plate was great in connection with increasing traction. The subsequence breaking force of these growth plates was no influenced. The results obtained in this study indicated that limb lengthening might be achieved by chondrodiatasis with the proper distraction force and/or a slow distraction rate.

本研究是对新西兰幼兔股骨远端骨骺进行体内和体外牵张实验.16只兔右股骨经5~10N力每3天一次牵张.4周后的股骨长度值明显大于左侧和0N组.形态学变化是受牵张的骺板增厚,软骨细胞增殖且无骨骺分离骨折.体外骺板抗断裂测试显示夸张后的骺板强度不低于未经牵张的对照侧.8只动物16根股骨于体外接受20~100N力的牵张,其骺板的塑性变形度随牵张力值的增高而增大,抗断裂强度亦不低于未经牵张的骨骺.这些结果表明适量的牵张力和/或低牵张速率是实现“软骨分离”或骨延长的关键.
