The aim of this study was to promote an effective approach, which was surgicallyassisted buccal mandibular corticotomy in the lockbite side to rapidly expand mandibular arch to correct unilateral posterior lockbite using the principle of the distraction osteogenesis. The typical subject was a 16yearold patient with unilateral posterior lockbite. The orthodontic treatment had been done before the surgery. After the surgery, reciprocal elastic traction between upper and lower arch were used immediately...

The typical subject was a 16yearold patient with unilateral posterior lockbite. The orthodontic treatment had been done before the surgery. After the surgery, reciprocal elastic traction between upper and lower arch were used immediately in the lockbite side. The results demonstrated that unilateral posterior lockbite could be effectively corrected with unilateral buccal mandibular corticotomy and rapid mandibular expansion, and the stability appeared satisfactory. This article also discussed the indications of the approach and processes of treatment.

利用牵张成骨(distractionosteogenesis)原理,设计了一种新的、可供临床应用的矫正单侧全部后牙锁的正畸-外科联合治疗方法。按设计在下颌第一双尖牙近中、最后磨牙远中及该骨段根尖下3mm作骨皮质和骨松质切开术;术毕即用术前设计制备的口内牵张器矫治牙颌畸形。对2例单侧全部后牙锁的治疗结果表明,锁畸形完全矫正,咀嚼功能恢复正常,疗程短而效果稳定,是一种行之有效的方法。文中还对该方法的适应证、治疗过程和方法等进行了讨论

Objective To evaluate the results of distraction

目的通过应用口内型牵引器行牵引成骨延长下颌骨,矫治小下颌及偏颌畸形,对牵引成骨技术在颌骨畸形矫治中的应用价值和技术手段进行初步的评价。方法8例小下颌患者接受了双侧下颌骨牵引成骨治疗。安置牵引器后第5~6天开始牵引,6例牵引速度为0.8mm/d,2例为1mm/d;牵引到位后固定71~139天拆除牵引器。结果最大牵引幅度24.4mm,最小幅度8.5mm。所有病例在拆除牵引器时均能看到完全成熟的新骨。最长随访期为10个月,无复发倾向。小下颌畸形、颜面不对称畸形均得到了明显的改善。牵引过程中,1例出现颊部软组织感染,1例下唇麻木。未出现牵引器松脱、未成骨及骨过早联合等情况。结论牵引成骨技术因其手术较小、操作简单,可以避免植骨且并发症少,应用于颌骨发育不良的矫正治疗,不失为一种有效的治疗手段;但该技术并不能完全作为单一的矫治手段进行颌面畸形的外科治疗。此外,正确的牵引方向的控制尚有待于进一步研究。

Purpose: To study the effects on development of the dental germs on the osteotomy line during the elongation of mandible by distraction osteogenesis(DO).Methods: There were 8 cases of micrognathia resulted from temporomandibular joint ankylosis received the management of mandibublar elongation by distraction osteogenesis. In 3 of the cases, there were dental germs on the osteotomy line in the mandible. The range of the distraction distance we gained in 3 cases was from 14mm to 22.5mm. Radiographs were taken preoperation and postoperation to observe the development of the dental germs.Results: After a period of follow up(range 7-12 months) we found that all of the dental germs on the osteotomy line developed very well in the enlarged distraction gaps and the teeth erupted from the new bone as normal. There was no significant difference of DO consolidation period between the cases with and without dental germs in the distraction gaps.Conclusions: Injury of the dental germs should be avoided during the operation of mandibular osteotomy and fixation of the distracter. Distraction osteogenesis and the development of dental germs on the osteotomy lines will not be affected by each other.

目的:通过对3例下颌骨截骨牵引延长过程的观察,研究牵引成骨对牙胚生长发育及萌出的影响。方法:共有8例小下颌畸形患者接受了下颌骨截开牵引延长治疗以矫治小下颌畸形。其中3例患儿的下颌截骨线上有尚未萌出的牙胚。3例中6侧下颌骨最大牵引延长幅度为22.5mm,最小幅度14mm。在截骨前后、牵引前后及随访过程中X线纵向随访观察,研究牙胚的位置改变及其生长发育的变化过程。结果:经过7~12个月的随访,发现所有在截骨线上或附近的尚未萌出牙胚,在截骨间隙牵开扩大后牙胚将移至牵引间隙中,而且能正常地生长发育、萌出。新骨的沉积钙化速度及新骨成熟所需时间与其他病例无明显差异。结论:手术应避免损伤截骨线上及邻近的尚未萌出的牙胚;在颌骨截开牵引延长的过程中,牵引成骨将不影响牙胚的生长发育,牙胚可以在经牵引形成的新骨中正常地生长发育及萌出;截骨间隙中牙胚的存在不影响新骨的形成。
