Cooling strategies and transport theories for brain hypothermia resuscitation
      
Under such adverse situations, the potential for cerebral protection through hypothermia has been intensively investigated in clinics by lowering brain temperature to restrain the cerebral oxygen demands.
      
It is now clear that hypothermia would serve as the principal way for neurologic protection in a wide variety of emergency medicines, especially in cerebral damage, anoxia, circulatory arrest, respiratory occlusion, etc.
      
Compared with the expanded trials in clinics, only very limited efforts were made to probe the engineering issues involved, which turns out to be a major obstacle for the successful operation of brain hypothermia resuscitation.
      
Aiming to provide a complete overview of the brain hypothermia resuscitation, this article comprehensively summarizes the recent progresses made in theoretical, practical and experimental techniques in the area.

General hypothermia was produced in 27 rabbits under nembutal anesthesia by immersing in an ice water bath, during the course of which, the carotid blood pressure was registered, the depressor and pressor reflexes elicited by faradic stimulation of the central end of the depressor nerve and by temporary occlusion of one side of the carotid artery respectively were examined. General hypothermia was produced in 27 rabbits under nembutal anesthesia by immersing in an ice water bath, during the course of which, the carotid blood pressure was registered, the depressor and pressor reflexes elicited by faradic stimulation of the central end of the depressor nerve and by temporary occlusion of one side of the carotid artery respectively were examined. Besides, both reflexes were also observed in five nembutalized rabbits with differential brain cooling and in another five with local refrigeration of the sinus and the depressor nerves in order to elucidate the relative importance of the brain and the afferent nerve in the effect of hypothermia. Finally, the influence of adrenaline given intravenously upon the arterial pressure was compared in another 4 rabbits at various levels of general hypothermia. During the course of general hypothermia, the depressor reflex remained unchanged until the colonic temperature fell below 30℃, and then diminished progressively with the further fall of body temperature, finally disappearing at about 22-23℃. The pressor reflex, on the other hand, showed an initial exaggeration followed by gradual diminution. Rewarming of the animal restored both reflexes to the level before cooling. Correlation of the strength of the stimulus with the size of the induced depressor response in the course of general hypothermia revealed in about 1/3 of the cases the characteristic phasic changes of parabiotic inhibition. While the general course of the changes of vascular reflexes with differential brain cooling resembled that in general hypothermia, the former procedure was, however, more effective. The local refrigeration of the afferent nerves produced the least effect. The brain apparently played an important role in the effect of hypothermia on the vascular reflexes. The fact that the pressor response to the injection of adrenaline was still quite significant at the colonic temperature of 22-23℃ indicates that the cardio-vacsular effector was more resistant to the general hypothermia.

(一)本实验是對家兔全身低溫過程中血管內感受性加壓和減壓反射的變化進行了研究,並闡明了这种變化的機制。 (二)在低溫初期,血管內感受性减壓反射無明顯變化;自31℃以下,反射隨體溫降低而呈直线的減弱。加壓反射則初期增强,隨後減弱。復溫後,兩種反射恢復正常。 (三)選擇性頭部低溫與全身低溫一樣地引起血管內感受性反射的減弱,這說明全身低溫時的上述反射變化主要取决於低溫對神經中樞的作用,而這種作用可能使中樞處於間生性抑制狀態。 (四)低溫對反射弧的傳入和傳出環節以及效應裝置亦有作用;但在一般低溫狀態下,這種作用對反射性效應的影響不大。 (五)低溫至28℃左右時,仍有相當明顯的內感受性反射,這一事實具有一定的臨床意義。本文承吳襄教授指正和馮鎮沅同志提出寶貴意見,陈玉霜同志在工作中给予經常的協助,謹此一併致谢。

The effects of temperature on the rhythmic activity of Purkinje fibers (PF)were significant. The rhythmic activity disappeared when temperature was lowered down to 30±3℃. During the course of temperature reduction, a kind of prepotential (Pre) may occur about 60 msec before each action potential in VF. The Pre could appear without its subsequent action potential and disclosed that its amplitude was about 10 mv and its duration was about 200 msec.On the Purkinje Fibers, the notch of the action potential (after phase 1) might repolarize to 50%when the superfusate temperture was reduced to 30℃, and repola-rize to 100% when the temperature was 25℃. So the fast component of the PF action potential, whose duration was about 70 msec associating with a 40 msec refractory period, could be separated from the subsequent slow one.Key words, hypothermia, prepotential, action potential, graded response, cardiac ventricular fiber.

This report summarizes the results of a I1/2-year observation on hibernation of the ground squirrels (Citellus dauricus) under laboratory conditions during 1980, 10-1982, 4 .The body temperatuer (Tb) of the homoiothermal or euthermal animal has its own pattern of circannual rhythm which does not completely depend on the seasonal change in the ambient temperature (Ta).In the months following the emergence from hibernation, animals usually have a higher and steady Tb, the Tb of euthermic animals (inguinal skin temperature) fluctuates between 32 and 37.5℃, with an average of 35.6℃ during April to June, and declines progressively after the gain in body weight reaches a peak.In August, subnormal body temperature below 32℃ is obtained in some animals, which may be considered to be the sign of the beginning of a new hibernation season. During November to Feburary the Tb of euthermic animals varies from 28 to 36℃ averaging 31℃, about 5 ℃ lower than that in the nonhibernation season.An obvious cause for this difference is that the hibernating animals are frequently interrupted by periodic arousal, and that transitional body temperatures between hypothermia and hpmeothermia may be resulted. In addition, the lowering of Tb suggests the influence of season on the fall-down of set-point in the hypotba-lamic center during sleep.As the room temperature lowers in the fall, tendency for hibernation of the ground squirres increases, and the number of animals in shallow hypothermic state (31.9-15℃) rises progressively. From September to December, the percentage of hypothermic Tb (below 31.9℃) rises from 47% to 84.8%, indicating the progression from shallow hibernation to deep hibernation.In January and Feburary, over 85% of the Tb is hypothermic, and mostly below 15℃ indicating that the animal population is at the deep hibernation period. In Spring, a rise in room temperature seems to have accelerating influence on the emergence from torpor. 70% of the animals emerge when their Ta reaches 10℃. Termination of the torpor season for the ground squirrel population is distinct.The overall hibernating season of Citellus dauricus under laboratory conditions is about 6.5 months, from the second half of September to March.

观察了达乌尔黄鼠在实验室内冬眠的一般情况。常温黄鼠的体温有着规律性的年周期,与环境温度的年周期变动不完全呈依从关系。出眠初期(4月下旬),动物体温高而稳定。4月至6月常温黄鼠的平均体温(皮温)为35.6℃,波动菹围32—37.5℃。随着体重达到顶峰,体温逐渐降低。8月份部分黄鼠出现低于32℃的低常体温,表明部分黄鼠自8月盛夏开始冬眠。但就整个种群而言,北京地区实验室内黄鼠冬眠季自9月下半月开始。3月底止,共6.5个月。秋季室温下降,动物入眠趋势增长。浅低体温(31.9—15℃)的比数逐渐升高。9月至12月,低体温(低于31.9℃)的百分比从47%增至84.8%,反映了动物从浅冬眠向深冬眠过渡。1月至2月份,低体温占85%以上,深低体温(低于15℃)占绝对优势。标志着动物种群的深眠月份。秋季动物从常温期向冬眠期转化的界线是不清的,而春季从冬眠期向常温相转化的界限却比较明显。
