目的:通过正常组与疾病组室壁运动速度的对比分析,以验证该技术的准确性,为定量评价局部室壁运动提供新方法。方法:分别对28例正常组与15例陈旧性前间壁心肌梗塞组、20例扩张型心肌病组(DCM)前间隔及左室后壁运动速度进行测量,并同时将二维图像作对比分析。结果:陈旧性心肌梗塞组室间隔收缩期峰值速度明显低于正常对照组P<0.001;DCM组前间隔与后壁收缩期峰值速度明显低于正常对照组P<0.001。2DE-TDI显像中,正常组收缩中期前间隔与左室后壁以明亮的蓝色及红黄色编码,疾病组彩色编码暗淡或消失。结论:TDI将M型与多普勒超声技术结合起来,不仅可以评价心脏整体功能,而且还能够对局部室壁的运动速度进行分析,是一有价值的显像技术,值得进一步研究。

Objective To evaluate quantitatively electro mechanical function of sinoatrial node (SAN) with intracardiac ultrasonic tissue Doppler imaging.Methods It was that collected time sequence tissue Doppler images of SAN using an intracardiac ultrasonic catheter via superior vena cava, using the junction of anterior wall of superior vena cava and superior ridge of right atrial auricle as anatomic marks. It was that located the SAN region in the images with an ellipse crop box(the shape of the SAN region is like an ellipse in the images) and subdivide it into several regions.It was that separated the Doppler information (color signal) from the anatomical information (gray scale signal) and converted it to numerical values of acceleration automatically using dedicated software. The function parameters (i.e., the mean and variance of acceleration, the acceleration time curve, cumulative distribution function, etc.) at these regions separately was calculated.Results The three major waves on the acceleration time curve of SAN in one cardiac cycle were temporally related to specific simultaneous ECG phases (i.e., the first wave occurred at the very beginning of P wave, the second wave appeared during P R interval and the third wave started after QRS wave). The acceleration value and distribution were different among the total, inner and outer regions of SAN. The cumulative distribution function also provided an overview of the acceleration distribution within SAN at different phases.Conclusions The electro mechanical function of SAN could be evaluated quantitatively with time sequence intracardiac ultrasonic tissue Doppler images feasibly and practically. It may improve the understanding of the relationship between myocardial electro mechanical activation and anatomy within SAN, and conduce to SAN target pacing and precision ablation.

Objective\ To assess regional wall motion abnormalities(WMA) with various velocity\|related indices derived from myocardial pulsed Doppler imaging(MPD), and to compare the accuracy in the detection of WMA with MPD, tissue Doppler image(TDI) and 2 dimension echocardiography(2\|DE).\ Methods\ MPD, TDI and 2\|DE were performed in 30 patients with coronary artery disease and 27 normal subjects by using commercially available echocardiographic system(Ving\|Med System Ⅴ).\ Velocity\|related indices derived from MPD including peak velocity of S wave, E wave and A wave, A/E and MVG (myocardial velocity gradient) were applied for the detection of WMA.\ For evaluation of WMA, left ventricle was divided into 20 segments.\ The sensitivity, specificity and accuracy in the detection of WMA with MPD, TDI and 2\|DE were compared.\ Multi\|factor stepwise regression was employed for analyzing the significance of velocity\|related indices for detection of WMA.\ Results\ Pulsed Doppler velocities from left ventricular wall were characterized by S wave, E wave and A wave.\ A/E and MVG derived from MPD were the most sensitive indices and were superior to TDI and 2\|DE for detection of WMA.\ The sensitivity in the detection of WMA with MPD, TDI and 2\|DE were 73 3%, 50 0% and 46 7%, and specificity were 85 2%, 74 5 and 70 3%, respectively.\ Among velocity\|related indices, multi\|factor stepwise regression showed A/E and MVG were the most important contributors for detection of WMA.\ Conclusion\ MPD is a sensitive and accurate method for detection of WMA in patients with coronary artery disease, velocity\|related indices derived from MPD may be used to detect WMA accurately and quantitatively.\ Among velocity\|related indices, A/E and MVG are the most important parameters for detection of WMA.\;

目的 应用心肌组织脉冲多普勒频谱 (MPD)速度指标检测节段性室壁运动异常 (WMA) ,探讨 MPD在室壁运动功能异常检测中的准确性。 方法 冠心病患者 30例 ,正常对照组 2 7例。利用 Ving- Med System 超声心动图仪进行 MPD检查 ,检测左室壁运动功能。用于室壁运动分析的 MPD速度指标为 S波、E波、A波的峰速度 ,A/ E比值及跨壁速度梯度 (MVG)。按照冠状动脉分布将左室壁划分为 2 0个节段进行观察。 结果  S波、E波及 A波是左室壁运动脉冲多普勒频谱的三个主要特征性频移成分。正常冠脉供血节段与异常冠脉供血节段的MPD速度指标差异有显著性。 结论  MPD是检测冠心病室壁运动异常的敏感、准确的方法。 MPD的速度指标可以准确、定量地检测室壁运动异常
