An clinical and pharmacokinetic study for a drug delivery system (DDS) of gentamycin loaded chitosan bar were carried out with the purpose to evaluate its efficacy and giving further data for its clinical applications. Eighteen cases of chronic osteomyelitis were treated by surgical necrectomy with implantation of gentamycin load chitosan bar in the prepared bone cavity. After operation, the concentration of gentamycin in serum and wound drainage fluid were examined at different times and blood urea nitrogen (BUN) and serum creatinine (Cr) as well. The clinical results were evaluated by the conditions of wound healing and clinical and roentgenographic manifestations. The results showed that the serum gentamycin concentration reached its peak level (0.86 μg/ml) at 24 hours after operation and lasted for 4 days. No increase in the concentrations of BUN and Cr were observed after implantation. The gentamycin concentration in wound drainage fluid was several hundred times higher than the minimum inhibitory concentration (MIC) for staphylococcus aureus. All of the 18 cases were followed up for 24.8 months (in an range of 6~34 months) 16 patients received initial cure and without any recurrence. So, it could be concluded that the gentamycin loaded chitosan DDS was a simple and effective method for the treatment of chronic osteomylitis without the necessity to carry out a second operation to remove the drug carrier, and it was sound to popularize its clincal application.

对几丁糖庆大霉素局部药物释放系统治疗慢性骨髓炎的疗效及体内动力学行为进行研究,为其临床推广应用提供理论依据。采用外科病灶清除术后残腔内置入几丁糖庆大霉素缓释药治疗慢性骨髓炎18例。监测术后不同时间庆大霉素血药浓度,血尿素氮(BUN)和肌酐(Cr)值,局部引流液中庆大霉素浓度。以术后切口愈合情况、临床表现和X线片评定疗效。结果,血药浓度峰值(0.83μg/ml)在24小时出现,维持时间为4天;术后BUN和Cr值无增加;术后局部引流液中庆大霉素浓度为金黄色葡萄球菌最低抑菌浓度数百倍。18例患者均获随访,随访时间6个月~34个月,平均24.8个月。初期愈合16例,无一例复发。认为,几丁糖庆大霉素缓释药是临床治疗慢性骨髓炎的有效方法,具有简便安全,且无需二期取出载体的优点,为临床推广应用提供了理论依据。

Objective To conduct a clinical study and release test for a drug delivery system (DDS) of gentamicin loaded chitosan bar in attempt to extend its clinical application. Methods The diffusion behavior of gentamicin from chitosan DDS was detected using in vitro, in vivo release test. Eighteen cases of chronic osteomyelitis were treated by chitosan DDS. The clinical result was evaluated by wound healing and clinical and X radiographic manifestation. Results Concentration of gentamicin about 926 7 μg/bar/day was released from DDS within the first 24 hours by in vitro diffusion test. It fell and sustained by 25th day with rate of 4 0 μg·bar -1 ·day -1 . In in vivo study, serum gentamicin concentration reached its peak level (0 92 μg/ml) 24 hours after implantation. No increase was observed in the concentration of BUN and Cr. In all bone tissue around the bar, 8 weeks after oderation the gentamicin concentration exceeded the minimum inhibitory concentration for the common causative organisms of osteomyelitis. The follow up time was 24 8 months (6-34). The initial cure ratio was 2/18 and the recurrence ratio zero. Conclusion The gentamicin loaded chitosan bar is a good DDS with sustained antibiotic effect in vivo and in vitro. It is an effective method for the treatment of bone infection.

目的 研究几丁糖庆大霉素药物释放系统释放行为及临床应用疗效。 方法 以 3 0只家兔为实验对象 ,采用体外、体内释放实验评价几丁糖庆大霉素缓释药缓释效果 ;临床上采用几丁糖庆大霉素缓释药治疗慢性骨髓炎 18例 ,以术后切口愈合情况、临床表现和X线表现评定疗效。 结果 体外释放实验显示第 1天释放量为 92 6 7μg/颗 ,其后下降并以 4 0 μg·颗 -1·d-1较低水平稳定释放 ,达 2 5天以上 ;体内释放实验显示血药浓度峰值 ( 0 92 μg/ml)在 2 4h出现 ;术后血尿素氮 (BUN)和肌酐 (Cr)值无增加 ;术后第 8周 ,药棒周围骨组织中庆大霉素浓度仍在骨髓炎常见致病菌最小抑菌浓度 (MIC)以上。临床应用 18例患者均获随访 ,随访时间 6~ 3 4个月 ,平均 2 4 8个月。初期愈合率88 9% ( 16/ 18) ,无复发。 结论 几丁糖庆大霉素缓释药具有较好体内外缓释作用 ,临床上对于治疗骨感染有应用前景。

METHODS: Drug delivery system consisted of gentamicin sufate dispersed in a copolymer containing oleic/linoleic acid dimer (OAD/LOAD) and sebacic acid (SA) in a 1:1 weight ratio. The gentamicin release from [Poly(OAD/LOAD:SA)-gentamicin] was tested in water, 0.9% saline, and phosphate buffer 0.1 mol/L. RESULTS: The gentamicin concentration peak was found on d 2, then slowly decreased, considerable amount of gentamicin was still released on d 50. From d 2 to d 50, the gentamicin concentration in the releasing fluids was from 59 to 42128-fold and 1.8 to 1314-fold of the MIC for Staphylococcus aureus and Escherichia coli, respectively. Staphylococcus aureus and Escherichia coli were strongly inhibited by the releasing fluids for 50 d. The gentamicin release and anti-bacterial activity in the three media were similar, only in 0.1 mol/L phosphate buffer, from d 2 to d 14 it was lower. CONCLUSION: Poly(OAD/LOAD: SA)-gentamicin was useful to treat chronic osteomyelitis.

目的:研究聚(油酸/亚油酸-癸二酸)-庆大霉素[Poly(OAD/LOAD:SA)庆大霉素]对常见菌是否具有强大的抑菌能力。方法:分别在0.1 mol/L磷酸盐缓冲液(pH 7.4)、蒸馏水、0.9%生理盐水三种介质中测定Poly(OAD/LOAD:SA)-庆大霉素体外释药情况和释出液的抑菌能力。结果:Poly(OAD/LOAD-SA)-庆大霉素在三种介质中的最大释药能力发生在第2日,然后缓慢下降,至第50日尚有相当高浓度的庆大霉素释出,从第2日到第50日,释出液中庆大霉素的浓度分别是对金黄色葡萄球菌和大肠杆菌最小抑菌浓度的59-42128倍和1.8—1314倍;三种介质的释出液对金黄色葡萄球菌和大肠杆菌均有强大的抑菌能力。除从第2日到第14日在磷酸盐缓冲液中的释药和抑菌能力较弱外,药丸在三种介质中的释药特点基本相似。结论:Poly(OAD/LOAD-SA)-庆大霉素用于治疗慢性骨髓炎具有良好的前景。
