281 cases of severe virus hepatitis were analysed retrospectively.Among them 69 were fulminant hepetitis,117 were subacute severe hepatitis and 95 were chronic severe hepatitiis.The relationship between the laboratory findings and the causes of death were analysed.According to our autopsy materials,we briefly discussed the factors affecting prognosis,recognize the impending complications and gave some preventive therapy It is an important thing for decreasing death rate of severe virus hepatitis.

本文回顾了我院281例重症肝炎死亡病例,其中急重型69例,亚重型117例,慢重型95例。对281例死亡病例的并发症、实验室检查与死亡的关系进行了分析,并结合23例尸检病理改变,对重症肝炎的诊断分型及影响预后的因素进行了讨论。提出早期预防,早期发现并发症,及时治疗是提高重症肝炎治愈率的重要环节。

The article reports 245 cases of chronic hepatitis B treated with Dongwuhua tablets. The short—term clinical effect is expressed by a effective rate of 82.9% in the treatment group and 69.9% in the control group (p<0.01). This proves the effectiveness of the drug in restoring liver function, inhibiting and eliminating hepatitis B virus. Especially in repairing liver tissue the therapeutic effect is better than that of the control group.

本文报道用冬五花片治疗慢性肝炎245例,临床近期效果:治疗组有效率为82.9%,对照组有效率为69.9%,P<0.01。说明该药在肝功能恢复、乙肝病毒抑制和清除方面均有效。经肝穿刺检查发现该药对肝组织有明显修复作用,疗效明显优于对照组。

T lymphocyte,skilling effect on the hepatic cells with HVA is gene-rally considered to play an important role in hepatic necrosis related to vi-rus hepatitis(esp.Type B).If it is so,necrosis of the hepatic cells sho-uld be in accordance with infiltration of lymphocytes.In fact,however,while being re-examined,all the 18 autopsy tissue sections with fulminanthepatitis(9 Type A and 9 Type B),we found only a few lymphocyteswere infiltrated in the necrotic liver lobules in 11 cases.Judging from our finding,in addition to the fact that necrosis of virushepatitis is closely related to the attack upon hepatic virus antigen by the Tlymphocytes,we also found the following factors(at least in fulminanthepatitis).1.Necrosis is associated with antibody-dependent cytotoxicity andmacrophage-mediated cytotoxicity.2.Necrosis is not always caused by T lymphocytes entering thehepatic cells,but by the killing effect rising from the secreted lymphoto-xic factors that pass through the hepatic sinusoid and Disse's space intothe hepatic cells.3.The possibility should not be excluded that hepatic virus itselfcaused the sub-vital damage to the hepatic cells.4.The deformation of hepatic vasculae,Congestion and hemorrhageof the tissue caused by the immunity reaction are considered to have someeffects on the hepatic necrosis,5.The serious cholestasis inaccompaniment in hepatic necrosis mightdamage the hepatic cells.

通过对18例急性、亚急性重型病毒性肝炎(甲型和乙型各9例)肝细胞坏死与肝内淋巴细胞浸润关系的观察,探讨病毒性肝炎所致肝细胞坏死的机理,认为除与 T 淋巴细胞直接侵入,杀伤带有病毒抗原的肝细胞外,尚可能与其他一些因素有关,对此从五方面进行了分析和讨论。有关病毒性肝炎(主要是乙型肝炎)肝细胞坏死的机理已进行了大量研究,普遍认为是由于 T 淋巴细胞杀伤带有肝炎病毒抗原的肝细胞所致。因此从理论上讲肝小叶的坏死区内应看到淋巴细胞与肝细胞表面接触及侵入肝细胞内的现象,肝细胞的坏死程度应与淋巴细胞的浸润程度相一致。但在实际观察重型病毒性肝炎的肝脏切片时,发现有些病例在广泛坏死的肝叶内仅见很少的淋巴细胞浸润。为此,我们对此问题进行了研究,以期对肝炎所致肝细胞坏死的机理有更进一步的认识。
