Objective To investigate the myocardial micro vascular impairment and the relationship between its severity and left ventricular hypertrophy(LVH) in patients with essential hypertension(EH) Methods Myocardial contrast echocardiography(MCE) was performed in 24 hypertension patients (group 1) and 8 healthy subjects (group 2) before and after dipyridamole injection The rest value of A(local capillary density), β(micro bubble velocity representing the blood velocity), and A·β (representing local myocardial blood flow) were calculated The ratio A, ratio β, and A·β were the parameters before and after dipyridamole injection showing the coronary flow reserve (CMVFR) Results Compared with healthy subjects, the rest value of A,β, and A·β in EH patients were slightly higher ( P >0 05) After dipyridamole injection, A,β,A·β, ratio A, ratio β and CMVFR were obvious lower( P <0 05 and 0 01) than those in controls A and A·β were increased, and ratio A and CMVFR were decreased with phases of disease A and A·β correlated positively with systolic and diastolic blood pressure( P <0 01), CMVFR correlated reversely with diastolic blood pressure( P <0 05) in EH patients In patients with LVH, A, β and A·β were increased, ratio β and CMVFR reduced in patients with LVH than those without LVH A positive correlations between A, A·β and left ventricular mass and left ventricular mass index( P <0 01) and negative correlation between CMVFR and interventricular septal thickness( P <0 05) was found Conclusion In patients of hypertension the rest myocardial microvascular blood flow was increased, with flow reserve and endothelium independent vascular relaxing ability impaired along with the rarity of capillary These damages are progressing with disease severity The microvascular function is impaired more seriously in patients with LVH than those without LVH
