Objective: To observe the protection of cardial function in patients with chronic renal failure(CRF) when treated with Losartan. Results: After treatment, BP of the two groups decreased and V S,TVI S,V E,V E/V A increased significantly than before. Before and after treatment, there was no significant difference in BP between the two groups; before treatment, there was no difference in V S,TVI S,V E,V E/V A of the two groups, but after treatment, V S,TVI S,V E,V E/V A in group of treatment were significant greater than those of group of controls. Conclusion: There is good protection of cardiac and improvement of cardial function in patients with CRF when treated by Losartan, and these action do not depend on the function of reduction of BP.

目的 :观察氯沙坦对行维持性血透治疗的慢性肾功能衰竭 (CRF)患者心血管保护作用。方法 :6 0例维持性血透患者 (血肌酐≥ 70 0 μmol·L- 1 ) ,治疗组 (给予氯沙坦 5 0~ 10 0mg·d- 1 )及对照组 (给予非血管紧张素受体拮抗剂和非ACEI类降压药 )各 30例 ,记录用药前后各组患者血压 ,运用多普勒组织成像 (DTI)技术观察用药前后两组患者二尖瓣环峰值收缩速度 (VS)、收缩时间速度积分 (TVIS)、舒张早期速度 (VE)、舒张晚期速度 (VA)、VE VA 比值。结果 :治疗组及对照组患者治疗后血压均明显下降 ,二尖瓣环VS、TVIS、VE、VE VA ,则均明显上升 ;治疗组与对照组血压分别在治疗前、后相比并无明显差异 ;治疗前两组间二尖瓣环VS、TVIS、VE、VE VA 并无显著性差异 ,但在治疗12个月后 ,治疗组二尖瓣环VS、TVIS、VE、VE VA 则明显高于对照组。结论 :氯沙坦对行持续性血透的CRF患者具有良好的心脏保护作用 ,可有效改善其心功能 ,且不依赖于其降压作用

目的 :探讨应用组织多普勒技术 (DTI)测量慢性肾功能不全 (CRF)的心肌运动速度频谱 ,评价心功能的价值。方法 :5 3例CRF ,根据内生肌酐清除率 (Ccr)分成尿毒症组 34例 (A组 )和氮质血症组 19例 (B组 ) ,2 8例正常对照组 (C组 ) ,在左室侧壁二尖瓣环、间隔二尖瓣、右室游离壁三尖瓣环 ,以及左室后壁心内膜、心外膜五个位点 ,记录收缩期S波、舒张早期E波、舒张晚期A波峰值、速度时间积分 ,以及时间指标。结果 :A、B组各瓣环E波及积分、E/A比值及E/A积分比值 ,左室后壁舒张期最大跨壁速度梯度均较C组显著降低 ,左室侧壁二尖瓣环S峰及S积分、间隔二尖瓣环S积分也较C组显著降低。心电图Q波至三处瓣环E峰的时间间隔在A、B两组有显著差异 (P <0 .0 5 )。结论 :CRF患者左右心室舒张功能减退 ,左右心室间舒张不同步 ,左室收缩功能也有减退证据。DTI技术可以很好地评价此类患者的心室功能。

Objective To investigate cardiac structure and function in diabetic nephropathy patients starting continuous ambulatory peritoneal dialysis by echocardiography and Doppler tissue imaging (DTI).Methods Nine diabetic nephropathy (DN) patients and 21 non-DN patients were enrolled to perform echocardiography and pulsed wave-DTI (PW-DTI).Their blood pressure,fluid volume and biochemical indexes were also measured.Echocardiography indexes included left ventricular ejection fraction (LVEF),the ratio of early-diastolic to late-diastolic peak velocities of pulsed Doppler blood flow at the mitral valve orifice (E/A).DTI indexes included systolic peak velocity (VS),early-diastolic and late-diastolic peak velocity (VE,VA) and VE/VA.Results ①On echocardiography,the prevalence of left ventricular hypertrophy (LVH) in DN group was higher than that in non-DN group (P<0.05).Both groups had normal LVEF and decreased E/A.There were no significant difference for LVEF and E/A between two groups.②The VS of the septum,anterior wall and the average of mitral annulus is significantly lower in DN group than that in non-DN group (P<0.05).The VE /VA of the septum was the same (P<0.05).Conclusion Diabetic nephropathy patients starting peritoneal dialysis had left ventricular regional and / or global systolic and diastolic dysfunction even with higher residual renal function.Therefore,dialysis should be started as early as possible for DN patients.

目的 采用超声心动图和多普勒组织成像 (DTI)方法观察初行持续不卧床腹膜透析(CAPD)糖尿病肾病 (DN)患者的心脏结构和功能。方法 选择 9例初行CAPD的DN患者和 2 1例非DN的终末期肾病患者 ,均行超声心动图和脉冲 DTI(PW DTI)检查 ,同时测定血压、血容量及生化指标。超声心动图反映心功能的指标有左室射血分数 (LVEF)、二尖瓣口舒张早期血流速度和心房收缩期血流速度比 (E/A)。结果  ( 1)超声心动图发现DN组左室肥厚 (LVH)的发生率明显高于非DN组 ,两组LVEF均在正常范围 ,舒张功能减退 ,且无显著性差异 ;( 2 )PW DTI探测DN组二尖瓣环间隔、前壁及均值的VS均低于非DN组 ,二尖瓣环间隔的VE /VA也低于非DN组。结论 DN患者在肾功能尚未严重恶化而行透析治疗时 ,已存在局部和 (或 )整体的左室收缩和舒张功能减退。对于DN患者 ,透析治疗应该提前
