Objective To test device integrity and objective auditory reaction for cochlear implant patients intraoperatively. Methods Our protocol for intraoperative testing of the implant device includes device electrode impedance test and neural response telemetry (NRT), which measures the electrically evoked auditory nerve compound action potentials (ECAP). Results We completed electrode integrity tests and NRT intraoperatively on 40 patients with the Nucleus CI24M cochlear implant. We have measured normal implant function on all 40 patients and obtained ECAP results from 39 patients. Out of 33 patients with normal inner ear, typical ECAPs were recorded in 195 electrodes in all 198 testing electrodes(98.5%). In 7 patients with inner ear Mondini dysphasia, affirmative ECAP waveforms were recorded in 26 electrodes in all 42 testing electrodes. The basal electrode ECAP threshold was higher than that of the epical one. Conclusions The intraoperative measurements of device function and NRT are quite simple, quick and with high accuracy. Inner ear malformation is an important factor that may affect NRT results.

ABSTRACT Neural response telemetry (NRT)is a method that enables direct measurements of the electrically evoked compound action potential (ECAP) of the auditory nerve of cochlear implant patients using Nucleus C124M cochlear implant system.In this project,twenty patients who were implanted with Nucleus C124 M underwent measurements of the ECAP using the NRT system .It was expect to observe the changes of the responses by changing the parameters in the NRT 2 04 software.It is concluded that the parameters that make great influences to the response waves are CL (current level ),stimulation pulse width,gain,delay and masker advance.The parameters that make great influences to the record time are stimulation rate and number of sweeps.The main parameters that need to be changed in the measurement are CL,delay,gain and stimulation rate.In order to reliably and quickly record the ECAP responses from cochlear implant recipients,a specific protocol is proposed for the measurement.

Objective To provide an objective method to estimate T-levels and C-levels using neural response telemetry (NRT) thresholds for children, from whom we can not get accurate responses because they are very young or have other disabilities, by comparing NRT thresholds and T/C levels. Methods Seventy patients implanted with the nucleus CI24M multiple cochlear implant system participated in this study. 329 electrodes were tested. The software used in this study was NRT 2.04. Monopolar stimulation mode was used during NRT threshold measurement.No. 3,5,10,15,20 electrodes were tested for each patient. The T-levels and C-levels were obtained at the same visit. Results 92.7% of all the tested electrodes recorded NRT responses.There were variations in the amplitudes and thresholds of NRT responses across subjects and electrodes. NRT thresholds fell within the different points of the MAP dynamic range. The mean of NRT thresholds was shown to fall between the mean of T-levels and C-levels. The NRT thresholds and T/C levels reduced from basal to apical ends of the cochlear. Conclusions Considerable variability across subjects was noted. So it is difficult to estimate T-levels and C-levels accurately by testing only NRT thresholds only. NRT technology provides a new objective method for estimating T-levels and C-levels for children who are unable to cooperate with audiologists during mapping after operation. The new NRT technology should be improved in the future.
