     BACKGROUND & OBJECTIVE: Lymphocytic leukemia and bone marrow involvement of non-Hodgkin's lymphoma (NHL) can be diagnosed by bone marrow morphology or lymph node biopsy combined with bone marrow examination. The data of original and differentiation status of tumor can be analyzed by immunophenotype of bone marrow. These are necessary for diagnosis and treatment of lymphocytic malignancy.
     背景与目的:骨髓形态学检查可诊断淋巴细胞白血病,结合淋巴结活检可诊断非霍奇金淋巴瘤(non-Hodgkin’s lymphoma,NHL)侵犯骨髓,但形态学仅是初步的诊断,采用免疫分型而获得肿瘤细胞来源及发育阶段的资料是目前诊治淋巴系统恶性肿瘤所必需的。

Objective:To explore the changes of serum levels of soluble interleukin2 receptor (sIL2R) in lymphocytic malignancies and its clinical implication.Methods:The serum levels of sIL2R were measured in 60 cases of patients with various malignant lymphocytic proliferative disorders by using ELISA method.At the same time,the ratio of CD4+/CD8+ of peripheral blood mononuclear cells was also determined in 43 patients by using fluorescence activated cell sorter(FACS).Results:The serum levels of sIL2R in patients with multiple myeloma(MM),nonHodgkin's lymphoma (NHL) or acute lymphoblastic leukemia(ALL) were significantly higher than that of normal value. The ratio of CD4+/CD8+ measured in 43 patients were markedly lower than that of normal value. In MM,NHL and ALL patients, the higher values of sIL2R were revealed in severe cases or late stage of these diseases. The levels of sIL2R were independant of the infection in these cases.Conclusion:sIL2R levels significantly elevated as the disease progressing,and the immune function decreasing,and the sIL2R level may be used as an effective clinical marker.

目的:探讨可溶性白细胞介素2受体(sIL-2R)在恶性淋巴细胞增殖性疾病中的改变及其临床意义。方法:用酶联免疫吸附法测定60例恶性淋巴细胞增殖性疾病患者血清sIL-2R水平,用流式细胞仪(FCM)检测其中43例患者外周血单个核细胞中CD4+、CD8+细胞数。将患者以不同病期,是否伴有感染等情况分组进行比较,并对3例非霍奇金淋巴瘤(NHL)患者进行治疗前后的sIL-2R随访。结果:多发性骨髓瘤、NHL及急性淋巴细胞白血病患者血清sIL-2R含量高于正常人,其水平与疾病的临床分期、病程密切相关。上述患者外周血单个核细胞中CD4+细胞数与CD8+细胞数的比值明显低于正常人。血清sIL-2R水平在此类疾病中与感染无关。结论:在恶性淋巴细胞增殖性疾病中,sIL-2R水平随着疾病的进展及机体免疫功能的下降而明显升高,可作为一个有效的临床监测指标。

This study was to investigate the anti lymphocytic malignancy immunologic effects induced by two types of the idiotypic nucleic acid vaccines which were constructed from the genomic DNA and RNA of the human B lymphoma cell line respectively. Namalwa cell line and BALB/c mice were used as the models. The gene fragments of the IgH variable region(IgHV), which were obtained from the genomic DNA and RNA of Namalwa cell respectively, were cloned into the eukaryocytic expression vector pcDNA 3.0 to be used as the idiotypic nucleic acid vaccines. After transfecting COS cells with one of vaccines constructed from the genomic DNA by using LipofectAMINE,the result of transcription was identified by using RT PCR. The experimental mice were immunized by intramuscular injection with two types of vaccines. The specific anti idiotypic antibody was detected by indirect immunofluorescence assay. The results showed that the nucleic acid vaccine constructed from the genomic DNA can be transcribed in COS cells, the transcription product turned shorter,and the intron region of 86 bp was spliced accurately. When immunizing the mice, two vaccines both induced the anti idiotypic antibody against Namalwa cell, the anti idiotypic antibody could be detected since the fourth week after immunization, and got to the peak of titer on the sixth week. It was concluded that the nucleic acid vaccines against lymphoma can be constructed from both the genomic DNA and RNA.

本研究观察从人B细胞淋巴瘤细胞系基因组DNA及RNA分别构建的表达载体作为核酸疫苗诱导小鼠产生抗肿瘤免疫反应的情况。分别提取人B淋巴瘤细胞系Namalwa细胞的基因组DNA及RNA ,将免疫球蛋白重链可变区 (IgHV)基因片段克隆到pcDNA 3.0真核表达载体中作为独特型核酸疫苗 ,其中DNA来源的核酸疫苗用LipofectAMINE转染COS细胞 ,然后用RT PCR观察转录及剪接的情况。两种不同来源的核酸疫苗分别肌内注射免疫小鼠后 ,用间接免疫荧光法检测抗独特型抗体的生成。结果发现 ,基因组DNA来源的核酸疫苗在COS细胞中能成功地转录 ,转录产物大小为 4 77bp ,其中 86bp的内含子区域被剪接 ;两种疫苗免疫动物后均可诱导针对Namalwa细胞特异的抗独特型抗体 ,并于第 6周达高峰。结论提示 ,淋巴瘤基因组DNA和RNA来源的IgHV基因片段均可制备核酸疫苗 ,能诱发小鼠产生抗淋巴瘤免疫反应
