This article is based on the observation of the temporomandibular joint(TMJ) and on the analysis of its morphological structure. The normal values of 15 items, used to represent the structure of the joint, have been established.Three indices, that is the glenoid fossa index, the articular tubercle index and the condyle index, are accepted to express the geometrical characters and to reflect the close relationship between the two functional parts of the joint. Besides, the frequency distributing curves are presented. It is concluded that the male's TMJ is eminently larger than the female's, while there is no distinct difference between left and right sides and between groups with or without occlusion. This result might meet the needs of the Clinical research as well as the design of the artificial TMJ.

本文观察测量并分析了颞下颌关节的形态结构,建立了能较客观反映该关节形态的15项指标的正常值。用3项特征指数,即关节窝指数、关节结节指数和髁突指数,能较好地反映该关节的几何形态,并表达其功能部位的相互关系,以及用频数分布曲线反映其分布规律。本文认为,男性的颞下颌关节显著大于女性,而左、右侧之间,有无咬(牙合)组之间的差别均不显著。本文结果可为该关节的临床研究、病变分析乃至人工颞下颌关节的设计,提供形态学依据。

Five specimens of the human adult cadavers were prepared for exploration of the axis of the temporomandibular joint (TMJ) during passive movements. The ligaments of the joint were retained first and then removed. X-ray films of each specimen with and without ligaments have been taken in eight different positions and the movement of the joint was analysed. In specimen without ligaments, the condylar process rotates around a central axis about 6 mm underneath the condylar crest. The movement occurs between the condylar process and the articular disc. In specimen with ligaments intact, the opening of the mouth acts on a fixed axis, which is eccentric and lies about 12 mm underneath the condylar crest. The condylar process glides with the disc together in a foreward and downward direction.

This is refered to as edentulous jaw position. The comparative study was made between two groups. The result showed that after all the teeth lost, the upper and lower residual ridges contacted as closely as possible, the condyle became able to move backward and upward in glenoid fossa, with mental part of mandible moved upward and forward. The vertical dimension decreased. This movement stopped when the upper residual ridge contacted with lower one of ramus of mandible. Basing upon this, a concept of edentulous jaw position is put forward for the first time. It indicates that condyle is not a fixed fulcrum of masticatory movement, while the tooth can be the fulcrum of masticatory movement. According to this study, it is possible that a movable functional occlusal position with a movable condylar pathway exists. But if it is beyond the limit of its physiological adaptation, TMJDS may be the result.

本文通过对10例无牙颌总义齿修复后戴总义齿保持在正中硷位和不戴总义齿使上下颌残余齿槽嵴尽量闭合时颞颌关节的薛氏位X线片的对比分析,发现后者髁状突在关节凹内位置发生向后上旋转移位,下颌颏部发生了向上向前旋转移位。通过对其初步探讨,认为这一现象的发现有助于全面了解颞颌关节的生理功能和颞颌关节紊乱综合症的发生。对理解咬合运动的支点、髁道的可变性及指导总义齿的修复均有重要意义。
