本文报道采用原位低温灌注肾切开取石术治疗复杂性肾结石,手术安全、简便,效果较好。该手术方式不需特殊设备,时间不受限制,保护肾功能,取石满意。

Objective: To observe the clinical effect of extracorporeal shockwave lithotripsy on 84 cases of complicated renal lithiasis. Methods: 84 consecutive cases of complicated renal stones were treated by extracorporeal shockwave lithotripsy with homemade HX 902 Type lithotriptor. In order to avoid the formation of ureteric stone streets, breaking the stone in piecemeal was recommended. Results: 82 cases of 84 consecutive cases with complicated renal lithiasis (97.6%) were treated successfully....

Objective: To observe the clinical effect of extracorporeal shockwave lithotripsy on 84 cases of complicated renal lithiasis. Methods: 84 consecutive cases of complicated renal stones were treated by extracorporeal shockwave lithotripsy with homemade HX 902 Type lithotriptor. In order to avoid the formation of ureteric stone streets, breaking the stone in piecemeal was recommended. Results: 82 cases of 84 consecutive cases with complicated renal lithiasis (97.6%) were treated successfully. In 6 months the renal calculi of 52 cases were eliminated completely and the renal calculi of 17 cases were mostly eliminated. The stonefree rate was 82.1 %. The renal calculi of 13 cases were removed partly. Conclusion: It is safe, effective and painless to treat complicated renal lithiasis with extracorporeal shockwave lithotripsy.

目的 :探讨X线定位系统碎石机治疗复杂性肾结石的临床效果。方法 :应用国产HX 90 2型水囊式碎石机治疗复杂性肾结石 84例。碎石时采取先易后难 ,分次碎石 ,力争冲击一部分就要彻底粉碎 ,以防止“石街”形成。结果 :84例中多发性结石 5 2例 ,结石直径大于 2 .5cm者 33例 ,鹿角型结石 3例 ,孤立肾结石 2例 ,蹄铁型肾结石 1例。碎石成功率为 97.6 %(82 / 84 )。 6个月内结石完全排净 5 2例 (6 1.9% ) ,基本排净 17例 (2 0 .2 % ) ,治愈率为 82 .1%。部分排出 13例 (15 .8% )。结论 :体外冲击波碎石是复杂性肾结石较为安全、有效的理想治疗方法 ,碎石治疗时对不同类型结石所采用的方法应有所区别
