Objective:To study the relationship between Fcγ receptor ⅢB(Fcγ RⅢB) genotype and severe chronic periodontitis or generalized aggressive periodontitis.Methods:DNA from 35 subjects with severe chronic periodontitis,25 with generalized aggressive periodontitis and 95 of periodontal healthy controls were sampled by buccal swab and extracted using Chlex-100 method.Polymerase chain reaction with sequence specific primers (PCR-SSP)was carried out to determine the FcγRIIIB genotype.The data were analyzed with χ 2 test. Results: NA1/NA2 heterozygote was the predominant genotype in the 3 groups.No statistically differences of FcγRIIIB genotype distribution were found between any two of the stuied groups.The gene frequencies of NA1 and NA2 were 0.6 and 0.4 respectively.Conclusion:No direct relationship exists between FcγRⅢB genotype and the occurrence of severe chronic periodontitis and generalized aggressive periodontitis according to the present study.

目的 探讨中性多形核白细胞 (polymorphonuclearneutrophils ,PMNs)表面FcγRⅢb受体遗传多态性及人类免疫球蛋白G重链遗传标记因子 G2m(2 3)因子与侵袭性牙周炎的关系。方法 侵袭性牙周炎患者 2 1例 ,健康对照 2 6名 ,提取静脉血基因组DNA及血清 ,分别用PCR和免疫斑点的方法测定FcγRⅢb基因型和G2m(2 3)因子。结果 侵袭性牙周炎患者FcγRⅢBNA1 /NA1基因型检出率高于健康对照 (P <0 0 5) ;侵袭性牙周炎患者中FcγRⅢb基因型为NA1 /NA1 ,并且G2m(2 3)阳性的个体比率比健康对照组高 (P <0 0 5) ;G2m(2 3)因子在侵袭性牙周炎患者与健康对照组中的分布差异无显著性。结论 FcγRⅢBNA1 /NA1基因可能是中国人侵袭性牙周炎的易感基因 ;FcγRⅢb基因型为NA1 /NA1 ,G2m(2 3)阳性的中国人可能更容易患侵袭性牙周炎

Objective:To analyse subgingival predominant microflora of aggressive periodontitis (AgP) and chronic periodontitis (CP), and to provide experimental evidence of classification, etiology and diagnosis of periodontal diseases. Method:Subgingival plaque samples of AgP and CP from epidemiological survey of middle school students (16 cases) and clinic diagnosis of periodontal department patients (24 cases) were inoculated in non-selective medium (CDC) and selective medium(TSBV) to inspect predominant microflora.Result:When localized AgP and moderate CP patients were compared, there was a significant difference in the number of Actinobacillus Actinomycetemcomitans (Aa) positive ( p <0.05) and facultative anaerobes ( P <0.01), the total number of anaerobes in generalized AP and severe CP increased compared with localized AgP and moderate CP (P<0.05). Conclusion:Subgingival microflora in the early stage of AgP is obviously different from CP. Aa is an important risk factor for initiation of AgP.

目的 :分析侵袭性牙周炎 (aggressiveperiodontitis ,AgP)与慢性牙周炎 (chronicperiodontitis ,CP)的龈下优势菌群 ,为探讨牙周炎分类、病因和诊断提供实验依据。方法 :将中学生流调筛选 (16例 )及牙周病专科就诊(2 4例 )的AgP和CP患者 ,采集龈下菌斑样本 ,在厌氧菌基础培养基 (CDC)和选择性培养基 (TSBV)上培养分析。结果 :局限型AgP患者的伴放线放线杆菌 (Actinobacillusactinomycetemcomitans ,Aa)及兼性厌氧菌的检出率显著高于中度CP患者 (P <0 .0 5 ,P <0 .0 1) ,而广泛型AgP和重度CP患者的厌氧菌总数较局限型AgP和中度CP患者显著增加 (P <0 .0 5 )。结论 :局限型AgP和中度CP的龈下优势菌有明显差别 ,Aa是一个重要的危险因子。
