[Purpose]To study the risk and protective factors of stomach cancer and to offer epidemiologic basic for comprehensive prevention and treatment for stomach cancer. [Methods] A case-control study was conducted in Linzhou Henan province. It included 346 cases of stomach cancer and 1541 controls of normal population. The item of investigation included general information, situation of residence diet slid dietary habits, mental status and history of illness and cancer family. The odds ratio of each variable were individually calculated by using unconditional logistic regression method. Using principal component and factor analysis some factors were selected for multivariety analysis. [Results]History of upper degestive tract illness(OR = 7. 42), psychic trauma in ten years(OR = 2. 16), physics contact in occupation(OR = 2. 70), history of cancer family (OR = 1. 43 ), high wb intake(OR = 1. 31 ) and low socioeconomic status et al, were risk factors for stomach cancer. Fresh vegetables and fruits could reduce risk of stomach cancer(OR = 0. 68, OR = 0. 78 respectively, P trend < 0. 01 ). Meanwhile, the results also showed that vinger and garlic have protective function (OR = 0. 84, OR = 0. 87 respectively, P trend < 0.01 ). [Conclusion]TO treat upper digestive tract disease actively, to dispel bad lifestyle and bad habits, to eat fresh vegetable, fruits, vinger and garlic often can reduce incidence of stomach cancer.

[目的]探讨胃癌发病的危险因素及保护因素,为综合防治提供流行病学依据。[方法]采用病例-对照研究方法,对 346例胃癌病人和1541例正常人群对照进行回顾性调查。单因素分析计算出各因素的比值比及其95%可信限,初选因素(P< 0.1)进行主成份因子分析,应用非条件Logistic回归模型进行多因素分析。[结果]胃癌的危险因素主要有:上消化道疾病史、十年 内有精神创伤、职业物理接触、癌症家族史、高盐饮食及社会经济地位低等因素。主要保护因素有:新鲜蔬菜、水果高摄入量和经 常食用醋类及大蒜类食品(OR值均小于1,趋势检验P<0.01)。[结论〕积极治疗上消化道疾病,去除不良饮食及生活习惯,经常 食用新鲜蔬菜和水果及醋类和大蒜类食品,可减少胃癌发生的危险性。

Objective:In order to find out the relationship between the risk of stomach cancer and factors of dietetic history,morbid history and bad habits.Methods:A case control study was conducted for stomach cancer in Harbin.The odds ratio(ORs) and 95% confidence interval(95% CI) were calculated by noncondition Logistic regression.The trend of grade variables was calculated by χ 2 M-H .Results:The risk was faintly increased by smoking and by drinking adjusted by confoundings.The risk was enhanced by gastric chronic diseases,bad dietetic habits,havey salt,bad emotion,oppressed mind recent years,disharmonious and cracking marriage,poor human relation respectively and so on.The risk was reduced about 50% for eating more shallot and garlic.Conclusion:The statistical relations were significantly obtained between the risk of stomach cancer and tumor family history,gastric chronic diseases,bad dietetic habits and neuropsychical factors.The risk was reduced by eating more shallot and garlic.

目的 :查明饮食史、疾病史、不良嗜好等诸因素与胃癌的关系。方法 :调查病例和对照的暴露史 ,用非条件L ogistic回归方法计算各因素的比值比和 95 %可信限 ,趋势检验用 χ2M- H检验法。结果 :经混杂因素调整后吸烟、饮酒微弱地增加胃癌危险性。胃慢性疾病史、不良饮食习惯、喜重盐、情绪较差、近些年精神处于压抑状态、婚姻生活不和谐和破裂及人际关系较差等神经精神因素均与胃癌有明显关系。多食用葱蒜类食品可降低胃癌危险性 5 0 %左右。结论 :肿瘤家族史、胃慢性疾病史、不良饮食习惯、及某些神经精神因素与胃癌危险性存在明显的统计学关系 ,多食用葱蒜类可拮抗胃癌的发生。
