The acute and chronic effects of propylthiouracil (PTU) on serum thyroid hormone concentrations were assessed with that of methimazole as control. For the study of acute effects, among 54 untreated patients with hyperthyroidism selected at random, a single dose of PTU 200 mg and methimazole 20 mg were given to 29 and 25 patients respectively. And then 15 patients of each group were randomly selected for the study of chronic effects;PTU was given orally 100 mg t.i.d., and methimazole 10 mg t. i. d. for 4 weeks....

PTU is better than methimazole in treating severe hyperthyroidism and thyroid crisis as well. There was no significant difference between the chronic effects of both antithyroid drugs on serum thyroid hormone levels.

本文报告29例甲亢患者一次顿服PTU 200mg后血清甲状腺激素水平的急性变化,及15例患者日服PTU 300mg连续4周后血清甲状腺激素水平的慢性变化。并以Methimazole为对照进行比较,探讨了PTU急性和慢性作用的机理及临床实用意义。本研究结果表明,PTU可能通过对5'脱碘酶的抑制,减少外周组织T_4转换成T_3,迅速控制血清中生物活性较强的T_3水平,在严重甲亢或甲亢危象时应首先选用。PTU及Methimazole长期应用均可使血清甲状腺激素水平显著下降,且两药效果无明显差别。

Deiodinase activity was expressed in terms'of T3→T4 conversion rate (CR). The results showed that the average CR in rat brain homogenates of LI group was significantly higher than those of the other two groups, it is 2.73 times of that in N group. There were no significant differences among the three groups in serum T3 concentration and T3 content of brain homogenate. But serum T4 concentration of LI group was only 10.9% of that in N group and brain homogenate T4 level was also lower, being only 38.5% of the latter. It suggested that T4 5'-deiodinase in the immature rat brain is more effective than in adult one on modulating thyroid hormone function in brain. Thus, the increased activity of brain T4 5'-deiodinase may compensate for the decreased T4 pool, maintaining a normal brain T3 content in congenital iodine-deficient rat.

将成年Wistar大鼠随机分成三组,缺碘组(LI组):饲以低碘饲料并饮用无离子水;加碘对照组(NI组):饲料同上而饮水加碘化钾;以及普食组(N组):饲以标准实验室饲料,饮用自来水。以它们的第一代仔鼠(20日龄)为实验对象,测定了全脑匀浆的T_4 5'-脱碘酶活性、血清及匀浆的T_4和T_3含量。发现LI组匀浆T_4→T_3的转化率显著高于其他两组,血清及匀浆的T_3含量三组间无显著差别。但LI组的血清T_4含量为N组的10.9%,其匀浆的T_4水平也只有N组的38.5%。提示未成熟大鼠脑组织T_45'-脱碘酶活性的提高,可以补偿T_4代谢库的不足,维持先天性缺碘仔鼠脑组织T_3含量正常。
