     Cerebral ischemic penumbra was firstly proposed by Astrup in 1977 which means neurons remain structural intact but functional inactive, while the function of pumps on cellular membrane and ion gradient were preserved. Recently a new concept of polymolecular penumbra was raised by Sharp.

Objective:To investigate the relationship between the expression of NR1,NR2A and NR2B subunits of N methyl D aspartate receptor and the cerebral ischemic penumbra in basilar gangalia after cerebral ischemia.Methods:Perminant middle cerebral artery occlusion(MCAO) rat models were made using intraluminal filament method.Employing a semiquantitative immunobloting technique,the expression study of NR1,NR2A and NR2B subunits of basilar gangalias in both sides were determined. Results:NR1 protein level dramatically decreased at 1 hour after MCAO in basilar ganglia,and then increased significantly.NR2A and NR2B subunits displayed similar pattern of temporally dependent expresson to NR1 in the same region.Conclusion:The expression of the three subunits increase gradually at basilar ganglia from 1 and 6 hours after MCAO in rats, which supports the theory of penumbra.

Objective To investigate the expression of glucose transporter 3(GLUT 3) transcription level at different ischemic time in rats' focal cerebral ischemic penumbra and core of cortices.Methods Focal ischemic models of middle cerebral artery occlusion (MCAO) in rats were made by inserting nylon surgical thread. Brain samples were harvested from ischemic penumbra and core of cortices. The change of GLUT 3 mRNA was assessed by RT PCR.Results GLUT 3 mRNA in the penumbra regions slightly increased at 3 h after ischemia, and reached a peak at 24 h, in 96h following ischemia it came back to the normal level. As for ischemic core regions, GLUT 3 mRNA had a short time increase at 3 h after ischemia, and then rapidly decrease to the low level. Conclusion GLUT 3 mRNA expression was notably up regulated in the penumbra regions after focal cerebral ischemia,it may be a protective reaction against ischemic injury.

目的 研究大鼠局灶性脑缺血不同缺血时间皮质半暗带和中心区葡萄糖转运子 3(GL UT3 )转录水平的表达规律。方法 用插线法建立大鼠局灶性脑缺血模型 ,剥取缺血半暗带及中心区皮质组织 ,采用逆转录 -聚合酶链反应 (RT- PCR) ,测定 GL U T3 m RNA水平的变化。结果 缺血半暗带 GL U T3 m RNA在缺血 3小时升高 ,2 4小时达到高峰 ,96小时后基本恢复正常。缺血中心区 GL U T3 m RNA在缺血后 3小时有一短暂的升高 ,随后迅速下降呈低水平表达。结论  GL UT3 在缺血半暗带的表达上调 ,有可能是机体对缺血损伤的保护性反应。

Objective\ To define the histologic range of cerebral ischemic penumbra after focal cerebral ischemia reperfusion in rats.\ Methods\ A model of middle cerebral artery occlusion/reperfusion w as established. The ischemic penumbra and central area were distinguished by TTC , Toluidine blue, Nissl and HE staining methods. \ Results\ The ischemic area located at lower frontoparietal cortex and lateral striatum 2 h after reperfusion, then increased gradually and peaked on 1-2 d and expanded to the upper frontoparietal cortex, piriform cortex and me dial striatum, and returned to reperfusion of 2 h level on day 7-14. There was a transitional area in which the staining color and the structure of neuron wer e between normal and ischemic area. The number of neuron reduced significantly w ith main characteristics of necrosis in ischemic central area and apoptotic prop erty in ischemic penumbra. \ Conclusion\ The ischemic central area located at lower frontoparie tal cortex and lateral striatum, while the ischemic penumbra at upper frontopari etal cortex, medial striatum and piriform cortex.

①目的 界定大鼠局灶性脑缺血再灌注后脑缺血半影区的范围。②方法 应用线栓法建立大鼠大脑中动脉闭塞再灌注模型 ,氯化三苯基四氮唑 (TTC)染色、甲苯胺蓝染色、焦油紫染色和苏木精 伊红染色界定脑缺血半影区和中心区。③结果 脑缺血再灌注 2h ,缺血病灶位于纹状体外侧区和额顶叶皮质下部 ,随着缺血再灌注时间的延长病灶范围逐渐扩大 ;再灌注 1~ 2d最大 ,波及纹状体内侧区和额顶叶皮质上部以及梨状区皮质 ,之后逐渐缩小 ;再灌注 7~ 14d恢复到再灌流 2h的水平。缺血病灶与周围正常脑组织之间有一形态和结构过渡区 ,该区的着色和细胞形态结构界于病灶和正常脑组织之间 ,即缺血半影区。缺血中心区神经细胞明显减少 ,呈现坏死特征 ;缺血半影区细胞主要呈现核固缩、染色质凝聚等凋亡特征。④结论 脑缺血再灌注缺血中心区位于纹状体外侧区和额顶叶皮质下部 ,缺血半影区位于纹状体内侧区和额顶叶皮质上部以及梨状区皮质。
