Objective To observe the ultrastructure changes of tissue and cells in distraction gap during the process of distraction osteogenesis(DO).Methods After a mandibular cortitomy was made between the second and third premolars bilaterally, self produced intraoral tooth supported distraction devices were cemented onto the second and third premolars. The distraction devices were activated 0.25 mm every 12 hours to advance the anterior segment 8 mm in 16 days. The animals were sacrificed on 8 d, 16 d, 32 d, 48 d after the start of activation respectively, then observed under transmission electron microscope. Results In early phrases of DO, mesenchymal cells proliferated and differentiated into fibroblast like cells and osteoblasts, whose ultrastructures showed to have active protein synthesis and secreting function. Newly formed collagen, bone canaliculus and the mineration of bone matrix could be clearly observed.Conclusion The tissue and cells in the distraction gap are in active remodeling process under distraction force, which is the basis for new tissue regeneration.

目的 观察在牵张成骨过程中 ,牵引区内组织与细胞超微结构的变化特点。方法 用自制的牙支抗下颌骨牵引装置 ,对山羊下颌骨进行牵引 ,0 .2 5mm /12h ,共牵引 16d ,分别于开始牵引的 8、16、32、48d取材 ,标本固定之后进行透射电镜观察。结果 间充质细胞在牵引早期大量增生 ,并不断分化为成纤维样细胞和成骨细胞 ,后两种细胞的超微结构显示出具有活跃的合成和分泌功能 ,清晰可见新形成的胶原纤维、哈氏管、以及骨基质矿化的过程。结论 牵引区内组织和细胞在牵引力的作用下处于活跃的改建过程中 ,这正是新组织再生的基础

Objective: To probe the mechanism of expression and effect of Bone Morphogenetic Protein2 (BMP-2) and basic fibroblast growth factor (bFGF) which result in successful osteogenesis. Methods: Twenty-eight adult dogs were randomly divided into DO group (n=12) ,acute lengthening group(n=12) and normal group(n=4). 4 animals were respectively sacrificed in the 6th day of distraction, 2, 8 weeks after completion of distraction. Callus and the same regions of normal mandible were harvested. The expression of BMP-2 and bFGF were investigated by means of immunohistochemistry.Results: In DO group and acute lengthening group, during the distraction period, the strongly positive stain of BMP-2 and bFGF were both found in the osteoblasts around the newly formed trabecular bone and the bone formation front, mesenchymal cells and fibroblasts from the periosteum and distraction gap and the fibrous collagen. There were no statistic difference between the stain grays of two groups; after 2 weeks of consolidation. There were significant decreasing expression of BMP-2 and bFGF in callus of acute lengthening group comparing to DO group (P<0.05); after 8 weeks of consolidation, the stain intensity of BMP-2 and bFGF in the two groups were both markedly decreased(P>0.05). Conclusion: During the distraction osteogenesis, the distraction force and other factors such as micro-trauma may stimulate the production of BMP-2 and bFGF in local distraction gap. BMP-2 and bFGF may both participate in the effects of promoting the plenty osteogenesis.

Objective: The purpose of this study was to investigate the effects of distraction force on the proliferation of rabbit nasal chondrocytes in vitro and the relation between force distraction and proliferation rate of rabbit nasal chondrocytes. Methods: The fourth-passage chondrocytes were harvested from the nose of neonatal and 6-week-old rabbits. A continuous static distraction force was applied on the cells in vitro using a cellular static distraction device, and the proliferation of nasal chondrocytes was examined using a flow cytometry. The experimental groups were divided into seven groups under continuous static distraction force (5 Kpa, 10 Kpa) from 0 to 12 hours. Results: The results showed that the proliferation of nasal chondrocytes was promoted as the application time of static distraction force was prolonged (0~10 hours in 5 Kpa groups and 0~8 hours in 10 Kpa groups). The maximal proliferation appeared, when the nasal chondrocytes were cultured under 5 Kpa for 10 hours ,and 8 hours under 10 Kpa ( P <0.05). The proliferation of cells in the 5 Kpa group was more obvious than that in the 10 Kpa group ( P <0.05). The proliferation of cells in the neonatal rabbit group was more obviously than that in the 6-week-old rabbit group ( P <0.05). Conclusions: The data prove that static distraction stimulates in chondrocytes can influence and regulate the growth of nasal chondrocytes. It might provide a new methord to treat nasal deformity of cleft lip for the neonatal or even older children with the application of nasal cartilage distraction.

目的:探讨牵张力对体外培养的不同年龄兔鼻软骨细胞增殖活性的影响及牵张力大小与兔鼻软骨细胞增殖活性改变的量效关系。方法:将第4代体外培养的新生及6周龄新西兰白兔兔鼻软骨细胞置于细胞膜式牵张力施加装置上培养,流式细胞仪检测不同的牵张力( 5kPa、10kPa)在0 ~12h内对兔鼻软骨细胞增殖活性的影响。结果: 0~10h内5kPa牵张力组软骨细胞增殖指数随着牵张力作用时间的延长不断上升,增殖指数峰值位于10h处; 0~8h内10kPa牵张力组软骨细胞增殖指数随着牵张力作用时间的延长不断上升,增殖指数峰值位于8h处;5kPa较10kPa牵张力对体外培养兔鼻软骨细胞具有更大的促增殖作用;牵张力对体外培养的新生兔兔鼻软骨细胞具有更大的促增殖作用。结论:牵张力可促进体外培养的新生及六周龄新西兰白兔兔鼻软骨细胞增殖活性,提示我们鼻软骨牵张方法不仅适用于临床矫治新生儿唇腭裂伴发鼻畸形,而且有可能用于矫治1岁左右婴儿甚至更大年龄患儿的唇腭裂伴发鼻畸形。
