The positive rate of streptococcus sobrinus of control group was 3.3% , there was significant difference between them ( P <0.05). Compared with those of the streptococcus mutans group, dmft, dmfs and somooth-surface caries of the streptococcus mutans and streptococcus sobrinus group were statistically significant( P <0.01). Conclusions: Streptococcus sobrinus cooperated with streptococcus mutans in caries process and it has relation with the rampant caries and smooth-surface caries of primary teeth.

目的:探讨远缘链球菌与乳牙龋病的关系。方法:采有TYCSB培养基作细菌筛选,运用分子遗传学手段即DNA的G+ C mol% 测定作最终鉴定。结果:患龋儿童组与无龋儿童组变形链球菌检出率有显著差异(P<0 .01),远缘链球菌检出率有显著差异(P< 0.05) ;患龋儿童组中能同时检出变形链球菌和远缘链球菌的群体,其龋失补牙数、龋失补牙面数及平滑面龋数均与只能检出变形链球菌的群体有显著差异( P< 0.01) 。结论:变形链球菌是龋病的主要致病菌,远缘链球菌对龋病过程有协同作用,与乳牙平滑面龋的发生、发展有关。

Objective To detect and distinguish Streptococcus mutans (S.mutans) and Streptococcus sobrinus (S.sobrinus) quickly in epidemiology and investigate the distribution of S.mutans in the oral of children with rampant caries.Methods Designed minor groove binder (MGB) probes according to the gtf gene of S.mutans and S.sobrinus. Detected 9 reference strains of Streptococcus mutans group by MGB probes in real time and after cultivation. Evaluated the results of these two methods. 92 dental plaques from pre-school children with rampant caries were detected in real time with MGB probes.Results The primers could amplify the target sequences specificity and distinguished S.mutans and S.sobrinus from each other using MGB probes. Though the fluorescence occurred earlier in S.mutans than in S.sobrinus, they had the same results in nature. In 92 children with rampant caries, the detective ratio of S.mutans was 96.7% and that of S.sobrinus was 32.6%. All the samples which could detect S.sobrinus were positive for S.mutans. Conclusion The primers and probe designed from gtf genes of S.mutans and S.sobrinus can amplify the target sequence and distinguish them from each other in real time.

目的 寻求一种快速检测变形链球菌(S.mutans)和远缘链球菌(S.sobrinus)的方法,研究变形链球菌群在 儿童猛性龋患者口中的分布。方法 设计并合成针对S.mutans和S.sobrinus葡糖基转移酶基因(gtf)的特异性引 物和小沟聚合物探针(MGB探针),对9株变形链球菌群参考菌株直接提取DNA及扩增纯培养后提取DNA分别进 行检测,比较二者检测结果的异同。采集92例猛性龋儿童菌斑样本,用MGB探针进行实时检测。结果 采用MGB 探针可以特异性地鉴别S.mutans和S.sobrinus,直接检测和扩增纯培养后的定性检测结果完全一致,前者荧光出现 的时间略迟。92例猛性龋患儿中S.mutans检出率为96.7%,S.sobrinus检出率为32.6%,所有检出S.sobrinus的菌 斑样本均可检出S.mutans。结论 采用特异性MGB探针方法可以对菌斑中S.mutans和S.sobrinus进行实时检测, 提高检测效率。

Objective To find out horizontal transmission of oral Streptococcus mutans(S.mutans) in nursery children through analyzing the similarity of S.mutans[WTBZ] genotypes.Methods The study group included 24 nursery children between 3 and 4 years of age.Dental plaque samples were collected with sterile toothpick and cultured on MSB plates for 48 h.Individual Streptococcus mutans group colonies representative of the colonial morphologies were subcultured on TPY plates. These strains were identified to species level biochemically.AP_PCR fingerprinting was preformed after identification.S.mutans isolates from different children with very similar fingerprinting profiles were examined by chromosomal DNA fingerprinting analysis.Results Streptococcus mutans group were isolated in oral cavities of 66.7% children,58.3% in caries_free and 75.0% in caries children.A total of 46 S.mutans isolates from 24 subjects were analyzed by AP_PCR,and 29 different amplitypes were identifyied,45.8% carried two genotypes.There were 2 genotypes of S.mutans isolated repeatedly among 12 nursery children.Conclusion The presence of matching genotypes of S.mutans among nursery children suggests horizontal transmission.

目的通过对幼儿园全托儿童之间口腔变形链球菌水平传播的研究,为儿童龋病预防提供新的思路。方法选择24名3~4岁全托儿童为研究对象,其中无龋组和有龋组各12名。采用无菌牙签采集牙面菌斑,厌氧培养48 h,根据变形链球菌的典型形态挑取单个菌落进行次代纯培养,经形态学和微量生化鉴定后将分离的菌株行AP_PCR扩增,对来自不同个体基因型相似的菌株再进行DNA指纹分析。结果24名受检儿童中,变形链球菌检出率为66.7%,无龋组和有龋组检出率分别为58.3%和75.0%。经AP_PCR扩增,24名儿童口腔中检测出29个不同基因型的变形链球菌,其中携带2个以上基因型儿童占45.8%。经DNA指纹分析,有2种基因型在11名全托儿童口腔中有重复检出。结论变形链球菌在全托儿童口腔中可能存在水平传播。
