The pedicle screws hook plates (PSHP)and the operative technique were designed to reduce slipping and fix the vertebral arch only. The main procedures of the operation include: clear fibrous tissue of the defect, reduction of the slipped vertebral body and fixation of the lytic arch, decompression of the nerve roots and bone grafting bilaterally of the defect. From July, 1993 to July, 1994, 12 patients were operated, the average age was 38 years. The percentage of slipping (PS) , the angle of slipping ( AS ) and the disc index (DI) measured on the roentgenograms between pre and postoperatively were compared. The PS decreased obviously in postoperation and the average of correction was 66%, the AS was closed to the normal angle of the disc, the DI increased in some extent. The avesage follow-up period was 6.3 months. Clinical evaluation was done with the score rating system of the Japanese Orthopaedic Association(total 29 points), preoperative average score 17.3 and follow-up average score 26.8. The improvement rate was 81.2%.

Aim: To evaluate the rigidity of compressed screw of the cortical bone (CSCB) compared with the others four common technique of internal fixation and testify about experimental data in mading a choice of the internal fixation of olecranon fracture. We first reported 24 patients of olecranon fvacture by CSCB. Results: The rigidity of internal fixation of the wires was the smallest and the hook plate was the biggest. The intraedullary cancellous screw fization gave little reliable result. CBCS was suporior to tension hand wiring at oblique osteotomies (P<0. 01), but no statistically significant difference was shown between CBCS and tension hand wiring at transvers osteotomise (P>0. 05). Comminuted osteotomies were held most rigidity by hook plate (P<0. 01). CBCS has been used in 24 patients and 22 patients of olecranon fracture were followed up with an average period of 14 months affer internal fixation with CBCS. Complete bony healing was foud in all patients and 90. 91% of the patients were excellent and good. Conclusion: Because of its reriable fixation, easy to insert and less damages, CBCS is the ideal chioce of the internal fixation of the olecranon fracture at transverse and oblique fractures.

比较尺骨鹰嘴骨折加压螺钉与常用的4种内固定的强度,为临床选择内固定方法提供实验依据。用22具新鲜尸体肘关节做成横型、斜形和粉碎形鹰嘴骨折模型。用5种内固定后置于MD—10E万能电子力学试验机上,测定骨折处的位移与压力的关系;结果:钢丝内固定强度最小,钩钢板最大。髓内加压螺钉固定欠可靠。横形骨折皮质骨加压螺钉与张力带钢丝比较,统计学上无显著差异(P〉0.05),斜形骨折皮质骨加压螺丝钉强于张力带钢丝(P〈0.01)。粉碎形骨折使用钩钢板尤佳(P〈0.01)。临床应用24例中,22例平均随访14个月。骨折全部愈合,优良率达90.91%;认为皮质骨加压螺丝钉治疗横、斜形尺骨鹰嘴骨折,创伤小,操作简便,固定确切,是一种安全可靠的方法。

Objective To study the curative effect of surgical treatment of degree Ⅲ acromioclavicular joint dislocation using the clavicular hook-plate (CHP). Methods Twenty-four patients were from Beijing Chaoyang Hospital, Capital University of Medical Sciences, 21 patients were from Beijing Luhe Hospital. The CHP fixation was used to treat 45 cases of Allman degree Ⅲ acromioclavicularjoint dislocation. The broken acromioclavicular ligament was repaired directly in all cases. The broken coracoclavicular ligament was repaired directly in 19 cases and reconstructed with coracoacromial ligament in 26 cases. Results Out of 45 cases,41 cases had been followed up for 3-47 months (average 26 months). All the dislocated acromioclavicular joints returned to normal level 15-20 days after the treatment. All the patients could do their work and manual labour after a mean period of 3 months (2-4 months). There was no redislocation, plate breakage or screw loosening after CHP was taken away in 3-10 months (average 5 months). Excellent rate was 80.5% (33/41) , effective rate 14.6% (6/41),poor rate 4.9% (2/41), excellent plus effective rate 95.1%(39/41)according to Karlsson result evaluation. Conclusions The acromioclavicular joint can exercise earlier and recovered quickly when degree Ⅲ dislocated acromioclavicular joint is treated with CHP. The curative effect has been proved satisfactory.

目的 探讨AO锁骨钩钢板 (clavicularhook -plate ,CHP)治疗Ⅲ度肩锁关节脱位的疗效。 方法  4 5例患者资料来源于北京首都医科大学附属朝阳医院骨科 (2 4例 )和北京潞河医院骨科 (2 1例 )。均为AllmanⅢ度肩锁关节完全性脱位 ,应用CHP固定治疗。所有病例均直接修复断裂的肩锁韧带 ,其中 19例直接修复断裂的喙锁韧带 ,2 6例用喙肩韧带代替重建无法直接修复的喙锁韧带。 结果 获随访 4 1例 ,随访时间 3~ 4 7个月 ,平均 2 6个月。术后 15~ 2 0d ,肩关节活动基本正常。患者恢复工作和体力劳动为术后 2~ 4个月 ,平均 3个月。取出CHP时间为 3~ 10个月 ,平均 5个月。无钢板断裂、螺钉松动和再脱位。按Karlsson疗效评价标准 ,优占 80 .5 % (33 4 1) ,良占 14 .6 % (6 4 1) ,差占 4 .9% (2 4 1)。优良率达 95 .1% (39 4 1)。 结论 CHP治疗Ⅲ度肩锁关节脱位 ,可早期活动肩关节 ,关节功能恢复快 ,疗效满意。
