Isometric exercise (handgrip) was performed in 31 cases of hypertensive left ventricular hypertrophy(LVH) after combined treatment with nifedipine and atenolol for a mean period of 15 months .Left ventricular mass index (LVMI),cardiac systolic and diastolic function were determined by M mode echocardiography with two dimensional echocardiography monitoring.Handgrip induced average 18±4 mmHg increases of mBP significantly deteriorated cardiac systolic and diastolic function in patients (n=10) with remaining LVMI greater than 120 g/m 2,evidenced by decreases of SBP/ESV,FS,EF,and MEF (P<0.01),respectively.In contrast,no changes of systolic and diastolic variables were found in patients (n=21)with satisfying regression of LVH and LVMI less than 120 g/m 2 after exercise.LVMI was negatively related to post exercise FS (r=-0.44,P< 0.01 ) and MEF (r=-0.57,P< 0.001 ).These data demonstrate that the response of cardiac performance to isometric exercise in patients with hypertensive LVH after reduction of left ventricular mass depends upon the completeness of reversal,and the greater the remaining LVMI,the worse would be the cardiac response to the abrupt increases of afterload.

To ascertain the harmfulness of hyperinsulinemia in patients with essential hypertension(HT),oral glucose tolerance test(OGTT) and insulin release test(IRT) were made on 140 patients with HT.Each patient underwent cardiovascular,neurological and nephrological examination,including echocardiography,Holter′s monitoring,brain SPECT,CT,blood and urine analysis (albumin/24 h urine,creatinine clearance rate,β\-2 microglobulin).The HT was divided into 3 groups∶group 1,HT with normal OGTT and IRT (n=27,19.3%);group 2,HT with normal OGTT and hyperinsulinemia(HIS)(n=88,\{62.8%\});group 3,HT with impaired OGTT and HIS (n=25,\{17.9%\}).Another 52 cases of HT complicated with overt NIDDM served as group 4.It was found that the occurrence rate of nephropathy and neurological diseases (hemorrahge,infarction and TIA) increased steadily from group 1 to group 4,with no difference between group 3 and 4.Despite no difference in LV mass index among the 4 groups,the occurrence rate of impaired cardiac systolic and diastolic function,frequent ventricular and atrial ectopic beats increased from group 1 to group 4.The occurrence rate of target organ damage in HT with HIS is intermediate between HT without HIS and HT complicated with overt NIDDM.In view of organ damage,HT with HIS may be a transitional syndrome from simple HT to HT complicated with NIDDM.

对 14 0例高血压 (HT )病人进行口服葡萄糖耐量试验(OGTT)和胰岛素释放试验 (IRT )。每个病人进行心、脑、肾检查 ,包括超声心动图、2 4h动态心电图、脑 SPECT、CT检查、2 4h尿白蛋白定量、内生肌酐清除率、血、尿β2 -微球蛋白。分为 3组 :1组 ,H T并正常 OGTT和 IRT(n=2 7,19.3% ) ;2组 ,H T并正常OGTT和高胰岛素血症 (H IS) (n=88,6 2 .8% ) ;3组 ,HT并糖耐量低下和 HIS(n=2 5 ,17.9% )。另外 5 2例 H T并 型糖尿病为 4组。发现肾病和脑血管病 (出血、梗塞和短暂性脑缺血发作 )从 1组至 4组逐渐增加 ,3组和 4组之间无显著性差异。4组间左室重量指数无差异 ,心脏收缩和舒张功能减退 ,频发室早和房早的发生率从 1组至 4组递增。说明 H T并 H IS病人的靶器官损害发生率是介于 H T不并 HIS和 H T并 型糖尿病之间的中间类型。依器官损害观点 ,高血压并 H IS可能是单纯高血压 -高血压并 型糖尿病的一种过渡症侯群
