According to Okamotos method,assayed the hepatitis C virus genotypes of 28 patients with hepatitis C by PCR,18 patients genotypes were HCV RNA Ⅱ genotype,10 patients genotypes were HCV RNA Ⅲ genotype.Especially studied the effects of therapy with α 2b IFN on humoral immunity and cellular immunity in patients with HCV RNA Ⅱ genotype or with HCV RNA Ⅲ genotype.The results showed that,before treatment,no strong evidence for a difference in humoral immunity:IgG,IgM and IgA,or in cellular immunity:percentage of CD4 + T subsets,percentage of CD8 + T subsets,CD4/CD8 ratio was observed between patients with HCV RNA Ⅱ genotype and patients with HCV RNA Ⅲ genotype(P>0.05).CD4/CD8 ratio of patients with HCV RNA Ⅲ genotype was higher than that of patients with HCV RNA Ⅱ genotype at the 8th week of interferon therapy(P<0.05).Both percentage of CD4 + T subsets and CD4/CD8 ratio of patients with HCV RNA Ⅲ genotype were significantly higher than that of patients with HCV RNA Ⅱ genotype at the 12th week of interferon therapy(P<0.01).At the 4th week after interferon therapy,percentage of CD4 + T subsets and CD4/CD8 ratio of patients with HCV RNA Ⅲ genotype were also significantly higher than that of patients with HCV RNA Ⅱ genotype(P<0.01).During and after interferon therapy,the humoral immunity:IgG,IgM,IgA and percentage of CD8 + T subsets of patients with HCV RNA Ⅱ genotype or with HCV Ⅲ genotype had some differences,but the differences were not satistically significant(P>0.05).The results indicated that,during and after interferon therapy,patients with HCV RNA Ⅲ genotype tended to have higher percentage of CD4 + T subsets and higher CD4/CD8 ratio than patients with HCV RNA Ⅱ genotype,this may be one of causes why the effects of therapy with interferon in patients with HCV RNA Ⅲ genotype were better than in patients with HCV RNA Ⅱ genotype.

根据Okamoto基因分型法对28例丙肝患者的病毒基因进行了分型,18例为HCV-Ⅱ型,10例为HCV-Ⅲ型。重点研究了α-2bIFN治疗HCV-Ⅱ型患者和HCV-Ⅲ型患者免疫功能的不同改变:在α-2bIFN治疗前,HCV-Ⅱ型患者和HCV-Ⅲ型患者之间IgG、IgM、IgA、CD4百分比、CD8百分比及CD4/CD8比值6项指标无明显差别。α-2bIFN治疗8w时,HCV-Ⅲ型患者CD4/CD8比值明显高于HCV-Ⅱ型患者CD4/CD8比值。α-2bIFN治疗1个疗程(12w)时,HCV-Ⅲ型患者CD4百分比,CD4/CD8比值明显高于HCV-Ⅱ型患者。α-2bIFN治疗1个疗程后第4周(16w)时,HCV-Ⅲ型患者CD4百分比,CD4/CD8比值仍明显高于HCV-Ⅱ型患者,但在整个治疗过程中,HCV-Ⅲ型和Ⅱ型患者IgG、IgM、IgA和CD8百分比4项指标改变无显著差异。α-2bIFN治疗HCV-Ⅲ型患者,其CD4百分比,CD4/CD8比值改变明显高于HCV-Ⅱ型患者。这可能是α-2bIFN治疗HCV-Ⅲ型患者疗效明显好于HCV-Ⅱ型患者重要原因之一。

A study was described for identifying hepatitis C virus genotypes by restriction endonuclease cleavage of sequence amplified by polymerase chain reaction (PCR) from 5'-non-coding region,that the nucleotide sequence was from region 28 to 284 (HCV-J), GenBank, D10750. Using the enzymes of both Hae Ⅲ/Rsa I and of both Hinf I /Mva I, followed by cleavage with BstUI or ScrFI, according to the showed variant genotypes above. It was possible to identify and distinguish HCV genotypes as la, 1b, 2a, 2b, 3a, 3b, 4a and 6a. The method was used to investigate the prevalence of those genotypes in 38 cases of HCV infection in our hospital. All of the samples showed that they were genotype 1b, which was the same as what we have reported before in 1994. The method and its key innuence factors were also discussed.

丙型肝炎病毒(HCV)的基因分型在临床和流行病学的研究方面有重要意义。以多聚酶链反应的方法扩增HCV的5’-非编码区第28至284区段(HCV-J株),并以复合限制性内切酶水解的方法按Simmonds基因分型的标准,将HCV分为6型和若干亚型。结果:检测标本38份,均为1b型,与以往的检测结果类似。

Objective To analyse the frequency of various hepatitis C virus genotypes in different populations with HCV infection from Guizhou area.Methods Ninty eight HCV isolates obtained by using RT nested PCR from 37 patients with chronic hepatitis C (PHCs), 15 patients with hemopathy (PHs), 12 blood donors (BDs) and 34 intravenous drug users (IVDUs) were genotyped by using the second generation line probe assay. Results Among 37 HCV isolates from PHCs, there were 34 (91.9%) genotype 1b, 3 (8.1%) mixted genotypes (1 for 1a+1b, 1 for 1b+2, 1 for 1a+1b+2a/2c, respectively). Among 15 PHs, 14 (93.3%) were type 1b, 1(6.7%) were type 1b+2, among 12 BDs, all(100.0%) were type 1b. Among 34 IVDUs, 13(38.2%) were type 1b,10(29.4%) were type 6a, 1(2.9%) was type 3a, 4(11.8%) were type 3b and 6(17.6%) were mixted types(2 for 1a+2, 1 for 1a、1b+2, 1 for 1b+3b, 1 for 3b+2, 1 for 1b+6a, respectively).Conclusion The results showed that HCV subtypes 1a. 1b, 2a/2c, 3a, 3b and 6a belonging to 4 HCV genotypes(1,2,3,6) were found from different populations in Guizhou, of these, 3a, 3b and 6a were firstly reported. HCV 1b was commonly found in BDs, PHs and PHCs and its frequency was above 90.0%, 3a, 3b, 6a were only found in IVDUs and their total frequency was above 50%.

目的 分析贵州地区丙型肝炎病毒(HCV)感染株的基因型。方法 用第二代线探针核酸杂交方法,对98 例HCV感染者血清进行基因分型。结果 12 例献血员血清,均为HCV1b 型;15 例血液病患者中,14 例(93-3% )为HCV1b ,1 例(6-7% )为HCV1b + 2 型。慢性丙肝患者37 例中,HCV1b 型34例(91-9% ) ,混合感染基因型3 例(HCV1a + 1b 、HCV1b + 2 、HCV1a .1b + 2a2c 各1 例)。静脉药瘾者34 例,其中HCV1b 感染13 例(38-2% ) ,HCV6a10 例(29-4% ) ,HCV3b4 例(11-8% ) ,HCV3a1 例(2-9 %) ,混合感染6 例(17-6 %) ,大多为HCV2 复合其他基因型。结论 贵州地区HCV感染者中,以HCV1b 基因型为主,其次为HCV6a ,尚存在HCV3a 和HCV3b 。其中HCV3a、3b 、6a ,在本地区系首次报告,但主要存在于静脉药瘾者中。虽然有混合感染基因型的存在,但其基因基础需测序分析
