Objective: To measure the cervical spinal curvature by the method of arc degree for the sake of establishing a reference marker for the clinical assessment of cervical spine function and providing the radiologic data for designing of cervical intervertebral fusion ware. Methods: The length of cervical spinal curving strings and arcs were measured on middle potential lateral radiographs in 378 normal persons cervical spines by computer imaging processing system, and the physiological curvature were figured out through solution of dualistic once equation .Results: The average value of cervical spinal curvature was (34.57±5.26)°and normal value scope ranged from 24.27°~44.88° in 95% persons. No statistically significant difference between males and females ( P >0.05). Cervical spinal curvature was larger in low age group (11~30 years) than other groups and decreased following achievable age. There were obviously change in group of 31~60 years, furthermore curve increase occurred in senility group (60 years above ). Conclusions: There is a relativity that have no linearity correlation between the cervical spinal curvature and age. The unbalance of foundation mechanics results in the variation of cervical spine curvature. To maintain and restore the cervical spinal curvature is profitable for preventing and curing cervical vertebral diseases.

目的 :为临床颈椎功能评定建立参考指标并为颈椎椎体间融合器的研制和应用提供依据。方法 :采用计算机图像处理技术 ,对 3 78例正常人体颈椎中间位侧位片弦、弧长度进行测量 ,通过二元一次方程求解计算出正常颈椎生理曲度。结果 :男女颈椎生理曲度平均值为 ( 3 4.5 7± 5 .2 6)° ,95 %的正常值范围为 2 4.2 7°~ 44 .88° ,男女组之间无统计学差别 (P >0 .0 5 )。低年龄组 ( 11~ 3 0岁 )颈曲较大 ,随年龄增长颈曲减小 ;3 1~ 60岁之间男女颈曲呈波动变化 ;高龄组 ( 60岁以上 )颈曲随年龄的增加又有增大趋势。结论 :人体生理颈曲与年龄存在相关性 ,但非线性相关 ;颈曲的变化是颈椎基础力学失衡的结果 ,维持和恢复颈椎生理曲度对预防和治疗颈椎疾患有积极作用

Objective To investigate criteria for health evaluation of spinal column in fighter pilots by image findings and clinical evaluation. Objective To investigate criteria for health evaluation of spinal column in fighter pilots by image findings and clinical evaluation. Methods Forty six fighter pilots aged 25~45, were involved in this investigation. A specially designed grading system for clinical evaluation of spinal column was used to collect the quantified clinical data. A-P view and lateral view X-rays film of whole spine, MRI scanning of cervical vertebrae, CT scanning of lumbar vertebrae were taken for determining quantified curvatural, structural and degenerative changes of spinal column. The correlations between clinical evaluation and image findings were determined statistically. Results ① Age and flying time are significantly related with degree of clinical manifestation and image structure and degenerative changes.② Abnormal cervical spinal curvature and scoliosis are common in fighter pilots, but they are not correlated with degree of clinical manifestation. ③ The indexes of spinal degeneration (including narrowing of intervertebral space, disc protrusion, wedging of vertebra and osteophyma) are correlated each other in certain extent, but do not related with degree of clinical manifestation. Conclusion The degree of clinical manifestations of spinal column in fighter pilots does not parallel with structural and degenerative changes from imaging diagnosis. The evaluation criteria of spinal column simply based on structural and degenerative changes from imaging diagnosis alone should be reconsidered.

目的 探讨歼击机飞行员脊柱健康的评价标准。 方法 顺序采集46 名歼击机飞行员特别设定的体检资料,包括脊柱临床评价,全脊柱立位X线前后位及侧位摄片,腰椎双斜位摄片,颈椎MRI,腰椎CT等资料。将脊柱的生理曲度及异常曲度情况,结构退变情况和脊柱临床评价都给予量化,并进行统计分析,比较各项指标之间关系。 结果 ①飞行员的年龄和飞行时间与其颈椎和腰椎问题的大小都具有显著的相关性。②颈椎曲度异常和脊柱侧弯情况普遍存在,但与脊柱临床评价并无相关性。③脊柱退变的各项指标(包括椎间隙狭窄、椎间盘突出、椎体楔形变、椎体骨刺等)之间都有一定的相关性,但与脊柱的临床评价并不相关。 结论 歼击机飞行员的脊柱健康的临床评价与其结构形态的退行性改变并不平行;单纯以脊柱结构的改变来确定飞行员脊柱健康的标准值得商榷。

Objective:To study the etiological factors and mechanism of the abnormality of untraumatic cervical spinal curvature in the youth.Method:Cases of 123 young outpatients with abnormality of untraumatic cervical spinal curvature were reviewed retrospectively.The necessary history talking and physical examinations were made to gain the information about the patients' employment,working and living condition,posture of sleeping,the height of sleeping-pillow and the continuous head lowering time.The degree of the abnormality was estimated by measuring the depth of cervical vertebra bow from the lateral radiograph.Result:The depth of cervical vertebra bow was related closely to the head lowering time each day,the working and living conditions,the height of sleeping-pillow.Conclusion:The abnormality of untraumatic cervical spine curvature is related to the employment situation,the working environment and living habit.Dynamic disequilibrium of cervical spine is one of the main mechamism leading to the abnormality of untraumatic cervical spine curvature.

目的:探讨青年出现非创伤性颈椎生理弧度异常的可能病因和机理。方法:收集门诊非创伤性颈椎生理弧度异常的青年患者123例,通过询问病史和体格检查,了解职业类别、工作生活环境、睡眠姿势和枕头高度、每天低头时间的长短和每次低头时间长短等情况。同时在颈椎侧位片上测量颈椎弓深,以评价颈椎生理弧度异常的严重程度。结果:青年颈椎弓深与每天低头时间、每次低头持续时间、工作生活环境、睡眠枕头高度等存在密切相关性。结论:青年非创伤性颈椎生理弧度异常与职业性质、工作环境、生活习惯等有关,颈椎动力性平衡系统失调是导致青年非创伤性颈椎生理弧度异常的主要机理之一。
