Objective To analysis of the pace and discuss the evoked potential of lower limbs spasm after brain injury treated with SPR before and after operation. Methods The pace analysis and evoked potential were detected 3 days before operation. The posterior root of spinal cord of spasm lower limbs were divided into branches and cut down the lower threshold branch after detected during operation. The ratio of cut root, muscle tension, myodynamia, body weight, muscle function were estimated and all of them <30%. Results The remove rate of spasm was 90% and function improvement rate was 80%. The pace improvement was significant changed better (p<0.01). Conclusion The muscular tension regulation of "large and small circle" theory was supported by the evoked potential before and after operation. The selected resection of posterior root could treat the limb spasm after adult brain injury and obtained satisfactory effects.

目的 采用选择性脊神经后根切断术 (SPR)治疗成人脑外伤后下肢痉挛 16例 ,对术前术后进行步态分析及诱发电位研究。方法 术前 3d进行步态分析及诱发电位测定 ,将痉挛下肢的L2 ~S1的脊神经后根进行分束 ,电刺激仪测阈值将阈值较低神经束切断。神经根切断比例结合肌张力、肌力、体重及肌群功能进行量化 ,均 <30 %。结果 本组 16例随访 4年 ,痉挛解除率 90 % ,功能改善率 80 %。术后步态明显改善 ,有显著差异P <0 0 1。手术前后诱发电位测定支持肌张力调节的“大、小环路”理论 ,支持成人脑外伤后肢体痉挛的机制。结论 选择性脊神经后根切断术能较有效的治疗成人脑外伤后肢体痉挛
