Purpose To evaluate the features of COPD in dynamic pulmonary CT scan. Materials and methods Thirty two COPD patients, verified by clinical and pulmonary functional tests(PFT) were scanned by electron beam CT at end inspiration and expiration in continuous volume scan (CVS) mode. The control group consisted of 24 young healthy men, with normal plain chest film and PFT results. The lung was divided into three equal regions to measure and quantify the lung attenuation, volume and the ratio of pixels<-900HU of the whole lung and different parts (pixel index, PI). Results The CT value of the whole lung at end inspiration in COPD patients was -819HU, similar to that of the control group -816HU, but obviously lower at end expiration than control group (-751HU VS -619HU). The gradient between the two phases were 68HU and 197HU respectively. At end expiration phase, the lung volume of COPD patient decreased by 25.90%, only half of that of control group (54.90%). Twenty eight (87.50%) of COPD patients had pixels lower than -900HU with mean 10.76%. The number in control group was 4(16.67%) with mean 0.12%. Conclusion The COPD patients have obvious airtrapping as compared with the normal group, which can be diagnosed and quantitated by pulmonary dynamic CT, which seems to be a promising method.

目的评价慢性阻塞性肺部疾病(COPD)在肺部电子束CT(EBCT)动态扫描时的表现。材料与方法对32例临床和肺功能检查确诊的COPD病人及正常对照组24例健康男性,使用EBCT分别进行了不同呼吸时相的连续容积模式扫描,将全肺等分为上、中、下三个肺区后测量和计算肺野密度和体积值,以及像素值<-900HU的肺区所占比例,即像素指数(PI)。对两组的结果作了比较。结果COPD组在深吸气末的CT值与正常组相似,分别为-819HU和-816HU,但在深呼气末时则明显低于正常组(分别为-751HU和-619HU),吸、呼气间的差值两组分别为68HU和197HU。在呼气CT上COPD组体积仅缩少25.90%,为正常组(54.90%)的一半,COPD组呼气CT上87.50%检出了<-900HU的像素,其PI值平均为10.76%,正常组仅4例(16.67%),平均PI值为0.12%。上述表现提示COPD患者中多数有空气贮留。结论COPD患者较正常人有明显的空气贮留,可用肺部动态CT扫描加以诊断和定量,后者是一种可取的,有发展前途的检查方法。

To make the qualitative dimensonless index characteristics value being of nonlinear mearings,the method for determining the qualitative dimensionless index characteristics value were discussed by using nonlinear fuzzy classified standard of index characteristics value. To embody experts experience and judgement sufficiently,the experts can give region number on evaluating value for some index,and then obtain the nonlinear index characteristics value by using fuzzy mathematical method of synthesis analysis. Examples show that the nonlinear fuzzy handling method is better than the linear fuzzy handling method.

对评价指标特征值采用等区间的划分方法以确定危险等级,忽略了指标特征值在一定条件下作进一步改进时的难易程度,具有明显的不合理性·为解决这一问题,利用模糊数学方法给出了定量指标特征值及其无量纲特征值的非线性模糊处理模型·为了使定性指标无量纲特征值的确定具有非线性意义,在采用定量指标无量纲特征值的非线性模糊分级标准的基础上,探讨了定性指标无量纲特征值的确定方法·为了充分体现专家的经验和判断,专家在为某一指标赋值时可给出区间数,然后运用模糊数学的综合分析法,以获得该指标的无量纲特征值·实例表明,对危险评价指标特征值及无量纲特征值的非线性模糊处理优于线性处理,它使危险评价结果更为符合实际·
