Objective To investigate the factors relative to the constipation in orthopedic inpatients and prevent the occurrence of constipation. Methods By using single factor anslysis and logistic regression analysis, the factors relative to constipation in 92 orthopedic inpatients were investigated. Results In this group, the incidence of constipation was 60.87 %. Single factor analysis revealed that the following variables were associated with constipation: amount of staple food, vegetable fruit, drink, the time of food uptake and the position of fracture. Logistic regression analysis demonstrated that the independent factors upon constipation were amount of vegetable and fruit uptake (preventive factor) and the time of food uptake (risk factor) after operation. Conclusion Pospoperatively enough amount of vegetable and fruit uptake can prevent the occurrence of constipation.

目的探讨骨科卧床病人便秘的相关因素 ,寻找预防病人便秘的措施。方法采用前瞻性调查及单因素与多因素的Logistic回归分析对 92例病人进行研究。 结果本组病人便秘发生率为 6 0 .87%。经单因素分析发现病人术后进食时间、蔬菜量、水果量、主食量、饮水量及疾病部位均与病人的便秘有关 ;Logistic回归分析显示仅术后进食时间、蔬菜量、水果量是与病人的便秘显著相关的独立因素 ,存在一定的因果关系 ,其中术后进食时间是便秘的危险因素 ,蔬菜量、水果量是便秘的预防性因素。结论术后让病人尽快恢复饮食 ,保证充足蔬菜量、水果量有利于便秘的预防。

Objective To discuss the risk factors and corresponding nuring countermeasure for compressive common peroneal nerve paralysis in orthopedic inpatients. Methods From May 1994 to December 2001, 7 orthopedic inpatients who suffered from compressive common peroneal nerve paralysis were analysed retrospectively, emphasized on the etiology. Results Total of the seven patento, 3 cases were injuried during functional exercise after peri - knee joint operations, 1 case with proximal tibia and fibula fracture was appeared during fixation by plastic brace, 2 cases were suffered in adjuvant traction using orthopdeic operating - table, 1 case was wounded on the healthy leg after arthroscopy. Five patients recovered completely and 2 patients got major reconery after conservative treatment. Conclusion nursing is one of the important risk factors for the development of compressive common peroneal nerve palsy. Understanding of the anatomical basis and risk factors, adopting of corresponding countermeasure may be useful to avoiding this complication.

目的 探讨骨科院内压迫性腓总神经麻痹的成因和护理预防对策。方法 回顾性复习1994年5月至2001年12月发生的7例骨科院内压迫性腓总神经麻痹,分析其发生原因。结果 7例病例中,其中3例发生于膝关节周围手术后功能锻炼过程中,1例系腓骨颈、胫骨平台骨折应用支具固定后,2例系骨科床牵引辅助复位者,1例为关节镜检截石位者。所有病例均接受保守治疗,经至少1年以上随访,预后良好。结论 护理因素在骨科院内压迫性腓总神经麻痹的发病中占一定地位,在了解其解剖学机理和成因后,采取针对易发因素的护理预防对策,在避免此病的发生具有非常积极的意义。

Objective: To study the effect of diet education on preventing constipation among orthopedic inpatients, and the relationship between diet education and food-taking pattern of postoperative patients. Methods:116 orthopedic inpatients who have been operated on spine, hip joint or lower extremity were randomly divided within 2 groups, which including study group ( n =57) and control group( n =59).Patients in study group were given diet education preoperatively and in the second day after operation. Patients in control group were given no intervention. The data collected include the sorts and amounts of food taken in the six days after operation and defecation score. Results: The amount of staple food in the first and second day after operation, and the amount of vegetable in the third day after operation taken by patients in study group higher than those taken by patients in control group(study group vs. control group: t =2.33,2.02,2.02, all P <0.05).Defecation scores of patients in study group(3.06±3.61) lower than those of patients in control group(5.04±3.62, study group vs. control group: t =2.99, P <0.05). The possible factors related to constipation were analyzed with multivariate linear stepwise regression analysis. The results showed that only the amount of staple food in the first day after operation was negatively related to constipation. Conclusion: Diet education can prevent constipation of these patients after operation. The inadequate amount of staple food and vegetable was the main unreasonableness that exists among these patients. So the adequate amount of staple food and vegetable should be emphasized when diet education is given and we should take the condition of patients' taking food more serious in the first day after operation.

目的 :探讨饮食教育对骨科术后病人进食结构的影响 ,预防便秘的发生。方法 :将骨科择期手术的 116例术后病人随机分为实验组 5 7例 ,对照组 5 9例。实验组于术前 1d及术后第 2日分 2次接受专门的饮食教育 ,对照组接受常规指导。记录病人术后 6d内的进食种类、量及排便情况。结果 :实验组术后第 1、2日主食进食量及第 3日蔬菜进食量均明显高于对照组 (t =2 .33、2 .0 2、2 .0 2 ,P均 <0 .0 5 ) ;术后便秘评分 ,实验组 3.0 6± 3.6 1,对照组 5 .0 4± 3.6 2 (t=2 .99,P <0 .0 5 ) ;与便秘相关的多因素的多元线性逐步回归分析显示 ,术后第 1日主食量与术后便秘评分呈直线负相关。结论 :对病人进行饮食教育能够有效降低骨科术后病人便秘评分 ,即饮食教育能够降低术后便秘的发生 ;饮食结构不合理可能主要由术后主食类、蔬菜类的量不足造成。因此进行饮食教育时应强调术后适宜的主食量和蔬菜量 ,特别应重视病人术后第 1日的进食情况。
