Objective To provide anatomic basis for the donor area of vascularized allogeneic bone transplantation. Objective To provide anatomic basis for the donor area of vascularized allogeneic bone transplantation. Methods The nutrient vessels of the humerus, radius,femur and tibia were observed on 40 sides of adult cadaveric upper and lower limbs perfused with red latex in artery respectively. Dried nutrient foramen was observed in the above donor site bone of 50 adult dry bones. Results There were 4 to 12 nutrient foramen in the proximal humeral metaphysis.The anterioror posterior circumflex arteries were the main nutrient vessels.The location of nutrient foramen of the humeral diaphysis was mostly in the anterior and medial side of the middle humerus.And the nutrient vessel came from humeral artery.The nutrient vessel of the lower end of radius mainly supplied by anterior interosseous artery.There were l to 3 nutrient foramen in the femoral diaphysis and its nutrient artery originated from the perforating branches of profound femoral artery.Nutrient foramen of the tibial diaphysis constantly located in the posterior of proximal and middle tibia.Its nutrient vessel came from the branches of posterior tibial artery. Conc1usion The donor bone in accordance with the recipient area bone in the appearance should be chosen according to the large segment bone defect of long bone.The length and diameter of above vessels are suitable for the anastomosis of microsurgery. 目的 为选用吻合血管同种异体骨移植的供区提供解剖学依据。方法 选用经动脉灌注红色乳胶的成人上、下肢标本各 40侧 ,着重对肱骨、桡骨、股骨和胫骨的滋养血管、进行观测 ;另选上述供骨区成人干燥骨各 5 0根 ,作滋养孔 (或称管 )观察。结果 肱骨上端有 4~ 12个滋养孔 ,旋肱前、后动脉是其滋养血管主要来源 ;肱骨干滋养孔多位于肱骨中段前内侧面 ,滋养动脉多发自肱动脉 ;桡骨下端滋养血管主要由骨间前动脉及桡动脉供应 ;股骨干滋养孔 1~ 3个 ,滋养动脉来自股深动脉的穿支 ;胫骨干滋养孔恒定位于胫骨中、上 1/3段后面 ,滋养血管来自胫后动脉分支。结论 根据长骨大段骨缺损 ,选用在外形上与受区相匹配的供骨 ,上述血管长度、口径均适合显微外科的吻接要求

Objective To explore the clinical application of allogeneic bone transplantation. Methods Related literatures on clinical application of various allograft bone materials of the past 5 years were extensively reviewed. Results The allografts could be processed into various kinds of transplanting materials, such as small blocks, large segments, morselised or granular material, and bone ring, so that they could be applied to bone defect filling, prosthesis revision, limb preservation after tumor resection, and joint fusion. All these applications ended up with good clinical results. However, complications resulted in the structural transplantation of large block bones could be overlooked. Conclusion The main advantage of bone allograft is that it can be integrated with host bone and be remodelled, so as to reconstruct bone structure and function. How to accelerate their integration and remodelling while exerting normal functions is the problem that requires solution.

目的了解异体骨移植相关临床应用的现状和趋势。方法广泛回顾近5年国内外异体骨移植的临床应用。结果异体骨可加工成小块骨、大段骨、颗粒骨及骨环等生物特性各异的移植材料,广泛应用于骨缺损填充、假体翻修、肿瘤切除保肢和关节融合等领域,并获得良好效果;但大块异体骨结构性移植带来的各种并发症不容忽视。结论异体骨移植的主要优点是能够与宿主骨整合并塑形,可替代自体骨移植,实现骨结构和功能重建。如何加速整合、塑形及减少并发症将是其临床应用急需解决的重要课题。

Objective To investigate the efficacy of the small cryopreserved allogenic bone transplantation in the reconstruction of bone defect caused by bone tumor.Method From November 2000 to November of 2004,the small cryopreserved allogeneic bone transplantation was successfully used in 22 patients with bone defect resulting from the tumorectomy of four limbs.Results Follow-up ranged from 6 to 80 months with an average of 30 months.One patient suffered recurrence and underwent re-resection and re-transplantation.Fractures...

Objective To investigate the efficacy of the small cryopreserved allogenic bone transplantation in the reconstruction of bone defect caused by bone tumor.Method From November 2000 to November of 2004,the small cryopreserved allogeneic bone transplantation was successfully used in 22 patients with bone defect resulting from the tumorectomy of four limbs.Results Follow-up ranged from 6 to 80 months with an average of 30 months.One patient suffered recurrence and underwent re-resection and re-transplantation.Fractures of allograft bone appeared in three cases because of the failure of fixation and were treated with the second internal fixation.Porosis demonstrated by radiograms appeared in the other cases,and no infection appeared.Conclusions The small cryopreserved allogeneic bone transplantation is a good choice for repairing bone defect caused by the resection of benign bone tumor.It is essential to resect the tumor thoroughly,choose small cryopreserved allograft bones correctly,and use appropriate dosage of antibiotics.In addition,adequate fixation,operation skill and avoiding early weight bearing are important for the success of operation.

目的探讨小块冷冻异体骨治疗良性骨肿瘤切除术后骨缺损中的临床应用及疗效。方法自2000年11月至2004年11月应用打压植骨技术对小块异体骨进行打压植骨来治疗四肢长骨良性骨肿瘤手术切除后残留骨缺损病例22例。结果本组随访6~80个月,平均31个月。因肿瘤复发行肿瘤再切除植骨术1例,3例患者因术后未行有效固定,外伤后出现植骨区骨折,二次手术行内固定术。X线片显示,其余病例的骨缺损腔隙均有骨小梁通过,无感染病例。结论用小块异体骨移植充填肿瘤切除后的骨缺损具有良好的临床效果。彻底切除肿物、正确使用同种异体冷干骨、熟练掌握打压植骨技术、适当固定、应用抗生素及恰当的负重时机是手术成功的关键。
