Restrospective study was made on the 2 ages groups (3—15, and ages over 16) with analysis on the treatment methods and complications, The results revealed: faster union and less complications in the younger age group, with average hospitalization of 31 days, longer traction time and slower union in the other group with mean hospitalization of 46 days. Cases of internal fixation had averaged hospitalization of 26 days and faster bony healing. Internal fixation with intramedullary nail or plates with pressure used to have better results than with common steel plates. Patients under 15 years old could be better treated with traction. Patients over 16 could have better results with internal fixation. The golden time for operation is 24-48 hours after trauma, but 12-24 hours for complicated open fracture. Cases of fat embolism can be thus reduced.

自1979~1989年收治股骨干骨折病人290例,293个骨折。将回顾病例分为3~15岁和16岁以上两个组,分析其治疗与并发症。结果3~15岁组骨折愈合快,并发症也少,平均住院时间31天;16岁以上组牵引治疗骨愈合慢,平均住院46天。而内固定治疗平均住院26天,骨愈合也快,内固定中普通钢板并发症多,加压钢板和髓内针较理想。因此认为,15岁以下病人以牵引治疗为宜,16岁以上者以内固定为好,争取在24~48h完成,复合伤及开放骨折最好在12~24h内完成,其可减少脂肪栓塞等并发症发生。

Since 1992 the treatment of femur intertrochanteric fractures by compression plate with mutiple screws had been used in our hospital- Under the test of stress,its effects on internal fixation of intertrochanteric fracture was four times higher than that of Mclanghlin nail. Its clinical applications showed that this way of internal fixation was easy to operate,effective in fractures reduction,and decreases complications, particularly for fragmental and unstable intertrochanteric fractures.

Theconventional plate group with high stiffness producedthe greater prestress and stress shielding value andslight circulation disorder in bone tissue in early stage,but the osteoporosity of tibias bone ocurred early(lmonth );conventional plate with low stiffness producedthe less prestress and stress shielding value and severecirculation disorder in bone in early stage, but the os-teoporosity ocurred late( 3 month ).We consider thecause of local osteoporosity of tibias bone after plate fixed were mainly prestress,and stress shielding is secondary in early stage(3 month). The local circula-tion is disorder is not important factor.

实验采用316L医用不锈钢制成等长等宽但底面和中央连接部位不同的普通型、凹面型和滑动型三种4孔钢板,并用以固定犬完整中段腔骨。以单纯普通皮质骨。螺丝钉固定作对照组。用静态应变仪测量不同条件下的胫骨应变值,观察不同时期活体骨组织内双硫蓝灌注后血供情况、骨结构改变及CT片图像分析观察胫骨灰度值的改变。结果发现不同固定方法使骨组织产生的顶应力不同。刚度较大的普通型钢板的预应力及应力遮挡作用最大,其早期皮质骨内缺血不明显,但其骨质疏松却出现较早(1个月);刚度较小的滑动型钢板的预应力及应力遮挡作用最小,其早期皮质骨内缺血明显,其骨质疏松却出现较晚(3个月)。作者认为在本实验条件下,导致骨结构改变的主要原因是骨内预应力,而应力遮挡作用的影响相对较小,皮质骨内血运障碍并非骨质疏松改变的主要因素。
