In order to explore macrophage function in acute hepatitis A and find out factors that have influence upon the macrophage funetion, macrophage function and serum endotoxin were assayed in 43 patients with acute hepatitis A by using of fluorescent-labelled staphylococi phagocytosis and quantitative limulus test established in our laboratory. In order to explore macrophage function in acute hepatitis A and find out factors that have influence upon the macrophage funetion, macrophage function and serum endotoxin were assayed in 43 patients with acute hepatitis A by using of fluorescent-labelled staphylococi phagocytosis and quantitative limulus test established in our laboratory. The results showed that phagoactivity of maerophage (PAM)was 82.58±8 . 79% , which was much higher than that in controls (n= 25,71. 08±3.10% , P<0. 01) but there was no significant difference in the phago-index of macro-(PIM) between patients with hepatitis (56.48±3.27%) and controls (56 .01%4.01) . No correlation was found between PAM, PIM and ALT, PA and SB. Positive correlation was found between PAM and endotoxin concentration, but there was no correlation between PIM and endotoxin. We also found that PAM was significantly decreased with recovery of hepatitis and elimination of endotoxin from serum.Onr data suggest that the increase of endoioxin conoentration, as a result of impaired liver function, as well as the release of inflammatory mediators (especially the clone stimulating factor) by the Knpffer cells under the effect of endotoxin might directly or indirectly stimulate the bone marrow to release more macrophages to compensate for the affected Kupffer cells and to elimiaste the circulating endotoxin. It is helpful to the recovery of hepatitis.

采用荧光标记的葡萄球菌法对43例甲型肝炎(甲肝)进行了外周血的巨噬细胞功能测定。同时用鲎试剂基质偶氮显色法检测血清中的内毒素。结果发现:43例甲肝的巨噬细;胞吞噬率(PAM)为82.58±8.79%,与正常人(n=25)的PAM(71.08±3.10%)相比有非常显著的差异(P<0.01),而巨噬细胞吞噬指数(PIM)两组比较无统计学差异(55.48±3.27Vs56.01±4.01,P>0.05)。PAM和PIM与ALT、前白蛋白及总胆红素之间无相关。但甲肝的内毒素浓度(62.66±2.05μg/L)与PAM有明显相关(r=0.7804P<0.01),与PIM则无关(r=0.146,P>0.05)。我们也观察了甲肝巨噬细胞功能的动态变化,发现随着甲肝的恢复,血清内毒素浓度随之降之正常,PAM也趋于正常,而PIM则无明显变化。

It has been known that enterogenic endotoxemia is an important pathogenetic factor in multiple organ failure.However, pathogenetic mechanism of enterogenic endotoxemia is yet unclear.In this work, experiments were performed on intact animal or isolated rat ilea-vascular perfusion apparatus and membrane vesicles produced from the extraction of brush border of rat intestinal mucosa.It is found that circulating endotoxin prompted intestinal endotoxin translocation in a positive feedback manner. Various regulatory peptides modulated the endotoxin translocation:endothelin stimulated and atrial natriuretic peptide inhibited the endotoxin translocation, and the pathway of enterogenic endotoxin translocation is mainly not transcellular, probably paracellular.It is also found that taurine significantly inhibited intestinal endotoxin translocation and protected the animals from endotoxemic injury.

肠源性内毒素血症是多器官功能衰竭的重要发病因素之一,但肠源性内毒素血症的发病机理迄今尚未阐明。本工作在离体动物,离体大鼠回肠血管灌流标本和分离的小肠粘膜刷状缘微囊制备上,观察到循环血中内毒素正反馈地促进肠道内毒素移位;多种调节肽参与内毒素移位的调控,内皮素促进而心钠素抑制其移位过程;肠道内毒素的移位主要不是通过跨细胞途径,而可能是通过旁细胞途径。本工作还发现牛磺酸具有抑制肠道内毒素移位、改善内毒素血症动物预后的作用,提示牛磺酸可能具有防治肠源性内毒素血症的临床应用前景。

The endotoxin release in the circulation may be involved in the continuous formation of neopterin during the late postburn stage.In addition,the presence of a constant high neopterin level is associated with a critical event in the development of burn sepsis.

为探讨血清新喋呤与大面积烧伤后内毒素血症及脓毒症的关系,对35例烧伤面积大于30%(30%~98%)患者的血清新喋呤、血浆内毒素的变化进行了动态观察。结果表明,烧伤后第3天大多数患者新喋呤升高(P<0.05),但与烧伤面积无显著相关(P>0.05)。严重烧伤第2周以后内毒素血症患者新喋呤水平显著高于无内毒素血症患者(P<0.05~0.01)。同时,伤后第14,21天内毒素血症患者的循环内毒素与新喋呤呈显著正相关(第14天:r=0.368,P<0.05;第21天:r=0.439,P<0.01)。在整个监测期间,脓毒症患者(15例)血清新喋呤水平持续升高,伤后第14,28天与非脓毒症患者相比差异有显著或非常显著意义(P<0.05~0.01)。提示烧伤可引起血清新喋呤升高,但与烧伤面积无关。烧伤后内毒素血症对新喋呤的不断产生可能有显著影响。血清新喋呤的持续升高与严重烧伤后脓毒症的发生与发展密切相关。
