Abstract Measurement of regional cerebral blood flow (rCBF) was performed in 40 patientS with hypertensive intracerebral haemorrhage involving the internal capsule and hasal ganglia and in 40 normal subjects. The CBF of stroke hemisphere and non-stroke hemisphere in patients was reduced more obviously than hemispheric mean cerebral blood flow in normal subjects of the same age, particufarly in stroke hemisphere. The rCBF in cerebral haemorrhage correlated with the clinical scverity of illness, localization of haematoma and bleeding volume. The rCBF in cerebral haemorrhage was the least in severe cerebral haemorrhage of the thalamus and large haematoma. Cerebral haemorrhage in the internal capsule and hasal ganglia could result in rCBF reduction of cerebellar and brain stem on the contralateral side of the haematoma.

Objective: To oberserve the evaluation of motor function with corticomyographic motor evoked potentials (MEPs) by transcranial magnetic stimulation. Methods: The clinical value of magnetic stimulation was investigated in 43 cases, including 12 normal cases and 31 cases with motor disturbance. The MEPs were recorded from the thenar and the extensor carpi radialis muscles with a pair of surface electrodes in response to transcranial magnetic stimulation. Recordings were performed when the subjects were at rest, and also during weak voluntary contraction. Results: The MEPs recorded from the contralateral side to the stimulation revealed an almost constant onset latency. However, the amplitude of the MEPs differed among individual cases of normal motor function, but was almost the same on both sides in the same case. Twenty cases of unilateral motor disturbance were classified according to their MMT gradings, and MEP recorded from the paretic side were compared with those from the intact side. A decreased amplitude of the MEPs recorded from the paretic side was always found. However, a prolonged latency of the MEP was not always seen. Changes in the MEP amplitude thus appeared to represent a good indication of motor function. Conclusion: None of the 43 cases receiving magnetic stimulation complained of side effects, and the magnetic transcranial stimulation method was thus concluded to be useful and applicable for clinical practice.

目的:观察磁刺激运动诱发肌电位对运动机能的评价。方法:用磁刺激装置对正常人12例,运动障碍患者31例进行了经颅脑刺激,记录运动诱发肌电位。结果:受检测的43例,无一例引起头痛和感觉异常,也无癫痫及意识障碍等副作用。正常人中,诱发肌电位的潜伏期相对恒定,振幅在个体间虽存有差异,但同一例左右侧几乎相同。对20例单侧肢体功能障碍的肌力按体征分级,比较患侧和健侧的诱发肌电位,发现患侧振幅较健侧明显减低。对肌力0~2级的病例,不能诱发出肌电位。结论:磁刺激运动诱发肌电位,在临床上可在数量上正确评价肢体的运动机能,并且经颅磁刺激法是安全的。

The object of this experimental study was to investigate the influence of low energy He Ne laser on the motor nerve cells of the spinal cord. The experimental study included as follws: (1) Four rabbits were used in this experiment. The L 5~6 spinal cord segment was irradiated by He Ne laser percutaneously, the nerve velocity of the comon peroneal nerve was measured in order to determine the function of the spinal motor nerve cells when the peripheral nerve was intact. (2) The common peroneal nerve was transected on one side wothout repair, two weeks after laser irradiation, the grey mater of the spinal cord of L 5~6 segment was procured for electronic microscopic examination. (3) The common peroneal nerve on the contralateral side was transected and followed by end to end anastomosis, and laser irradiation was done on the same spinal cord segment. Two weeks after irradiation, the nerve velocity of the common peroneal nerve and the toe expanding test were investigated. The results were: (1) the He Ne laser can influence the spinal motor nerve cells function as expressed by latent rate when the peripherial nerve is intact. i.e. the nerve velocity is slower than mormal, and the amplitude is markedly decreared. (2) the change of the microstructure of the spinal motor nerve cells is comparatively slight in the 10 and 15 minutes groups. (3) the recovery of the nerve velocity and the toe expansion are more earlier in the 15 min. group. In short, the low energg He Ne laser can influence the function of the spinal motor nerve cells.

旨在探索低能量氦-氖激光对脊髓运动神经细胞的影响。用4只家兔经皮肤照射L5,6脊髓节段后,测腓总神经潜速率,了解周围神经完整时脊髓运动神经细胞的功能;切断家兔一侧腓总神经而不修复,照射L5,6脊髓节段,2周后取L5,6脊髓段灰质作电镜检查;切断家兔一侧腓总神经,对端吻合后,照射L5,6脊髓节段,2周后观察腓总神经潜速率及展趾反射。结果表明:周围神经完整时,激光照射L5,6脊髓节段后,可引起腓总神经潜速率变化,波幅有明显降低。切断腓总神经侧的运动神经细胞超微结构以每日照射10及15min者的变化较轻微。腓总神经完整侧也有一定变化。吻合腓总神经侧的展趾反射,以每日照射15min者恢复较快。认为,低能量氦-氖激光照射L5,6脊髓节段后,可引起脊髓运动神经细胞的功能变化,促进神经修复后肢体功能的恢复。
