By case-control study, the risk factors and CT results were assessed in 32 aged patient diag- nosed by CT as having silent cerebral infarction,and a control group of 32 aged patients with symptomatic cerebral in- farction. There was no statistical significant difference (p>0.05) in risk factors between the SCI group and the control group,there existed marked difference (P< 0. 0l ) in the size of focal infarctions,with 96.08% focal infarction smller than 15 mm in the SCI group and 76. 60% large than 15 mm in the control group. It is suggested that the lacuna cerebral infarction might be one of the major causes of silent cerebral infarction in aged patients.

Objective To study the diagnostic value of the magnetic stimulation motor evoked potentials (MEP) and somatosensory evoked potentials (SEP) in neurologic function after spinal cord injuries (SCI), especially the motor conductive function. Methods Thirty-two cases of SCI patients received MEP examination elicited with a Mag-2 magnetic stimulator. MEP were recorded bilaterally at abductor pollicis brevis(APB) and anterior tibialis (AT)muscles. F waves and M waves were also recorded to calculated the central motor conductive time (CMCT). SEP examination was performed with a Keypoint electromyograph. The clinical status of the patients was divided by the ASIA 92 principles. Results In SCI group, the latency and amplitude of the APB MEP were (28.88±13.85) ms and (0.32±0.34)mV respectively, the latency and amplitude of the AT muscles MEP were (42.60±16.07)ms and (0.40±0.73)mV respectively. All of them were significantly different from the results of control group (P< 0.05). Among them the complete paralysis cases had no MEP signals, while the MEP in incomplete injured group had decreased amplitude and increased latency. There was significant correlation between the MEP latency and the ASIA motor scores (P< 0.05). The SEP signals of most SCI cases obliterated or delayed,which had no correlation with the results of MEP examination or ASIA motor scores. Conclusion The muscle strength under the injured level is intensively correlated with the MEP abnormal. MEP examination can provide an objective and quantitative standard for judging the motor functional status in the cord, while the SEP examination can not reflect the severity of motor lesion.

Objectives:To investigate the effect of the vascularized tissue transplants on apoptosis after spinal cord injury (SCI).Methods:The adult experimental rats received lumbar spinal cord contusion, and were divided into three groups:The SCI group(group A),contusion with pedicle paraspinal muscle transferred group(group B), contusion with the pedicle omentum transferred group(group C). The apoptosis of spinal slice from the injured spinal cord were examined by the methods of the terminal deoxynucleotidal transferase mediated DUTP biotin nick end labelling (TUNEL) reaction and the expression of Bcl 2 was measured immunohistochemically in 1,3,7,14 and 28 days post operation respectively.Results:The rate of apoptosis sequence and the strong expression sequence of Bcl 2 protein expression were found in all three groups. The former was A>B>C ( P <0.005), and the later was C>B>A ( P <0.05). These sequences were parallel by a significant improvement in neurological function recovery. Conclusions:It indicates that the pedicle omentum transplants can inhibit the apoptosis and improve rats hind limb function after SCI.

目的 :探讨带蒂的大网膜和椎旁肌移植覆盖脊髓对脊髓损伤后细胞凋亡的影响。方法 :应用改良的 Allen's法致伤脊髓 ,将动物分为单纯脊髓损伤组 (A组 )、损伤 +椎旁肌组 (B组 )、损伤+大网膜组 (C组 ) .移植后 1、3、7、14、2 8天对脊髓损伤区进行细胞凋亡的检测 (TU NEL)以及 Bcl- 2蛋白表达的测定 (免疫组化法 )。结果 :A、B、C组中均发现凋亡细胞及 BCL- 2蛋白阳性表达 ,图象分析发现 ,细胞凋亡率为 A>B>C,Bcl- 2蛋白阳性表达顺序为 C>B>A。结论 :带血运的组织移植能抑制脊髓损伤后的细胞凋亡
