AIM To clarify the mechanism of BMP inducing osteogenesis. METHODS An experimental model of 12 mm bone defect in the left ulna of the rabbit was adopted, and these rabbits were divided into experimental group, controlgroup and blank group at random. BMP 10 mg/TGF β 1 10 μg/carrier, BMP 10 mg/carrier and carrier alone was implanted into the bone defect of each group respectively. Roentgenography, histological and electronic microscope examination was used to compare the osteogenesis effects of each group. RESULTS The experimental group had a better osteogenesis effect than control group, either in callus formation or bone union genesis appearing; osteoblast and chondrocyte appeared earlier in experimental group than in control group and nonunion taken was placed in the blank group. CONCLUSION TGF β 1 has the effect of promoting inducing osteogenesis of BMP. Through the effect of promoting differentiation and proliferation TGF β 1 leads mesenchymal cells, chondrocyte and osteoblast to differentition and proliferation; osteoblast produce type Ⅰ collagen; calcification of bone matrix is enhanced by TGF β 1.

目的 我们通过观察转化生长因子 β1 (transform inggrowth factorβ1 ,TGF- β1 )和骨形态发生蛋白 (bone morpho-genetic protein,BMP)共同修复骨缺损 ,达到进一步阐明BMP诱导成骨机制的目的 .方法 实验采用日本大耳白兔左尺骨中段 12 mm骨缺损实验模型 ,分为实验组、对照组和空白组 ,缺损内分别植入 BMP/ TGF- β1 /载体、BMP/载体、载体 .通过 X线、组织学及电镜对比观察三组骨缺损修复情况 .结果 实验组骨缺损无论是骨痂出现还是骨连接发生时间均较对照组明显提前 ,空白组骨缺损形成骨不连 .实验组成骨细胞和软骨细胞无论出现的时间、数量和功能活跃程度均优于对照组 .电镜观察见成骨细胞、软骨细胞内线粒体丰富、内质网增多 ,而对照组则相对较少 .结论  TGF- β1 具有促进BMP诱导成骨的作用 .TGF-β1 通过促进间充质细胞分裂增殖为 BMP提供靶细胞、促进成骨细胞及软骨细胞的分裂增殖、促进骨细胞合成产生 型胶原、促进基质钙化、减少骨吸收等作用来促进 BMP的诱导成骨

Objective To evaluate the value of calcium phosphate cement (CPC) artificial bone graft in the repair of segmental radius defects in rabbits. Methods CPC was implanted into the segmental radius defects of rabbits, and the implanted materials were taken out at 4, 8, 12, and 16 weeks after implantation, HE staining and Masson trichrome staining were taken and the effect of bone defect repair was evaluated. Results New cartilages and new immature bone tissues were found in the bone substitute in 4 weeks after operation. So were mass cartilage cells, osteoblasts, collagen tissues and more small vessels. In weeks 8~16 after operation, the new bones was formed continuously, the artificial bone was gradually rebuilt to mature lamellar bone, bone trabecula, and medullary cavity. Conclusion With good biocompatibility and osteogenesis effect CPC can be an ideal bone substitute, and its clinical use in future seems promising.

目的 观察自制水泥型羟基磷灰石 (CPC)人工骨修复兔桡骨大段骨缺损的成骨效果。方法 将自制的CPC人工骨植入新西兰兔桡骨大段骨缺损模型中 ,术后 4、8、12、16周取材 ,采用HE染色和Masson三色染色分析评价骨缺损修复效果。结果 CPC植入后 4周有新生软骨形成及未成熟的骨组织 ,可见大量的软骨细胞、成骨细胞、胶原组织及较多的小血管。 8~ 16周新骨继续生成 ,人工骨逐渐改建为成熟的板层骨、骨小梁和髓腔结构。结论 自制CPC人工骨有良好的生物相容性和成骨效果 ,可能是一种较理想的骨移植替代材料

Objective:To investigate the osteogenesis effects and biocompatibility of the graft of bone matrix gelatin(BMG).Methods:BMG was made of tubal cortical bones of healthy bovine or swine according to the method of Urist and was standby after sterilization of ethylene oxide or Co 60 .Seventy-two cases of nonunion of long bone fractures were treated by internal fixation and implantation of BMG.Postoperative observations of incisions and X-ray evaluations of the circumstance of fracture union were carried out.Results:Of the 72 cases,incision union of the first rate happened in 69 cases and the second rate took place in 3 cases.The latter we could found exudations of clear liquids in 2 to 3 weeks.But no bacteria could be found after cultivations.Sixty-six cases were followed up for 1~3 years and all fractures were found union,of which postoperative union in 2 months in 38 cases,3 months in 23 cases and 4 months in 5 cases.The implanted BMG was observed totally absorbed and replaced by normal bone tissues in 51 cases when the internal fixations were taken out and none of zoonosis was found in the follow-up patients.Conclusions:Heterogeneous BMG still has function of osteoinduction after spanning different genus and species and no severe immuno-rejection was found.It can replace autograft in the treatment of bone non-union.

目的 :临床观察骨基质明胶植入的成骨效果及生物相溶性。方法 :取健康的猪或小牛的管状皮质骨 ,根据Urist法制备成骨基质明胶 ,包装后环氧乙烷或钴60 消毒备用。 72例四肢长骨折不愈合患者 ,手术内固定治疗 ,并在骨折端植入骨基质明胶。术后观察切口愈合情况及定期拍X线片评价骨折愈合情况。结果 :72例植入骨基质明胶的患者 ,3例出现切口局部轻度红肿、清亮的液体渗出 ,培养无致病菌生长。 66例患者获随诊 1~ 3年 ,全部骨折愈合。其中 3 8例术后 2个月骨愈合 ,2 3例术后 3个月骨愈合 ,5例术后 4个月骨愈合。 5 1例内固定钢板取出时观察植入的骨基质明胶已完全吸收 ,被骨组织取代 ,随诊的患者未发现传播人畜共患疾病。结论 :异种骨基质明胶跨种属应用仍具有诱导成骨作用 ,未见免疫排斥反应 ,可替代自体骨移植治疗骨不愈合。
