Objective To clarify the actions of tranilast,an anti allergic drug,on chronic constriction mononeuropathy through an experiment using an animal model according to the method of Bennett et al. Methods 36 rats were divided into 2 groups: one was treated with tranilast(200mg/kg·d-1,p.o every experimental day),the other was controlled with solvent only. Chronological changes of heat evoked withdrawal latencies on a hot plate,nerve coducting velocitics, and histopathological changes were compared between the two groups from one to four weeks after loose ligation in the sciatic nerve. Results The changes of the withdrawal latency and nerve conducting velocities in the tranilast treatment group were significantly longer than those in the control. The rats without tranilast treatment showed inflammation in and around the constricted nerve bundles,axonal degeneration,phagocytes invasion and interstitial edematous changes at 7 days,numerous axonal sprouts and remyelination at 14 days,and regeneration in the nerve bundles at 28 days. In contrast,the rats treated with tranilast showed less inflammation or nerve fiber degeneration,and better regeneration than those of the control. Conclusion The actions of tranilast appear to have beneficial effects on prophylaxis of a chronic constriction mononeuropathy and on preservation of the nerve functions in rats.

目的研究抗瘢痕增生药物曲尼司特在单纯慢性卡压性周围神经病变中的抗瘢痕形成作用。方法按照Bennett等报导的方法,将36只大鼠分为两组:实验组在整个实验过程中,每日经口给予曲尼司特200mg/kg,对照组每日给予药物溶剂10ml/kg。比较两组动物从7d到28d患肢热板试验、神经传导速度、组织病理学和电镜微细结构的改变。结果右下肢热逃避反射潜伏期和神经传导速度的改变,实验组较对照组明显变短,对照组坐骨神经在实验7d后表现为神经周围或神经束间炎性反应和轴索变性,14d大量的轴索发芽和再生的有髓纤维,28d神经束再生。实验组有效地预防了早期神经的炎性反应和轴索的变性。结论曲尼司特在大鼠单纯慢性卡压性神经病变模型中有对瘢痕形成抑制和神经功能的保护作用。

The present study was to investigate the changes of TTX R sodium currents and NaN/SNS2 sodium channel transcripts in DRG neurons in chronic constriction injury (CCI) of sciatic nerve 14 days after surgery. The transcript of NaN, a sensory neuron specific TTX R sodium channel, was significantly down regulated as well as the TTX R sodium current. The TTX R current density was significantly decreased in CCI group. At the same time, the properties of the TTX R currents revealed in the activation curve and steady state inactivation curve were unchanged. These results may provide at least a partial basis for the hyperexcitability of DRG neurons that contributes to hyperalgesia in this model.

目的 :观察电压门控钠通道NaN /SNS2在大鼠坐骨神经慢性压迫性损伤 (chronicconstric tioninjury ,CCI) 14天后的变化 ,探讨慢性神经痛的发生机制。方法 :以逆转录多聚酶链反应(RT PCR)半定量分析CCI后大鼠背根神经节 (Dorsalrootgamglion ,DRG)钠通道NaN/SNS2转录物的变化 ,以及全细胞膜片钳技术记录CCI对急性分离大鼠背根神经节TTX R钠电流的影响。结果 :CCI术后 14天 ,感觉神经元特异性的TTX R钠通道转录物NaN/SNS2下调 ,与对照组相比 ,下降了大约 30 % ,TTX R钠电流密度明显减弱 ,但不影响其激活与稳态失活。其激活曲线的V1/2 分别为 - 2 6 .6 378mV、- 2 6 .6 30 6mV ,k为 6 .786 4mV、6 .732 3mV ;失活曲线的V1/2 分别为 -35 .5 84 4mV、- 37.2 188mV ,k为 8.2 792mV、8.4 6 47mV。结论 :钠通道NaN/SNS2与慢性神经痛后初级感觉神经元过度兴奋有关。

Objective: To investigate the central mechanism of trigeminal neuralgia following chronic constriction injury (CCI). Methods: Rats were randomly divided into two groups: the operative group and the sham-operative group. In operative group, an unilateral CCI was produced by placing loose chromic gut ligatures around the infraorbital nerve (ION). In the sham-operative group, the ION was only exposed using the same procedure, but not ligated. Following the appearance of allodynia, rats were killed and fos-like immunoreactivity were quantified in the dorsal horn of caudal medulla and upper cervical spinal cord(C 1~2). Results: In CCI-ION group, an allodynia in response to mechanical stimulation of the territory of the ligated nerve was found on post-operative day 15. In both CCI-ION and sham-operative groups, the labeled c-Fos neurons were concentrated in laminae I~II. The total number of c-Fos neurons in the ipsilateral medullary dorsal horn of CCI-ION group was significantly greater than that in the contralateral side and the sham-operative group (P<0.01). Conclusion: CCI-ION can induce significant alterations in the central processing of afferernt signals, which may underlie neuropathic pain.

目的 :探讨慢性压缩性损害引起三叉神经痛的中枢机制。方法 :将 2 0只大鼠随机分为两组 ,手术组 (10只 )进行三叉神经分支 (眶下神经 )缩窄环术 ,即用两根铬线将眶下神经疏松环扎 ,对照组 (10只 )只暴露眶下神经 ,但不结扎。在痛觉超敏期 ,将大鼠处死 ,进行免疫组化 ,观察延髓尾侧段及上颈段 (C1~ 2 )后角c Fos蛋白表达。结果 :术后 15天左右 ,手术组动物出现痛觉超敏。两组动物c Fos蛋白主要分布于延髓尾侧段及上颈段脊髓后角Ⅰ、Ⅱ层内 ,手术同侧c Fos较对侧及对照组明显升高 (P <0 .0 1) ,在统计学上有显著差异。结论 :三叉神经慢性压缩性损害诱导中枢传入信号的改变 ,是构成神经性疼痛的基础。
