Objective:To determine the effects of interleukin 2 (IL2),superoxide dismutase (SOD) and malondialdehyde (MDA) on excretion of metabolic product.Methods:We selected 16 patients with maintenance hemodialysis ,and measured IL2,SOD,MDA two hours before hemodialysis ,one hour and four hours during hemodialysis ,as well as four hours after hemodialysis.Changes in IL2,SOD and MDA in patients with maintenance hemodialysis were compared with those suffering from chronic nephritis without hemodialysis and normal controls.Results:IL2 levels were highest at two hours before hemodialysis (884 15±10 51kU/L),and lowest at four hours after hemodialysis (417 36±7 35kU/L, P <0 01).On the contrary,SOD levels were lowest at two hours before hemodialysis (144 09±9 41U/L),and highest at four hours after hemodialysis (215 30±8 35U/L, P <0 01).MDA controls were highest in patients without hemodialysis (85 99±16 71U/L), and lowest at four hours after hemodialysis (24 19±7 11U/L, P <0 01).On unhemodialysis day,IL2 levels in patients with maintenance hemodialysis (609 41±7 24kU/L) were significantly higher than that of chronic nephritis(201 33±11 51kU/L),and also that of normal controls (125 13±12 46kU/L, P <0 01).Conclusions:Changes in IL2 are correlated with excretion of metabolic product in patients with maintenance hemodialysis.Determination of IL2 level may help in monitoring of adequacy of hemodialysis.

目的:探讨白细胞介素2(IL2)、超氧化物歧化酶(SOD)和丙二醛(MDA)对体内代谢产物排泄的影响及其意义。方法:选16例维持性血液透析患者,于透析前2小时,透析1小时、4小时及透析后4小时测定IL2、SOD、MDA,并于非透析日与非透析慢性肾功能衰竭患者及正常人对照。结果:IL2于透析前2小时最高(884.15±10.51kU/L),透析后4小时最低(417.36±7.35kU/L),P<0.01。SOD于透析前2小时最低(144.09±9.41U/L),透析后4小时最高(215.30±8.35U/L),P<0.01。MDA于非透析日最高(85.99±16.71U/L),透析后4小时最低(24.19±7.11U/L),P<0.01。非透析日IL2在维持性透析者最高(609.41±7.42kU/L),非透析肾功能衰竭次之(201.33±11.51kU/L),健康对照组最低(125.13±12.46kU/L),P<0.01。结论:IL2的变化规律与体内代谢产物清除状况相符,IL2可能作为是否充分透析的指标之一。

Objective:To investigate the changes in nitric oxide (NO) and endothelin(ET) levels during hemodialysis.Methods:Twenty patients with maintenance hemodialysis underwent separately acetate hemodialysis (AHD),bicarbonate hemodialysis (BHD),and acetatefree hemodialysis (AFHD).Blood samples were taken prior to hemodialysis,and 15,30,60,120,240 minutes after hemodialysis,respectively.Results: At 60 minutes postAHD,serum NO level showed a transient increase ( P <0 001),and then it gradually reduced to baseline.No significant changes in serum NO level were found at 120 minutes,while it was markedly reduced at 240 minutes after BHD.In patients treated with AFHD,NO level was decreased to the lowest value at 60 minutes ( P <0 001),and this level was maintained during the observation period.However,ET levels peaked at 240 minutes after AHD ( P <0 001).Similarly,it also increased at 240 minutes after BHD ( P <0 05),but no significant differences in ET levels were observed in patients with AFHD ( P >0 05).Conclusions:Acetate,especially in high concentrations,appears to be able to promote the release of NO and ET in patients undergoing hemodialysis.The effect of AFHD on NO and ET is superior to BHD,and BHD superior to AHD.

目的:了解血液透析(血透)患者血清一氧化氮(NO)及内皮素(ET)在血透过程中的变化。方法:20例维持性血透患者分别进行醋酸盐透析(AHD)、重碳酸盐透析(BHD)和无醋酸盐透析(AFHD),分别于各种方式透析时抽取透析开始时及透析15分钟、30分钟、60分钟、120分钟和240分钟时的管路动脉端血标本,用比色法测定NO水平,用放射免疫法测定ET水平。结果:AHD60分钟时NO呈一过性增高(t=6.26,P<0.001),而后降到透析开始时水平;BHD前120分钟NO水平无明显变化,240分钟时下降(t=2.53,P<0.05);AFHD时NO逐渐下降,至60分钟时达最低水平(t=4.76,P<0.001),而后稳定不变。AHD240分钟时ET达最高值(t=8.46,P<0.001);BHD240分钟时ET水平增高(t=2.70,P<0.05);AFHD时ET水平变化无显著性差异(F=1.43,P>0.05)。结论:醋酸盐尤其高浓度时能促进血透患者NO及ET的上升;NO能被透析清除;AFHD对NO及ET的影响优于BHD,BHD优于AHD。
