     Methods: Traumatic PVR was induced in pigment rabbits by intravitreal injection of platelet rich plasma. Objective To evalute the value of application of retinotomy in complicated retinal detachments. Methods A consecutive series of 46 eyes (45 cases)undergoing retinotomy between July 1994 and December 1995 in our department was reviewed retrospectively. Among them, there were 35 eyes of rhegmatogenous detachment with proliferative vitreoretinopathy (PVR ), 8 eyes of traumatic PVR and 3 eyes of tractional detachment. Results Total retinal and macular attachment were achieved in 24 of 40 ey6s (60% ) at the first operation. Retinal redetachment was found in 16 eyes in which the same surgery was performed in 12 eyes and amony them subretinal membranes were removed in two eyes simutaniously. removing subretinal memberanes of 4 eyes and relaxing retinotomy of 2 eyes were found to be successful among 8 eyes. The visual acuity was improved in 37 eyes (80% ) during a mean follow - up period of 7 monthes. Conlusion Rational choice of retinotomy for endodrainage of subretinal fluid,removing subretinal memberanes and relaxing irreflexable shortened retina could increase the successful rate of surgery for the complicated retinal detachment.

目的评价视网膜切开在复杂性视网膜脱离手术中的应用价值。方法回顾性总结了1994年7月至1995年12月在我科做视网膜切开手术的连续病例45例46只眼,其中裂孔性视网膜脱离伴增殖性玻璃体视网膜病变(proliferativevitreoretinopathy.PVR)35例35只眼、外伤性PVRS例8只眼、牵引性视网膜脱离2例3只眼。结果平均追踪7个月,内排液组40只眼一次手术成功24只眼占60%,有12只眼再发视网膜脱离做了第二次同样手术,其中同时取视网膜下膜2眼;取视网膜下膜5只眼和视网膜切开减张3只眼中,有6只眼视网膜复位,失败2只眼。视力改善37只眼占80%。结论在复杂性视网膜脱离中,合理选用视网膜切开、眼内排出视网膜下液、取除视网膜下膜和松解缩短、僵硬的视网膜,可提高手术疗效。

Methods Immunohistochemical procedures were applied to identify IgG and complement C 3 deposits in the specimens of epiretinal membranes surgically obtained from patients with PVR. Vitreous aspirates from 33 eyes with idiopathic and traumatic PVR and 8 normal eyes were examined for IgG and complement C 3 by using ELISA; the total vitreous protein (TVP) was determined by Coomassie Brilliant Blue Test; the relative activity of IL 6 in vitreous was tested by means of a proliferative response of IL 6 dependent cells to vitreous samples; and the biological activity of TNF in vitreous was measured with its killing effect on L 929 tumor cells. Results Diffuse deposits of IgG and C 3, TVP in epiretinal membanes of PVR eyes were significantly higher than those in the control group; the activities of IL 6 and TNF (0/8) were not detected in any of the normal control, IL 6 was detected in 13 of 17 vitreous samples and TNF was detected in 3 of 11 vitreous samples from PVR eyes; and vitreous IL 6 activity was found to be positively correlated with the severity of PVR. Conclusion The deposits of IgG and C 3, their elevation in amount and the detection of IL 6 and TNF activity in the vitreous samples from patients with PVR suggest that the cytokines and local immune reaction play a role in the pathogenesis of PVR.

目的检测白细胞介素6(interleukin6,IL6)、肿瘤坏死因子(tumornecrosisfactor,TNF)等炎性细胞因子在增殖性玻璃体视网膜病变组织和玻璃体液中的水平。方法用免疫组织化学方法检测视网膜前膜沉积的IgG和补体C3;用ELISA方法测定33只增殖性玻璃体视网膜病变(proliferativevitreoretinopathy,PVR)眼和8只正常眼玻璃体液中IgG、补体C3的含量;用考马斯亮蓝法测定玻璃体总蛋白;采用待测标本对IL6依赖细胞株增殖反应的影响测定玻璃体液中IL6的相对活性;用对L929肿瘤细胞的杀伤作用检测玻璃体液中TNF的生物活性。结果PVR眼视网膜前膜有大量IgG、补体C3沉积;PVR眼玻璃体液中IgG、补体C3、玻璃体总蛋白含量均明显高于对照组;正常对照眼玻璃体液中未检出IL6和TNF活性,17只PVR眼玻璃体液中有13只检出IL6活性,11只PVR眼玻璃体液中有3只检出TNF活性;玻璃体液中IL6活性增高与PVR病情程度有显著的相关性。结论视网膜前膜有IgG和补体C3的沉积,PVR眼玻璃体液中IgG、补体C3含量增加,IL6及TNF活性的检出?
