This article reported 18 cases with internal torsion gait in walking. The internal torsion gait may compensate the deformity caused by unstable hip and knee joint. Objective:To observe the X ray signs in rabbit lumbar intervertebral disc degeneration induced by instability of the spinal equilibrium system.Methods:Twenty seven New Zealand white rabbits were divided into two groups,fifteen as the operating group,and another twelve as the control group.The operating group was incised through posterior approach along spinous process,then detached below erector spinal muscle,and resected the supraspinal and interspinal ligament and the posterior half of the zygapophysial joint between the third and the sixth lumbar spine.The control group was incised only by the skin.All of the rabbits were bred in standard circumstance,and examined four and eight months later with X ray.Results:In the operating group,incision infection occurred in one rabbit,and two rabbits died two and five months after the operation respectively.By the end of four months,all lumbar spines showed recurvation and the majority discs showed endplate calcification.By the end of eight months,all the cartilage endplates were calcified and the majority discs showed narrowing.Statistic analysis displayed that the radiological abnormality such as wedging,narrowing,endplate calcification,and osteophyte formation from the third to the sixth disc were more common than in the control group with a significant difference.Conclusions:The equilibrium of spine is of importance to its physiological function.Disc degeneration may be induced by spine nonequilibrium.Recurvation deformity and endplate calcification are early and common,X ray signs in disc degeneration caused by spinal instability.

目的 :探讨腰椎失稳对椎间盘的影响及其X线表现。方法 :选用新西兰兔 2 7只 ,随机分为手术组 15只 ,对照组 12只 ,手术组沿L3 ~L6棘突作后正中切口 ,剥离骶棘肌 ,切除棘上、棘间韧带 ,咬除关节突关节外后 1/2 ;对照组作相同皮肤切口即缝合 ,分别于术后 4、8个月对腰椎行X线检查。结果 :除外手术组切口感染 1只 ,术后 2和 5个月各死亡 1只 ,其余兔存活完好至取材。术后 4个月 ,手术组均发生腰椎后凸畸形 ,出现软骨终板钙化的椎间盘超过半数 ,8个月时 ,所有椎间盘软骨终板均钙化 ,多数椎间隙狭窄。对椎间隙楔形样变、椎间隙狭窄、终板钙化和骨赘形成 4种退变征象发生频数的统计分析显示 ,手术组与对照组间的差异具有显著性意义。结论 :脊柱稳定对保持椎间盘生理功能意义重大 ,脊柱失稳可诱发椎间盘退变 ,后凸畸形和软骨终板钙化是失稳诱发的腰椎间盘退变早先发生和常见的X线征象

Objective To explore the possible mechanism of the erector spinal muscles in idiopathic scoliosis by comparing the expression and localization of neuronal nitric oxide synthase (nNOS) and inducible nitric oxide synthase (iNOS) of the thoracic erector spinal muscles on convex side and concave side. Methods The patient group comprised 8 females and 2 males who were scheduled for spinal surgery. The apex of scoliotic curve in these patients arose between T6 and T11. The mean age was 14.3 (range 12- 17) years, and the mean Cobb angle was 57.7° (range 45°-85°). Muscle biopsies were taken bilaterally during surgery from the superficial multifidus muscle at the apex of the curve between the 6th and 11th thoracic vertebral levels. Part of the tissue was fixed in formalin and stained with hematoxylin and eosin; the remaining tissue was snap frozen and processed for immunohistochemistry and Western blot. Immunocytochemistry for nNOS and iNOS were performed using the EnVision?two-step method. Western blot was done with antibodys to nNOS and iNOS. Immunoreactive bands were visualized by enhanced chemiluminescence according to the manufacturer's specifications (Amersham Corp). Results nNOS protein in the erector spinal muscles was localized at the sarcolemma. Western blot demonstrated that nNOS protein expression in the concave side of erector spinal muscles is more than that in the convex side. A significant decrease in nNOS protein and activity was found on the convex side of erector spinal muscles from idiopathic scoliosis patients; There was a little immunoreactivity to iNOS in erector spinal muscles. There was little difference in iNOS protein expression between both sides of the curve. Western blot detected the same results. Conclusion There is a greater expression of nNOS and iNOS on the concave side than on the convex side, suggesting nNOS and iNOS may play a role in the patho-genesis of idiopathic scoliosis.

目的 通过比较脊柱侧凸患者胸弯顶点处凸侧、凹侧竖脊肌的神经元型一氧化氮合酶(nNOS)及诱导型一氧化氮合酶(iNOS)表达,探讨竖脊肌在脊柱侧凸中的可能的机制。方法 选取2001年8~12月间我院收治的胸椎侧凸畸形患者10例,于术中取顶椎凸侧、凹侧竖脊肌组织各一块,另有2例胸腰椎爆裂骨折行后路固定融合的患者作为正常对照,于T10处取正常竖脊肌组织,采用免疫组织化学和Western印迹检测竖脊肌组织中nNOS、iNOS的表达和定位。结果 脊柱侧凸患者脊柱凸侧竖脊肌组织与凹侧竖脊肌组织和正常对照相比,10例患者中9例nNOS的表达明显下调,8例iNOS的表达也有所下调,但。iNOS表达总量较低。结论 脊柱侧凸患者双侧竖脊肌nNOS、iNOS蛋白表达不均衡,可能对特发性脊柱侧凸发生起促进作用。
