Objective To study the changes of plasma stress hormones of the hypothalamus-pituitary-adrenal (HPA) axis and pancreatic hormones in acute cerebral infarction (ACI) and its mechanism. Methods Plasma CRH (corticotropin-releasing hormore 40 cases only)、 ACTH (adrenocorticotropic hormone)、 CS (croticosteroid), INS (insulin)、 GLC (glucogan)levels were examined by RIA in 151 patients with ACI during acute stage and compared with those in 60 healthy controls. Results The plasma CRH (14.72±7.08ng/L), ACTH(24.01±8.17ng/L), CS(359.11±126.15ug/l), INS(16.34±11.13mIU/L) and GLC(210.81±102.57ng/l) levels of these patients were very significantly higher than those of the controls (12.45±4.26ng/L, 15.82±3.97ng/L, 140.0±51.74ug/L, 12.92±3.62mIU/L, 139.13±41.67ng/L respectively, P<0.01~0.001). Levels of pancreatic bormones also changed significanthy (P<0.05~0.001). Conclusion These results suggest that there above-mentioned hormones seem to take part in the development and progressing process of this desease, Measurement of their contents may be of help to judge the severity of illness, predict the patient's prognosis and evaluate the effectiveness of treatment.

目的 :研究急性脑梗死(ACI)下丘脑 -垂体 -肾上腺(HPA)轴激素与胰腺内分泌的变化 ,以探讨其发病机理。方法 :采用放射免疫分析法(RIA)分别对151例ACI患者进行CRH(促肾上腺皮质激素释放激素)、ACTH(促肾上腺皮质激素)、CS(皮质醇)、INS(胰岛素)及GLC(胰高血糖素)入院时始发状态血液检测 ,并与60例年龄相仿健康人组作对照。结果 :ACI者HPA轴激素显著高于健康正常人组(P<0.01~0.001) ,胰腺内分泌也发生了有重要统计学意义的变化(P<0.05~0.001)。结论 :CRH、ACTH、CS、INS与GLC参与了ACI的病理发生发展过程 ,测定其水平可作为判断病情程度、愈后转归及疗效评定的客观指标。

目的 :探讨急性脑梗死 (ACI)下丘脑 垂体 肾上腺 (HPA)轴激素及免疫细胞因子水平与中风病类之间的变化关系 ,以阐明其发病机理。方法 :采用放射免疫分析法 (RIA)和免疫放射分析法 (IMRA)分别对 2 6 6例ACI患者进行促肾上腺皮质激素释放激素 (CRH)、促肾上腺皮质激素 (ACTH)、皮质醇 (CS)、肿瘤坏死因子 (TNFα)、IL 6白细胞介素 6 (IL 6 )入院时血液检测 ,分析其在中风病类中脏腑、中经络者中的变化特点 ,并与 6 0位年龄相仿健康人作对照。结果 :ACI者HPA轴激素和免疫细胞因子含量中脏腑者显著高于中经络者 (P <0 0 1) ,中经络者又显著高于健康人对照组 (P <0 0 1)。 结论 :CRH、ACTH、CS、TNFα与IL 6参与了ACI病理发生发展过程 ,测定其动态变化可作为判断中医中脏腑、中经络患者病情程度、预后、转归及疗效评定的客观指标。
