Experiments were done in twenty-seven anesthetized rabbits which haveundergone vagotomy,neuro-muscular blockade and thoracotomy.The animals wereartificially ventilated by a positive-pressure ventilator,the rate and volume of which can be adjusted to maintain the end tital CO_2 constant.Phrenic nerve was exposed in the neck by a ventral approach and thenmounted on a bipolar electrodes immersing in a pool of mineral oil.The signalsof phrenic potentials as well as its integrated waves were recorded.Electric stimuli(rectangular pulses of 0.5 msec.duration at 90/sec.frequency)were deliverodto the philtrum area by bipolar electrodes.The main results are as follows:1.Specific changes in phrenic discharge patterns are observed during electricalstimulation of philtrum area in isolated rabbit respiratory center preparation2.Various results were obtained with different parameters of repeatedstimulation.Facilitatory effects were obtained at low voltage,whereas inhibitoryeffects were obtained at high voltage of repeated stimulation.In both cases thenormal rhyhmic phrenic discharges are marked.3.A single stimulus delivered during the inspiratory phase produces a cessationof phrenic discharge with a short latency of 5-10 msec.The duration of thiscessation persists for 20-40 msec.Whereas the same sort of stimulus is givenduring the expiratory phases,various patterns of(sometimes unrecognizable)phrenic bursts are obtained.The latency of these responses average 25 msec.4.Short train of stimuli(10-20 stimuli)applied to the inspiratory phasecauses premature termination of this phase,and initiate an expiratory phase.The threshold for producing this phenomenon is higher when the train is appliedearlier in the inspiratory phase than when it does later in the same phase.

Objective To observe the clinic effect of bilevel positive pressure ventilator on the chronic pulmonary heart disease.Methods One hundred and thirty-four patients associated with chronic pulmonary heart disease in acute exacerbation period were randomly divided into two groups.On the basis of conventional therapy,64 cases in treatment group adopted bilevel positive-pressure ventilator.Clinic curative effect,oxygen saturation(SaO 2),first second forced expiratory volume(FEV 1) were observed.Results Both groups improved the clinic curative effeiciency,SaO 2 and FEV 1(P< 0.05),but the above-mentioned indexes of the treatment group improved more distinctly.During the conventional therapy period,the indexes before the treatment showed no significant meaning between the two groups(P> 0.05).But after the treatment,most indexes showed significance through the statistic disposal(P< 0.05).Conclusion Bilevel positive-pressure ventilator on the treatment of chronic pulmonary heart disease proves clinic value.

目的 观察双水平正压呼吸机治疗慢性肺源性心脏病的临床疗效。方法 将 134例慢性肺源性心脏病急性加重期的住院患者随机分为两组 ,在原有常规综合治疗的基础上 ,治疗组 6 4例采用无创面罩双水平正压呼吸机通气治疗 ;观察双水平正压呼吸机对慢性肺源性心脏病患者临床症状、无创氧饱和度 (SaO2 )、第 1秒钟用力呼气容积(FEV1)的影响。结果 治疗组、对照组治疗后临床有效率、SaO2 、FEV1均有提高及改善 (P <0 .0 5 ) ,但治疗组治疗后上述指标提高更显著 ,两组治疗前上述参数差异无统计学意义 (P >0 .0 5 ) ,而治疗后大多数参数差异有统计学意义 (P <0 .0 5 )。结论 双水平正压呼吸机通气治疗慢性肺源性心脏病具有临床价值
