Objective The purpose of the study was to evaluate the long term effect and its influence on facial growth of suture distraction osteogenesis for closure of cleft palate. Method 14 Beagle dogs were used in the study. Six of them were in the experimental group, eight served as the control. Direct measurements were taken on dry skulls of the dogs and the data were analysed statistically. Results The results showed that bony healing at the midline between the two horizontal portions of the palatine bones appeared in all the experimental dogs. There were no significant differences in the variables of facial length and height between the two groups. But the facial width and the distance between the two vertical portions of the palatine bones were significantly less in the experimental dogs than the control. Conclusions It is suggested that bony repair of the cleft could be accomplished permanently with the technique of sutural distraction. this technique had no influence on facial length and height growth, but could led to underdevelopment of the width of the face and the respiratory tract of the palatal portion.

Objective To explore a new technique for repair of alveolar cleft by sutural distraction osteogenesis. Methods Nine 8-weeks mongrel dogs were used in this study,three being in the control group,six in the experimental group.Alveolar cleft model was created surgically in all animals. Two weeks later,a U-shaped distractor made of Ni-Ti memory alloy wire was insterted into the premaxilla to distract the mid-premaxillary suture.When the premaxilla of the cleft side approached the ipsilateral maxilla,periosteoplasty of the alveolar cleft was performed.The distractor was removed at two weeks after periosteoplasty.The results were evaluated clinically,radiographically,morphologically and histologically. Results The cleft model in dogs was stable and similar to the human alveolar cleft. In experimental dogs,the premaxilla was moved gradually toward the maxilla so that the cleft was closed. The distracted mid-premaxillary suture showed a gradually widened traingle,with its tip being posterior.The density of the distracted traingle suture was increased gradually.Bony repair was achieved completely at the cleft three months post-periosteoplasy.The morphology of the mid-premaxillary suture was restored. Conclusion The alveolar cleft could be repaired by the technique of mid-premaxilla suture distraction.

目的 探索用缝牵张成骨技术以组织再生的原理修复牙槽裂。方法 采用犬牙槽裂模型 ,牙槽裂形成术后 2周 ,于前颌中缝处安置缝牵张器 ,外张力 2 0 0g。牵张 2~ 3周 ,裂隙侧的前颌骨向侧方移动贴紧远中裂缘 ,用局部黏骨膜瓣修复牙槽裂隙。保持 2周后去除缝牵张器。临床观察、连续X线、头颅干骨和组织学检查评价治疗结果。结果 所有对照动物形成规则的牙槽裂 ,无自发骨性愈合。前颌中缝牵张后 ,裂隙侧前颌缓慢向裂侧上颌靠拢 ,2~ 3周与远中裂缘密切贴合。X线片示前颌中缝呈现由窄渐宽的三角形 ,三角形的尖端指向后方 ,三角区域的骨密度逐渐增加。骨膜成形术后 3个月 ,原牙槽嵴缺损处骨质连续性完全恢复 ,且牙槽骨段在三维方向上均与对侧相同。前颌中缝恢复正常直线形态。结论 该牙槽裂模型稳定性和重复性好 ;前颌中缝牵张诱导缝区新骨形成 ,一侧前颌向裂隙移位和局部骨膜成形可达到完善的牙槽裂修复。

Objective:To observe the response and changes of cranial suture to mechanical strain in growing goats. Methods: 12 growing goats were used in this study. A custom-made distractor was used for distraction of the coronal suture at a rate of 0.4 mm/day for 8 days. 3 animals were killed respectively at 0,2,4 and 8 weeks after completion of suture distraction. X- Ray examination was taken and the distracted suture samples were processed for histological analysis. Results: The coronal sutures of all goats were separated successfully. Signs of intramembranous bone formation and remodeling were found in the distracted suture,and the sutural structure almost returned to its normal state at 8 weeks after the end of distraction. Conclusions: Mechanical strain resulted from distractor can induce the adaptive remodeling in the cranial suture of growing goats.

目的 :研究处于生长发育期山羊颅骨冠状缝受张应力作用后的变化规律 ,探讨牙面矫形治疗的生物学原理。方法 :选用处于生长期山羊 12只 ,在颅骨冠状缝两侧安放自行研制的牵张器以每天 0 .4mm的速率进行骨缝牵张 ,连续加力 8d后停止。在牵张结束后第 0 ,2 ,4和 8周分别处死 3只动物 ,取骨缝标本进行X线检查和组织学观察。另选 2只同龄山羊作为正常对照。结果 :所有实验动物的冠状缝均被成功分离。在实验早期 (0 ,2周 ) ,扩大骨缝中可见大量胶原纤维沿牵张力方向有序排列 ,骨缝边缘有大量的成骨细胞并出现幼稚骨小梁。在实验后期(4 ,8周 ) ,牵张骨缝新生骨组织改建活跃 ,骨缝组织结构最终基本恢复到正常状态。结论 :骨缝组织受牵张力作用后将发生活跃的膜内成骨反应 ,主要集中于骨缝边缘 ,扩大的骨缝可以通过适应性改建恢复其正常结构
