The effects of thyrotropin-releasing hormone(TRH) on traumatic-hemorrhagic shock at a simulated altitude of 4 000 m were observed in rats. The rat model of traumatic-hemorrhagic shock was established by fracturing its right femur and bleeding until the blood pressure was 5. 86 kPa in an hour. The effects of thyrotropin-releasing hormone(TRH) on traumatic-hemorrhagic shock at a simulated altitude of 4 000 m were observed in rats. The rat model of traumatic-hemorrhagic shock was established by fracturing its right femur and bleeding until the blood pressure was 5. 86 kPa in an hour. Under general anesthesia, the right femoral artery and vein of the rat were catheterized for bleeding and blood pressure monitoring. The left ventricle of the rat was also catheterized for the determination of left ventricular pressure (LVP), and the maximal range of the changes of left ventricular pressure( ±dp/dtmax ).The changes of mean arterial pressure(MAP). LVP, ±dp/dtmax and blood gases were observed and the water content of the lungs and the brain determined before and after the administration of 5mg/kg of TRH. It was found that after TRH injection without blood transfusion, MAP came up immediately and reached 10.9 kPa at 5th min after injection, 70%higher than 6. 4 kPa of the saline group(P<0. 001). LVP and±dp/dtmax began to rise at the 5th minute and reached the peak at the 30th minute. The effects of TRH could last more than 1. 5 hours. In the saline group, MAP, LVP and ±dp/dtmax decreased gradually. TRH exerted no effects on blood gases and the water content of the lungs and the brain. On the basis of the findings listed above, it is believed that TRH exerts therapeutic effects on traumatic-hemorrhagic shock at high altitude.

本实验在低压氧舱内模拟海拔4000m复制大鼠创伤失血性休克模型,以期探讨促甲状腺素释放激素(Thyrotropin-ReleasingHormone,TRH)可否治疗大鼠高原创伤失血性休克。研究结果表明TRH(5mg/kg,iv)具有明显的抗高原创伤失血性休克的作用,给药后,治疗组动物平均动脉压显著升高(P<0.001),LVP,±dp/dtmax明显改善。提示TRH可用于治疗高原休克。TRH对血气和肺脑含水量无明显影响。

Aim To investigate the importance of μ, δ and κ opioid receptors in the cardiovascular depression following traumatic hemorrhagic shock, elucidate the facts that the subclass of these opioid receptors is involved in the pathogenesis of traumatic hemorrhagic shock. Methods With a model of traumatic hemorrhagic shock rat, the relationship between the changes of myocardial and brain μ, δ and κ opioid receptors and cardiovascular functions and effects of δ and κ opioid receptor antagonists on hemodynamic parameters of traumatic hemorrhagic shock rats were observed. Results The number of myocardial and brain δ and κ opioid receptors significantly increased, and they were significantly associated with the decreased hemodynamic parameters such as mean arterial pressure (MAP), left intraventricular systolic pressure (LVSP), the maximal rate of the change of intraventricular pressure (±dp/dtmax) and the area of pdp/dt vector loop (Lo). However, μ opioid receptor of heart and brain did not obviously alter and δ opioid receptor antagonist ICI78, 864 and κ opioid receptor antagonist Norbinaltorphimine (50μg, icv) could significantly reverse those decreased hemodynamic parameters of traumatic hemorrhagic shock rats. Conclusion These results suggest that opioid receptors, especially δ and κ opioid receptors, are closely related to the pathogenesis of traumatic hemorrhagic shock, and they play an important role in the depression of cardiovascular function following traumatic hemorrhagic shock.

Objective: To investigate the effects of thyrotrophin releasing hormone (TRH) or 5% NaCl/2. 5% acetate sodium/dextran70 (HAD) alone and in combination on traumatic hemorrhagic shock in rats athigh altitude (LaSa, Tibet). The changes in hemodynamics and survival time of the rats were thenobserved. Conclusion: TRH or HAD alone and incombination can exert significant promoting effects on traumatic hemorrhagic shock at high altitude.

目的:在西藏拉萨进行现场实验,观察促甲状腺素释放激素(TRH)与5%NaCl/2.5%醋酸钠/6%右旋糖酐70(HAD)伍用对高原创伤失血性休克大鼠的治疗作用。方法:初进高原大鼠27只,分乳酸林格氏液对照组(8只),TRH治疗组(7只),HAD治疗组(6只)和TRH与HAD合用组(6只),大鼠右侧股骨粉碎性骨折加放血(维持动脉血压6.0hPa1h)复制创伤失血休克模型,观察TRH(5mg/kg),HAD(4ml/kg)和两者合用对创伤失血性休克大鼠血流动力学指标和动物存活时间的影响,以等容量乳酸林格氏液作对照。结果:TRH、HAD单用或合用均能显著提升创伤休克大鼠血压,改善其左室内压(LVSP),左室内压最大变化速率(±dp/dtmax),实测心肌最大收缩速度(Vpm),心肌收缩向量环面积(Lo),明显延长休克动物的存活时间,合用效果稍优于TRH或HAD单用效果。结论:TRH、HAD均具有较好的抗高原创伤失血休克作用,两药可联合用于高原创伤休克的早期救治。
