Expression of bcl 2 in well, moderately and poorly differentiated primary gallbladder carcinomas were 82.8%, 82.4% and 50.0% respectively with significant statistical difference ( P <0.01). The positive rate of bcl 2 in stages Ⅰ,Ⅱ and Ⅲ of primary gallbladder carcinomas were 81.3% and 55.2% respectively with no significant differnce. The result suggests that expression of bcl 2 in primary gallbladder carcinomas has no correlation with clinic stages.

为探讨抗凋亡基因bcl2与胆囊肿瘤发生、分化及临床预后的关系,采用免疫组织化学方法对45例原发性胆囊癌和39例胆囊腺瘤抗凋亡基因bcl2的表达进行了观察。结果显示:bcl2在原发性胆囊癌和胆囊腺瘤的表达率分别为71.1%和87.2%,两者比较无显著性差异(P>0.05);bcl2在原发性胆囊癌高、中、低分化型的表达率分别为82.3%、82.4%和50.0%,彼此间存在显著性差异(P<0.01);bcl2表达在原发性胆囊癌临床Ⅰ~Ⅱ期与Ⅲ期的阳性率分别为81.3%、65.3%,彼此间无显著性差异,表明bcl2表达可能与原发性胆囊癌临床分期无直接关系。

Objective:To investigate proliferative cell nuclear antigen(PCNA) expression and its significance in the malignant and benign gallbladder disease.Methods:The expression of PCNA was examined immunohistochemically in primary gallbladder carcinomas(PGC,35),adenoma and adenoid lesions of gallbladder(10) and chronic cholecystitis(10).Results:PCNA LI was significantly higher in PGC than in gallbladder benign disease (P<0.01).There was a tendency that the higher the PCNA LI,the lower differentiated of PGC(P<0.01),and the correlatin index was 0.3719 (P=0.0278).PCNA LI was significiently higher in matastsis (P<0.01).Patients whose tumor had a PCNA LI egual to or less than 43.84% had significiently longer survival rates than those with PCNA LI equal to or greater than 43.84%(P<0.01).Conclusion:PGC is associated with the high proliferative activity and PCNA is of important value in differentiative diagnosis,judgement of matastasis and prognosis PGC. 目的 :研究增殖细胞核抗原 (PCNA )在胆囊良恶性病变组织的表达及意义。方法 :应用 SP免疫组化法检测 35例原发性胆囊癌 ,10例胆囊腺瘤及腺瘤样病变组织 ,10例慢性胆囊炎中 PCNA表达结果。结果 :PCNA在原发性胆囊癌 ,胆囊腺瘤及腺瘤样病变组织和慢性胆囊炎中的阳性表达指数分别为 43.84%± 18.5 3%、6 .35 %±4.17%、4.5 8%± 3.5 8% ,原发性胆囊癌 PCNA L I明显高于胆囊良性病变组织 (P<0 .0 1)。胆囊癌分化程度越低 ,Nevin分期越高 ,PCNA L I有增高的趋势 (P<0 .0 1)。胆囊癌分化程度与 PCNA L I相关系数为 0 .3719(P=0 .0 2 78)。PCNA L I≤ 43.84%的胆囊癌患者 ,其生存率明显高于 PCNA L I>43.84%者 (P<0 .0 1)。结论 :原发性胆囊癌的发生、发展与细胞异常增殖有关 ,PCNA对胆囊癌的鉴别诊断、判断转移与否及预后有重要价值

The positive rate of protein DPC4(Smad4) in high degree of differentiation was higher than in low degree. It was pointed out that tumor suppressor gene DPC4(Smad4) took part in the process of the occurrence and development of carcinomas.

目的 :探讨抑癌基因DPC4 (Smad4 )蛋白在原发性胆囊癌中的表达及其意义。方法 :采用免疫组织化学方法检测 2 0例原发性胆囊癌抑癌基因DPC4 (Smad4 )蛋白的表达。结果 :胆囊癌肿瘤细胞DPC4 (Smad4 )蛋白的阳性反应率为 75 % (15 /2 0 ) ,慢性胆囊炎粘膜阳性反应率为 10 0 % (13/13) ,两者比较差异有显著性意义 (P <0 .0 5 )。结论 :DPC4 (Smad4 )蛋白的表达与肿瘤分化程度有一定关系 ,高分化胆囊腺癌中DPC4 (Smad4 )蛋白阳性表达率明显高于低分化腺癌 (P <0 .0 5 ) ,提示DPC4基因表达参与胆囊癌的发生、发展过程。
