OBJECTIVE To analyse and evaluate the risk factors, clinical characteristics and prevention counter measures of hospital fungal infection in patients with malignant tumors. METHODS To adopt investigation way to review and analyse the clinical data of hospital fungal infection in 251 cases from 2 160 tumor patients. RESULTS The infection rate was up to 11.6%. The locations of infection were mouth cavity and upper respiratory tract(39 4%), and then the lungs (23.1%); the key fungi were Candida albicans and C.krusei; the main susceptible factors were age, cancer stage, repeated radiotherapy or chemotherapy and unreasonable using antibiotics and hormones. The infection rate in patients with using immunopotentiators was lower than that without using them (P<0.05). CONCLUSIONS The key to reduction of hospital fungal infection after malignant tumors is to reduce susceptible factors, increase immunity and timely examine and treat the infection.

目的 分析并探讨恶性肿瘤患者并发深部真菌感染的危险因素、临床特点及防治对策。方法 采用回顾性调查方法 ,分析 2 5 1例恶性肿瘤患者并发医院真菌感染的临床资料。结果  2 16 0例恶性肿瘤患者并发医院真菌感染 2 5 1例 ,感染率占 11.6 % ;感染部位以口腔及上呼吸道为主占 39.4 % ;其次是肺占 2 3.1% ;真菌类型主要是白色念珠菌和克柔念珠菌 ;主要易感因素有患者的年龄、癌症的分期、反复放化疗、不合理使用抗生素以及应用肾上腺皮质激素 ,应用免疫增强剂的患者感染发生率明显低于未用组 (P <0 .0 5 )。结论 减少易感因素、提高机体免疫力、及时诊断和治疗是减少恶性肿瘤并发医院真菌感染的关键。

Objective To understand the relationship between in-hospital fungal infection and inpatients' recovery, and bring forward effective preventive and curative measure.Methods During 1994-2000, the sample were collected including 280 air particle samples in the ward collected quarterly and 242 samples of 13 kinds of inpatients' secretion, which were cultured in culture box with 25℃±1℃ for 4-7 days. And the total number of fungal colony and the fungal spectrum were determined.Results 8 species including 10112 strains were separated from 522 samples including 280 air particle samples, and the total detected rate was 62.50% and the detected rate for penicillium was 51.51%. The average exposure quantity was 275.62/m 3. 8 species including 1680 strains obtained from 13 kinds of secretion from patients' incision were consistent with those obtained from air particle samples in hospital, for which the detected rate was 61.57%. The infection rate of incision was the highest in obstetric ward, which was 87.50%, and penicillium was the main species which was significantly correlated with species obtained from hospital air. With the increasing of patients' duration in hospital and the duration for incision healing, the fungal infectious rate could reach 100%, and the fungal infectious spectrum changed.Conclusions Fungi is the important pathogen for inpatients' infection, which should be paid much attention.

目的 为了解医院内霉菌感染菌相与住院病人的康复因果关系 ,探讨防治对策。方法 自 1994~ 2 0 0 0年对台州地区医院采集一年四个季节病房内尘粒样本 ,2 42份住院病人 13种分泌物样本等放置于2 5℃± 1℃霉菌培养箱培养 4~ 7d ,进行霉菌菌落总数测定和霉菌菌相组成鉴定。并对该院一年四季霉菌感染暴露菌相与住院病人危险度进行研究。结果  5 2 2份样本 ,其中 2 80份尘粒样本 ,共分离到 8个霉菌菌相10 112株霉菌 ,总检出率为 62 5 0 % ;以青霉为主 ,占 5 1 5 1%。霉菌平均暴露量为 2 75 62 /m3。 2 42份 13种术后切口等样本分离到的 8个菌相 1680株霉菌 ,总检出率为 61 5 7% ;产科住院病人术后切口霉菌感染最高(87 5 0 % )。随病人住院时间、术后切口愈合持续时间的延长 ,霉菌感染率可达 10 0 % ,感染菌相分布也有所变化。结论 霉菌是医院感染的重要病原菌 ,应重视预防与控制。
