Objective: To evaluate the value of MRM in diagnosing the diseases of spinal canal and to study the practical technique.Materials and Methods:The MRM and conventional MRI findings in 71 patients with spinal canal diseases were analyzed. The cases were consisted of the following diseases: intramedullary tumors 8, intradural extramedullary tumors 14, extradural tumors 9, vertebral tuberculosis 3, spinal injuries 7, spinal dysraphism 6, intervertebral disc hernia and bony stenosis of the spinal canal 24 cases.Results:MRM could distinguish structures with minor differences in signal, which could be hardly distinguished by the conventional MRI. MRM 3D MIP reconstruction was superior to the conventional MRI in demonstrating the anatomical relationship between the lesions and the spinal canal. For demonstrating the solid structures of tumors and extradural lesions, MRM was inferior to the conventional MRI.Conclusion: Compared with other myelography, MRM has the advantages of nonivasive, easy to study and without any complications. The accuracy of MRM in diagnosing the spinal canal diseases can be further increased if combined with conventional scan sequence of MRI.

目的:探讨MRM诊断椎管病变的价值及MRM技术方法。材料与方法:对71例椎管病变的常规MRI检查及MRM表现进行了分析,其中髓内肿瘤8例,髓外硬膜内肿瘤14例,硬膜外肿瘤9例,结核3例,脊椎外伤7例,脊柱闭合不全6例,椎间盘突出及骨性椎管狭窄24例。结果:MRM可显示常规扫描不能显示的信号差别小的结构,MRM3DMIP重建显示椎管整体结构与病变的关系优于常规扫描,但对肿瘤的实性结构、硬膜外病变的显示则不如常规扫描序列。结论:MRM与其他脊髓造影方法相比具有无损伤、无并发症、操作灵活方便等特点,与常规扫描序列相结合可显著地提高椎管病变的诊断价值

目的 :介绍快速FLAIR技术在椎管内病变MRI中的应用并评价其价值。方法 :用 0 .5T超导型MR扫描仪在常规T1、T2 WI后对 44例椎管内病变患者进行快速FLAIR成像 ,TR :5 0 0 0~ 6 0 0 0ms ,TI :15 0 0~ 2 0 0 0ms ,TE :90~ 12 0ms。 35例在快速FLAIR成像后再行T1WI增强扫描。结果 :椎管内髓外肿瘤 2 6例 ,髓内肿瘤 4例 ,脊髓炎症 11例 ,脊髓局限性坏死软化 3例。快速FLAIR图像清楚地显示所有病变 ,而常规T2 加权像对 9例髓外硬膜下肿瘤及 8例脊髓炎显示不佳。结论 :快速FLAIR技术在椎管内病变MRI中具有较高的应用价值 ,是常规T2 加权像的必要补充。
