This article mainly studies the effects of gentamicin enhancing renal insult to acute ischemia in rats. Male rats were given gentamicin sulfate, 100 mg/kg/d, i.p. for 2 days. Then bilateral renal pedicle cross-clamping (15 min) was performed, and renal ischemia was produced. It was found that Scr, FENa, RFI, kidney weight increased and Ccr decreased significantly. Under the light microscope, renal convoluted tubular epithelium was highly cloudy swelling, vacuolar degeneration and widely necrosis....

Significant differences were not found in both groups of gentamicin and ischemia. These data suggest that gentamicin can contribute to the development of ischemic as well as nephrotoxic acute renal failure.

本文观察了庆大霉素促进大鼠急性缺血性肾脏损害的作用。给予雄性大鼠硫酸庆大霉素100mg/kg/d,共2天,然后双侧肾蒂夹闭15分钟,造成肾缺血,可使血清肌酐、肾衰指数,滤过钠排出分数和肾重显著升高,内生性肌酐清除率明显下降,光镜下见肾曲小管上皮细胞高度浊肿,空泡变性及比较广泛的坏死。而单独应用庆大霉素或单独缺血均未见有显著的肾功能异常。提示庆大霉素可以促进缺血性肾脏损害的发生。

本文观察了山莨菪碱(654—2)对免肾缺血再灌注损伤的保护作用。在夹闭双侧肾蒂1h再灌注24h的动物,肾功能和肾超微结构均出现显著的损害,肾静脉血中脂质过氧化物(LPO)含量显著升高。在缺血再灌注前1h,静脉注射654—2,肾脏损伤显著减轻,且静脉血LPO含量与缺血前相比无显著差异,但与生理盐水+夹闭对照组相比则有显著差异(P<0.05)。提示654—2可减轻肾缺血再灌注损伤和减少自由基引发的脂质过氧化反应。

Seventeen patients with staghorn calculi underwent nephrolithotomy during which renal pedicle was not blocked and obtained satisfactory results. It is suggested that this approach is easily to perform with clear surgical field and minimal blood loss.

报告采用不阻断肾蒂肾实质切开新术式治疗铸形肾结石17例,效果满意。认为该术式操作简单,视野清晰,出血少,易于推广。
